Why Your Cat Won’t Use the Litter Box: 8 Problems and Practical Fixes

A cat avoiding the litter box may be ill, in pain, stressed, or reacting to its setup. Learn what to check at home and when to seek veterinary help.

A cat that stops using the litter box may be ill or in pain—not misbehaving. Arrange a veterinary examination for new or persistent house-soiling, and seek emergency care if your cat repeatedly strains to urinate but produces little or no urine. Once urgent and medical causes are addressed, check the box, litter, location, access, and household stressors; a small, reversible change can help identify what is getting in the way.

First, check for a medical problem or emergency

Urinary inflammation and other lower urinary tract conditions can make urination painful or urgent. Digestive problems can affect defecation, while kidney disease, diabetes, and thyroid disease can increase drinking or urination. Pain and age-related changes can also make it hard to reach or use a box. A cat may not look obviously ill, so a new or persistent change in elimination warrants veterinary assessment rather than an assumption that the cat has chosen to misbehave. Cornell Feline Health Center’s house-soiling guidance explains that medical, environmental, and behavioral factors can all be involved.

Repeated straining with little or no urine is an emergency

Repeated attempts to urinate, especially when only a few drops or no urine appear, can signal lower urinary tract disease, including a blockage. Cornell states: “A urethral obstruction is a true medical emergency, and any cat suspected of suffering from this condition must receive immediate veterinary attention.” Do not wait to see whether changing the litter or box solves it. Contact a veterinarian or emergency clinic immediately if obstruction is suspected. Cornell’s overview of feline lower urinary tract disease describes the condition and its urgency.

Tell urine marking apart from ordinary accidents

Look at where and how the urine is deposited. Spraying commonly leaves a small amount on a vertical surface; the cat may stand with its tail raised or twitching. Ordinary elimination outside the box is more often found on a horizontal surface. The patterns can point to different causes, so note what you observe and tell your veterinarian. Marking may occur even when a cat still uses the litter box, and it calls for different troubleshooting from a cat that cannot or will not use the box for ordinary elimination. Eight litter box issues to check

1. A medical issue is being mistaken for a behavior problem

Pain, urgency, increased urine production, digestive trouble, or reduced mobility can change a cat’s elimination habits. Start with a veterinary examination for a new or ongoing problem. Setup adjustments may still help, but they should not substitute for checking whether illness or pain is involved.

2. The box is too small, covered, or difficult to enter

A cramped interior can be uncomfortable, particularly for a large cat. A cover or liner may also put a cat off. Try a spacious, uncovered box as a practical test. Kittens, older cats, and cats with limited mobility may find a low-entry box easier to use. These are options to try, not universal fixes; observe whether your cat actually prefers the change.

3. The litter’s smell, texture, or depth does not suit the cat

A sudden switch, strong fragrance, or unfamiliar texture can lead some cats to avoid the box. Cornell says most cats prefer unscented litter and recommends making brand changes gradually. ASPCA guidance says cats generally prefer clumping litter with a medium-to-fine texture, but individual preferences vary. If a change is needed, transition gradually or offer separate boxes with different litter choices so the cat can show what it prefers.

4. The box is dirty, or there are too few boxes

Some cats avoid a box that already contains urine or feces. Scoop at least once a day. ASPCA recommends one box per cat plus one extra; in a home with two cats, for example, that means three boxes. Its guidance also recommends washing boxes weekly with warm water and unscented soap, baking soda, or no soap, and replacing the litter. An enzymatic cleaner can help clean accident sites, but cleanup does not replace investigating why the accident happened.

5. The location feels unsafe or is hard to reach

A busy, loud, or awkward location may make a cat reluctant to use the box. Choose an accessible spot away from heavy foot traffic and food bowls. In a multi-story home, consider convenient access on each level the cat uses. A box should not be placed where another animal can corner or block the cat.

6. Household stress or conflict affects access

A new person or animal, a changed routine, or tension between cats can contribute to house-soiling. In multi-cat homes, distribute boxes and other resources so a cat can reach them without passing a competitor or getting trapped. If conflict or stress continues, ask your veterinarian about environmental changes or help from a qualified behavior professional.

7. Urine marking is mistaken for a litter-box failure

Small deposits on vertical surfaces, sometimes made while standing with the tail raised or twitching, may be marking rather than ordinary elimination outside the box. Medical issues should still be ruled out. Persistent marking may need a different environmental or behavioral plan than changing the box itself; discuss next steps with your veterinarian, and do not give medication without veterinary guidance.

8. Pain, age, or reduced mobility makes the setup harder

Arthritis and other painful or mobility-limiting conditions can make climbing over a high side or reaching a distant box difficult. Cognitive changes and age-associated illnesses that increase urine production may also play a part. Arrange a veterinary assessment and make access easier with a low-entry box or boxes on the levels your cat uses. Cornell’s guidance on senior and special-needs cats discusses age-related needs.

Make changes in a way that helps identify the problem

  1. Write down what is happening. Note whether the issue is urination, defecation, or both; whether urine lands on a vertical or horizontal surface; how often the cat tries; and whether anything changed at home. Share the observations with your veterinarian.
  2. Address urgent signs first. Repeated straining with little or no urine calls for immediate veterinary attention, not a home experiment.
  3. Keep the current arrangement while testing one manageable change. For example, add an uncovered box in an accessible location or offer a different litter in a separate box. Changing one factor at a time makes the cat’s preference easier to interpret.
  4. Review access and cleanliness. Scoop daily, provide enough boxes for the number of cats, and check whether a cat can reach a box without noise, obstacles, or conflict.
  5. Reassess with your veterinarian if the problem persists. Ongoing house-soiling may need further medical evaluation or tailored behavioral help.
