A veterinarian may consider it when a cat has heart failure or low cardiac output and imaging does not show clinically relevant left ventricular outflow tract obstruction (LVOTO). Whether it is appropriate depends on the cat’s diagnosis and echocardiogram—not simply a murmur or a cardiomyopathy diagnosis.
What is pimobendan for cats?
Pimobendan is a heart medication with positive inotropic and vasodilating effects: it can strengthen cardiac contraction and relax blood vessels. The U.S. Vetmedin product label describes increased calcium sensitivity in heart muscle and inhibition of phosphodiesterase type III as its mechanisms, but that prescription-animal-drug label is for dogs, not cats. DailyMed’s Vetmedin label, updated April 22, 2026, lists canine congestive-heart-failure indications; it is not a feline approval or dosing label.
When might a veterinarian consider it?
The American College of Veterinary Internal Medicine (ACVIM) consensus says pimobendan can be considered in cats without clinically relevant LVOTO, while rating the supporting evidence as low. The 2020 ACVIM consensus also says it may be considered in acute heart failure when signs suggest low cardiac output—such as hypotension, hypothermia, or bradycardia—if dynamic LVOTO is absent.
LVOTO means an obstruction to blood leaving the left ventricle. Because pimobendan can affect cardiac contraction and blood flow, the presence and clinical importance of an obstruction are relevant to the treatment decision. A veterinarian may use echocardiography and the cat’s clinical condition to assess this; owners should not infer suitability from symptoms or a diagnosis name alone.

The consensus reported no studies of pimobendan use in cats with subclinical cardiomyopathy. That means the available evidence does not establish a reason to give it routinely to a cat merely because a murmur, screening scan, or cardiomyopathy diagnosis is present. Merck Veterinary Manual says the medicine is not approved for cats and that routine feline use is probably not warranted, while noting possible trial use when a cat no longer responds to conventional heart-failure drug treatment. Merck’s heart-failure reference reflects a cautious view; decisions remain case-specific as clinical practice and evidence evolve.
What is the dosage of pimobendan for cats?
The ACVIM consensus gives a commonly used reference range of 0.625 to 1.25 mg per cat by mouth every 12 hours for selected cases without clinically relevant LVOTO. This is a published guideline range with low-level evidence, not a dose for owners to calculate or start themselves.

A 2025 practical evidence review describes 1.25 mg by mouth every 12 hours as the most clinically utilized regimen in the practice it reviewed, estimating that it is about 0.25 mg/kg for an average-sized cat. This is the review’s summary, not a universal dosing standard. The treating veterinarian must determine the individual prescription based on the cat’s diagnosis, body size, echocardiographic findings, other medications, and response. Follow the label and instructions provided by that veterinarian; do not transfer a dog dose or alter a cat’s dose based on this article. The 2025 review also emphasizes that evidence remains limited.
What are pimobendan side effects in cats?
In a retrospective review of records for 260 cats with congestive heart failure, investigators judged possible pimobendan-related adverse effects to have occurred in 12 cats (4.6%). Reported events included reduced appetite or anorexia, vomiting, and subclinical hypotension. Pimobendan was discontinued because of suspected adverse effects in 4 of 260 cats (1.5%). These figures come from the reviewed clinical records, not a controlled trial: they do not prove the medication caused each event or predict an individual cat’s risk. Ward et al.’s 2020 retrospective study provides the cohort findings.

Contact the prescribing veterinarian if a new or worsening symptom appears after treatment begins. Severe breathing difficulty, collapse, or sudden hind-limb weakness or paralysis requires urgent veterinary attention; these signs can indicate serious heart disease or another complication and are not necessarily a medication reaction.

Available evidence supports carefully selected, veterinarian-directed use—not a general claim that pimobendan is safe or effective for every cat with heart disease. The ACVIM recommendation is based on low-level evidence, the reported safety cohort was retrospective, and the 2025 practical review calls for adequately powered randomized controlled studies using practical clinical outcomes. The evidence therefore does not establish that pimobendan prolongs survival or benefits all feline cardiomyopathies.

Before prescribing, the veterinarian can weigh the expected benefit against risks in light of the cat’s heart imaging and clinical state, including whether clinically relevant LVOTO is present. Follow-up lets the care team assess response and possible adverse effects. Keep giving, changing, or stopping the medicine only as the prescribing veterinarian directs.
