If your cat is pooping outside the litter box, arrange a veterinary assessment for a new or persistent change before treating it as a behavior problem. Constipation, diarrhea, pain, mobility limits, litter-box aversion, preferred surfaces, and stress can all play a role—and more than one cause may be present. The location of the stool alone cannot identify the cause.
What to do first when your cat poops outside the box
- Look for a change in health or stool. Note when the accidents began, how often they happen, stool consistency, straining or signs of discomfort, and changes in appetite, energy, or vomiting.
- Contact your veterinarian about a new or persistent pattern. Share your observations and let the veterinarian decide whether an examination or tests are appropriate. Do not assume the problem is behavioral based on where the stool appears.
- Seek prompt advice if diarrhea comes with other signs of illness. Cornell advises veterinary care when diarrhea lasts longer than a day or two and the cat also has signs such as poor appetite, lethargy, or vomiting. Some over-the-counter medicines can harm cats; do not give medication without veterinary direction.
- Review the box setup after health concerns are addressed. Check cleanliness, litter, size, entry height, location, and whether the cat can reach and leave the box comfortably.
- Track patterns and household changes. Record where and when accidents occur, which surface the cat chooses, and whether there has been a change in routine or conflict with another cat.
10 reasons cats poop outside the litter box
These are possible pathways, not ten separate diagnoses. Medical discomfort can overlap with box aversion or stress, and a habit can remain after its original trigger improves.
1. Constipation or painful defecation
Constipation involves infrequent or difficult passage of hard, dry stool. Straining or pain may make a cat avoid the box or seek a position or location that feels easier. A veterinarian can assess the cause; the accident itself does not establish that constipation is present.
2. Diarrhea, colitis, or bowel urgency
Digestive problems can make bowel movements frequent, urgent, painful, or difficult to control. A cat may not reach the box in time. Note how long diarrhea lasts and whether other signs of illness appear.

3. Another illness affecting elimination
Health problems can change stool frequency or make defecation uncomfortable. The stool’s location cannot distinguish among possible medical causes, so leave diagnosis and decisions about testing to a veterinarian.
4. Age-related mobility or cognitive changes
Stiffness, weakness, reduced senses, or cognitive changes may make it harder to find, enter, or use a box. A high-sided or enclosed box can add a barrier, particularly for a cat with limited mobility.

5. A painful association with the box
If passing stool hurt while the cat was using the box, the cat may come to associate the box with pain. Avoidance can continue even after the original medical problem improves, so addressing health alone may not immediately restore box use.
6. Litter aversion
A cat may dislike a litter’s texture or another feature. Preferences vary by cat; there is no single litter type that suits every cat. If health issues have been assessed, consider whether the cat’s choices suggest a litter preference.

7. An unpleasant or unsuitable box
A dirty or otherwise unsuitable box can contribute to avoidance. Consider whether it is kept clean and whether its size and design let the cat enter, turn around, and posture comfortably.
8. A difficult or intimidating location
A noisy, busy, intimidating, or hard-to-reach spot may discourage box use. Access can also be a problem in a multi-level home or for a cat whose mobility has changed.

9. A preferred surface or alternate site
Some cats repeatedly choose a particular surface or location. If the pattern continues, the alternate site may become a learned preference, even when the box remains available.
10. Stress or conflict with other cats
Changes in the household or routine can affect elimination. In a multi-cat home, another cat may intimidate the cat or control access to a box. A behavior can also persist after the initial stressor has passed.

How to make the litter box easier to use
Check access, size, and entry
For a cat with arthritis or declining mobility, VCA describes moving the box to an easier location and using a larger box with lower sides as possible accommodations. Compare the entry height and usable interior space with your cat’s ability to step in, turn, and posture. A box adjustment may help access, but it does not treat constipation, diarrhea, or another illness.
Review cleanliness, litter, and location
- Keep the box clean and observe whether the cat avoids it after it has been used.
- Consider whether the litter’s texture or other features may be unacceptable to this individual cat.
- Choose an accessible location away from high-traffic or intimidating areas.
- In a multi-level home, consider whether the cat can readily reach a box where it spends time.
Provide enough accessible boxes
ASPCA recommends at least one litter box per cat, plus one extra, in accessible locations away from busy or intimidating areas. Treat this as general guidance, then consider the layout and dynamics of your own home.
Look for stress and site preferences
Keep a simple record of accident locations and timing, chosen surfaces, household changes, and interactions with other cats. A consistent location or surface can help identify what setup needs attention, while a sudden change still warrants a health-first response.
When to get behavioral help
If medical problems have been treated or ruled out and the soiling continues, ask your veterinarian about a comprehensive behavioral history and treatment plan. A veterinary behaviorist can help with persistent cases. Do not punish your cat; before giving medication for a behavior problem, consult a veterinarian or veterinary behaviorist.
