When Do You Need Rabies Shots After a Dog Bite?

After a dog bite, wash the wound immediately and contact a clinician and local or state health department. Whether rabies shots are needed depends on the exposure, the dog, and the local risk.

You may need rabies shots after a dog bite if a clinician and local or state health department determine that the exposure could have transmitted rabies. Wash the wound thoroughly with soap and water immediately, then contact them promptly for a risk assessment. The decision depends on where the bite happened, the dog’s health and availability for observation or testing, and the circumstances of the bite—not vaccination status alone.

What to do immediately after a dog bite

  1. Wash the wound right away. Thoroughly flush it with soap and water. If available, povidone-iodine solution may be used as an additional wound-irrigation measure, but it does not replace washing or medical evaluation. CDC wound-care and PEP guidance.
  2. Get medical advice promptly. Contact a clinician and your local or state health department. A bite also needs assessment for physical injury and bacterial infection, separate from the rabies question.
  3. Share the relevant details. Tell them where and when the bite occurred, what happened, whether the dog appears healthy, whether it can be reliably observed or tested, and whether you have ever received rabies vaccination.

Do not wait for symptoms before seeking advice. Rabies risk assessment is specific to the exposure and location; a health official familiar with rabies should guide the decision. CDC clinical PEP guidance.

How the dog’s health and availability affect the decision

In the United States, a healthy dog, cat, or ferret that bites someone may be confined and observed for 10 days in coordination with public health. This applies even if the animal is vaccinated. If the animal becomes ill during that period, notify public health promptly. CDC guidance for veterinarians.

The 10-day observation period is not a reason to wait without contacting public health. Officials consider whether the animal can be reliably observed or tested, its health and species, the bite circumstances, and rabies risk in the area. What if the dog is vaccinated?

Vaccination is relevant information, but it does not by itself settle whether you need PEP. CDC guidance still calls for 10-day observation of a healthy biting dog, even when vaccinated. What changes if the bite happened outside the United States?

Rabies risk and access to treatment vary by country. CDC Yellow Book guidance says that qualifying mammal bites or scratches in countries where rabies risk is elevated should lead to PEP when the animal is unavailable for observation. If the animal remains healthy for 10 days after the exposure, it is considered not to have been infectious at the time of the bite. What rabies shots involve in the United States

Not previously vaccinated Wound cleansing, one dose of human rabies immune globulin (HRIG) at the start of PEP, and rabies vaccine on days 0, 3, 7, and 14. HRIG is not given in the same syringe or anatomical site as the first vaccine dose.
Previously vaccinated Rabies vaccine on days 0 and 3; HRIG is not given.
Immunocompromised A fifth vaccine dose on day 28 may be required; the treating clinician determines the appropriate schedule.

How long can you wait to get rabies shots?

Contact a clinician or public health authority even if time has passed. CDC says PEP can be recommended regardless of the interval since exposure if the person has not developed signs consistent with rabies. Do not wait for symptoms to appear. CDC clinical PEP guidance.

If you have started PEP and a dose is delayed or missed, ask your clinician or public health authority what to do next. CDC’s PEP calculator says short individual-dose delays may be unimportant, while longer lapses may have unpredictable or negative outcomes.
