Most cats recover from spay or neuter surgery without a serious problem, but anesthesia and surgery carry risks. Possible complications include breathing or circulation problems, bleeding, infection, pain, and incision damage. The signs that matter—and what is normal—depend partly on whether your cat had an abdominal spay or a scrotal neuter. Follow the operating clinic’s discharge instructions, and call it first if you are worried.
What complications can happen after a cat spay or neuter?
The nine items below are practical categories of possible complications and abnormal recovery signs, not a ranking of likelihood. Available guidance does not establish a rate for each one, and the risks are not equal. A veterinarian can assess your cat’s individual condition and surgical findings.

1. Anesthetic reaction or breathing and circulation problems
An unexpected reaction to anesthetic drugs or a problem affecting breathing or circulation can occur during surgery or recovery. Veterinary monitoring—including checks of heart rate, blood pressure, and oxygenation—helps detect problems. The Association of Shelter Veterinarians’ 2016 guidelines say that “Anesthesia-related death is most common during the postoperative recovery period, especially within the initial 3 hours after surgery.” This describes when such deaths are most likely to occur, not how often they happen.

2. Vomiting, regurgitation, or aspiration
A cat may vomit or regurgitate around anesthesia. Because anesthesia can interfere with normal swallowing and airway reflexes, material can enter the lungs and cause aspiration pneumonia, which VCA describes as potentially life-threatening. Follow the clinic’s own food and water instructions before surgery; fasting directions are not interchangeable between clinics.

3. Bleeding
Bleeding can happen during or after surgery. VCA notes that a cat spayed while in heat may have a slightly higher bleeding risk because blood flow to the reproductive tract increases; it does not give a rate. A small amount of intermittent seepage may occur during the first 24 hours in some incision cases, particularly after activity. Continuous bleeding or seepage that continues beyond 24 hours needs prompt veterinary advice.
4. Infection
An infection may affect the incision or occur internally. VCA describes postoperative infection as rare after castration, but redness, swelling, discharge, or a foul smell can be warning signs. Appearance alone cannot confirm an infection, so contact the clinic rather than trying to diagnose or treat it at home.

5. Incision swelling, bruising, or a growing lump
Mild redness or bruising can occur early in healing. The ASPCA’s postoperative instructions advise contacting its postoperative line if a bump or bruise seems to be growing, the site feels warm, redness is marked, or discharge appears. Your cat’s own veterinarian can determine whether a change is expected for that particular incision.
6. Incision opening or injury from licking and chewing
Jumping, rough play, licking, or chewing can strain or damage a healing wound. Wound trauma may also introduce infection. Contact the clinic if the incision appears to be opening or your cat has damaged it; do not apply creams, disinfectants, or other products unless the veterinarian directs you to.

7. Pain or distress that is not controlled
Pain-control needs vary with the operation, technique, age, and individual cat. If your cat seems persistently distressed or is repeatedly bothering the wound, ask the clinic whether pain management or the incision needs reassessment. Never give a human pain medicine unless a veterinarian explicitly directs you to: Cornell and the ASPCA warn that medicines such as aspirin, ibuprofen, and acetaminophen can be dangerous to cats.
8. Delayed or abnormal recovery from anesthesia
Some sleepiness, shivering, reduced appetite or drinking, and unsteady walking can occur after anesthesia, but the expected recovery depends on the cat and the clinic’s protocol. Call if these signs persist beyond the period your clinic described, worsen, or concern you. The ASPCA specifically flags an abnormal gait that lasts beyond 24 hours.

9. Other urgent recovery problems
Labored breathing, pale gums, marked weakness, vomiting, or diarrhea warrant immediate contact with the operating clinic or urgent veterinary care according to your discharge instructions. The ASPCA also advises contacting its postoperative line if a cat strains to urinate within 72 hours of surgery; use your own clinic’s number and emergency plan rather than assuming every practice uses the same protocol.

A female spay is an abdominal operation to remove reproductive organs; Cornell describes the incision as being closed in layers. Male castration removes the testicles through small scrotal incisions and is generally quicker and less complicated. Because the procedures and incision sites differ, an appearance that can be expected after one operation may not be normal after the other.

For example, the ASPCA’s male-cat protocol says one or two small openings on the scrotum can be expected, but that is not a universal rule for every clinic. Use the instructions you were given and ask the operating team if the site looks different from what they described.
What can be normal during early recovery?
- A slightly pink or mildly red incision, or some bruising, may occur early in healing.
- Some cats may not eat on the evening they return home, according to ASPCA discharge instructions. Follow your clinic’s guidance and ask for advice if appetite loss persists or comes with other concerning signs.
- A small amount of intermittent blood seepage in the first 24 hours can occur in some cases, especially after activity; the continuous or prolonged bleeding described above is different.
These are general observations, not a way to rule out a complication. If your cat’s condition worsens or you are unsure whether a change is expected, contact the clinic that performed the surgery.
How can you reduce incision problems at home?
- Restrict activity for the period your veterinarian specifies. VCA gives 7–14 days as general guidance for keeping a cat indoors and limiting activity after an incision; the ASPCA handout advises restricting jumping and play for seven days. These are different organizations’ instructions, not a universal schedule. Follow the operating clinic’s plan for your cat.
- Prevent licking and chewing. If your cat keeps trying to reach the incision, ask the veterinarian whether a properly fitted Elizabethan collar is appropriate. A recovery garment may be an option depending on the incision’s location and the veterinarian’s advice.
- Keep the incision dry and leave it untreated unless instructed. VCA advises against applying creams or disinfectants without veterinary direction. It also warns that hydrogen peroxide and alcohol can damage cells and delay healing.
- Use only veterinarian-approved medication. Do not substitute human or over-the-counter pain medicines for medication prescribed for your cat.
Who should you call if something seems wrong?
For a concern after surgery, start with the clinic that performed the operation: it knows the procedure, the discharge plan, and how to reach its after-hours service. For breathing difficulty, pale gums, severe weakness, or another rapidly worsening problem, follow the clinic’s emergency instructions or seek urgent veterinary care without delay. If you cannot reach the operating team, use a local emergency veterinary service.
