How to Help a Senior Dog Build Strength and Mobility Safely at Home

A safety-first guide to veterinary assessment, individualized low-impact activity, practical home adjustments, and monitoring mobility changes in senior dogs.

Help a senior dog move more comfortably by first finding out why movement has changed, then matching gentle activity and home adjustments to the dog’s needs. Stiffness, slipping, trouble rising, or avoiding stairs should not automatically be written off as aging: pain or a condition such as osteoarthritis may be involved. A veterinarian can assess the cause and, when appropriate, help shape a safe activity and rehabilitation plan.

Start with a veterinary assessment, not a workout

Changes in mobility can have several causes, and osteoarthritis (OA) is only one possibility. OA is a chronic, progressive joint disease that can affect dogs at any age. It involves cartilage loss and related joint changes that may cause pain, inflammation, and difficulty using a limb. Cornell Veterinary Medicine lists stiffness, gait changes, reduced activity, trouble with stairs or getting up, and muscle wasting among possible signs. Diagnosis involves a veterinary examination and may include imaging. OA has no cure, but pain and mobility can be managed with an individualized plan. Cornell’s osteoarthritis overview explains the condition and signs.

Arrange veterinary advice when a mobility change is new or worsening, or when you notice lameness, difficulty rising, reluctance to walk, behavior changes, or visible muscle loss. The veterinarian can assess for pain and other causes, discuss whether rehabilitation is appropriate, and advise what activity is suitable now. Don’t try to diagnose arthritis or compensate for possible pain by pushing a dog to exercise.

Choose activity for this dog, not a standard routine

For a dog whose veterinarian agrees that exercise is appropriate, the aim is controlled, low-impact movement suited to the dog’s current ability. Consistency and gradual adjustment matter more than intensity. The American College of Veterinary Surgeons describes controlled leash walks as an alternative to high-impact running or jumping and notes that consistent low-impact exercise can help build muscles around joints. Cornell also names leash walks and swimming as examples. Neither example is right for every dog, condition, or setting. ACVS guidance on osteoarthritis in dogs discusses exercise alongside veterinary care.

There is no universal safe number of repetitions, minutes, stair trips, or sit-to-stand exercises for a senior dog. A plan should reflect the dog’s impairments and progress according to its response and ability to manage each exercise. The 2022 review of home exercises and lifestyle changes emphasizes individualization; the COAST international OA consensus also supports professional guidance for an appropriate program. The 2022 review’s PubMed record describes this individualized approach.

The COAST Development Group’s 2023 consensus guideline recommends supervised in-clinic and at-home exercises for strengthening, toning, and improving mobility in dogs with osteoarthritis. That recommendation is for OA management; it is not a direction for every older dog to begin unsupervised exercises. The guideline advises regular, age- and breed-appropriate activity, minimizing high-impact or sudden loading—particularly for unconditioned dogs—and tailoring rehabilitation to the patient. Keep activity controlled and follow the plan you make with the veterinary team. Avoid suddenly increasing workload, high-impact activity, or strenuous exercise in a dog that is not conditioned for it. If movement seems more difficult or the dog deteriorates, stop escalating the activity and contact the veterinarian rather than adding a new drill.

Make the home easier to navigate

Home adjustments can reduce specific obstacles, but they are not treatments for arthritis and cannot guarantee a fall or injury will be prevented. Match each aid to a problem you observe, and consider the dog’s size, ability, and the layout of the home.

Slipping on a hard floor Place nonslip rugs or dog traction mats along the dog’s usual routes. Make sure each mat stays flat and stable; a shifting mat can create another obstacle.
Difficulty with a particular height change, such as getting into a car Consider a dog mobility ramp for that access point. Choose a location and setup the dog can approach and use; ask a veterinarian or rehabilitation professional if suitability is uncertain.
A larger dog needs help being lifted A fitted padded support harness may assist with lifting. Fit, safe handling, the dog’s comfort, and the caregiver’s ability all matter. Get professional advice on selection and use when needed.

Also check the routes your dog uses for hazards, including stairs and access to pools. A ramp or harness should address a defined access or lifting need; neither replaces diagnosis or a tailored care plan.

Coordinate home care with pain management and follow-up

For dogs with OA or chronic musculoskeletal pain, management may combine rehabilitation, appropriate activity, weight management, environmental changes, and veterinarian-directed pain care. The 2023 AAHA Senior Care Guidelines discuss early, multimodal pain and mobility management, rehabilitation, environmental modification, and monitoring. What is appropriate depends on the individual dog. AAHA’s senior-care pain-management guidance outlines this approach.

Do not start, stop, or change the dose of pain medicine or supplements without the veterinarian’s direction. The veterinarian needs to consider the dog’s health, possible contraindications, monitoring needs, and interactions. Mats, ramps, supplements, and exercise do not reverse arthritis, and home activity does not replace pain care or follow-up.

Track changes and share them with the veterinary team

Make brief notes about useful daily functions: how easily your dog rises, walks on its usual routes, handles a relevant access point, and behaves during ordinary interactions. Short videos of movement and daily activities can help the veterinary team assess changes and response to treatment; AAHA specifically notes the value of caregiver videos.

Keep veterinary rechecks in the plan when treatment begins or changes. COAST recommends re-evaluation after new treatments and when deterioration is reported. Share new difficulty, worsening function, or concerns about an exercise or home aid rather than waiting for the next routine visit.
