For many cats with chronic kidney disease (CKD), a veterinarian-selected therapeutic renal diet is the usual starting point. These foods are formulated to restrict phosphorus and adjust protein and other nutrients, but the right choice depends on your cat’s CKD stage, appetite, weight and muscle condition, other health problems, and follow-up bloodwork. Wet and dry renal foods are available; the best option is one your cat will reliably eat.
Why a therapeutic renal diet is usually recommended
Dietary management is a mainstay of feline CKD care, according to International Renal Interest Society (IRIS) recommendations. Cornell University College of Veterinary Medicine explains that therapeutic kidney diets restrict protein, phosphorus, and sodium and may help prolong life and improve quality of life. Your veterinarian should select a food for your cat’s individual condition rather than relying on a brand’s marketing or the words “low protein” alone.
Why phosphorus matters
Reducing dietary phosphorus is a central goal of renal diets. The 2026 International Cat Care consensus guideline reports typical phosphorus levels of approximately 80–130 mg per 100 kcal in renal diets, compared with as much as 400–600 mg per 100 kcal in non-renal diets. These are guideline-reported ranges, not universal label cutoffs or a target that applies identically to every cat. Your veterinarian can interpret food phosphorus alongside your cat’s blood phosphorus results and clinical status.
If blood phosphorus remains above the veterinarian’s target despite diet, IRIS discusses adding a phosphate binder and monitoring the response. Binders need veterinary direction; do not add one or choose a dose on your own.

How protein should be handled
Renal foods modify protein, but protein should not simply be eliminated. The appropriate balance depends on CKD stage, appetite, body weight and muscle condition, and other diseases. IRIS notes uncertainty about dietary recommendations for cats at stage 1, reinforcing the need to individualize the plan and adjust it as the cat’s condition changes.
When comparing foods your veterinarian considers suitable, look at protein and calorie density in the context of your cat’s nutritional condition. A food that is too difficult to eat or does not provide adequate intake can create a different problem from the one the diet is intended to address.

Wet or dry kidney food?
Both wet and dry therapeutic renal diets are available, according to Cornell Feline Health Center. Canned or other moist food contributes more water through meals, which may aid hydration; the 2026 iCatCare guideline highlights this advantage. Wet food does not replace a veterinarian’s hydration plan, and a cat that eats dry renal food consistently may do better on that than on wet food it refuses.
Consider moisture and format together with phosphorus per calorie, protein and calorie density, and your cat’s willingness to eat the food. If two or more diets are clinically suitable, ask your veterinarian how each fits your cat’s lab results and nutritional needs.

What if your cat refuses the renal diet?
Protecting food intake matters. A renal diet is not helpful if the transition leads your cat to eat too little. The 2026 iCatCare consensus guideline cautions against introducing a new food during illness or diagnostic investigation because the experience may create food aversion and make the food harder to accept later.
- Ask your veterinarian about timing if your cat is acutely unwell or undergoing diagnostic tests.
- If your cat will not eat the proposed food, or is eating less or losing weight, contact your veterinarian promptly rather than insisting on the transition.
- Discuss another suitable format or food and how to monitor appetite, body weight, hydration, and laboratory results.
Cornell summarizes studies in which cats with CKD eating prescription kidney diets lived two to three times longer than cats eating standard commercial cat food. That comparison is a study-based summary, not a guarantee for an individual cat; Cornell’s page does not identify the individual study or its publication date.

What to ask your veterinarian
- Which therapeutic renal diets suit my cat’s current stage, other conditions, and nutritional status?
- What phosphorus target are we aiming for, and how will we know whether diet is meeting it?
- Should moisture, calories, protein, or food format be a priority for my cat?
- When should we reassess appetite, weight, hydration, and bloodwork?
- If phosphorus remains above target, would a phosphate binder be appropriate?
