A dog can live happily with a permanently lopsided expression, but a newly drooping lip or ear may signal facial nerve problems and should be assessed by a veterinarian. The appearance alone does not establish a diagnosis: look for changes such as trouble blinking, drooling, or food and water falling from one side of the mouth.
What a lopsided smile can mean
What looks like a smile may reflect reduced movement in the muscles of the face. The facial nerve controls movement of the eyelids, lips, ears, and other facial muscles; when it is affected, one or both sides of the face may move less normally. Veterinarians describe this as facial paresis (partial weakness) or facial paralysis (loss of movement).
A lopsided appearance by itself is not enough to tell whether a dog has facial nerve dysfunction. Some dogs have an unusual expression without an established medical diagnosis. A sudden change, especially alongside problems with blinking, eating, or drinking, deserves veterinary attention.
Signs to watch for
- A drooping lip or ear, often on one side.
- Difficulty blinking or keeping one eye closed.
- Drooling, or food or water spilling from the mouth.
- Reduced tears or an eye that appears dry or irritated.
Reduced blinking or tear production can leave the eye vulnerable to dryness or injury. Note when the change began and whether it is getting worse; that history can help the veterinarian assess the problem.

Possible causes and veterinary assessment
Facial weakness has several possible causes, and the right evaluation depends on the dog’s findings. Merck Veterinary Manual lists trauma, inner ear infection, hypothyroidism, tumors, and unknown causes. VCA Animal Hospitals also lists ear inflammation or infection, injury, generalized nerve disease, immune-mediated disease, and complications following surgery; cancer is described as a rare cause. Many cases are considered idiopathic, meaning no cause is identified, but that label should follow veterinary evaluation rather than be inferred from appearance.
The veterinarian will take a history and perform a physical examination. Depending on the findings, assessment may include an ear examination, blood or urine tests, tear testing, cultures, or imaging such as X-rays, CT, or MRI. Not every dog needs every test; advanced imaging may be needed to investigate some suspected causes.

Sudden jaw weakness is a separate problem from facial nerve paralysis, although it can also make eating difficult. Cornell advises prompt assessment because different conditions can produce similar signs. Tell the veterinarian if the jaw itself seems weak or cannot close, rather than assuming it is part of a lopsided smile.
Care depends on the problem and its effects
Treatment, if needed, depends on whether an underlying cause is found. Supportive care may also be recommended to protect the eye or make eating and drinking easier; it does not treat the nerve condition itself.

- Eye protection: If blinking or tear production is reduced, a veterinarian may recommend lubricating eye drops. Use eye products only as directed by the veterinarian.
- Food and water: If facial weakness makes eating or drinking difficult, soft food or a special water container may help. Ask the veterinarian what is appropriate for your dog.
- Follow-up: The veterinarian can monitor the eye and facial function and advise whether further evaluation or ongoing support is needed.
Recovery and quality of life
Recovery varies. Some dogs improve over weeks or months, while others improve only partially or remain affected. Merck Veterinary Manual notes that idiopathic facial paralysis can be temporary or permanent. It also says, “Permanent paralysis may give the face an unusual appearance but does not usually affect the quality of life in dogs.” This statement is from Thomas Schubert, DVM, DACVIM, DABVP, in the manual’s pet-owner article, last updated September 2024.
Merck adds that if there has been no improvement after six months, the chance of recovery is poor. That is a clinical prognosis threshold in its guidance, not a prediction for an individual dog; your veterinarian is best placed to discuss the outlook in light of the cause and your dog’s progress.
