What to Ask Your Veterinarian Before Starting a New CKD Treatment

Ask what a new CKD treatment targets, why it fits your pet, what alternatives and risks apply, and how your veterinarian will monitor its effects.

Before your cat or dog starts a new chronic kidney disease (CKD) treatment, ask what specific problem it is meant to address, why it fits your pet, what risks or alternatives apply, and how you will know whether it is working. Confirm the diagnosis and stage first, then leave with a written plan for monitoring and for what to do if your pet’s condition changes.

Start by confirming the diagnosis and stage

CKD treatment is not one universal regimen. The appropriate plan depends on whether the patient is a cat or dog, the stage and substages of disease, test results, appetite and nutritional status, other health conditions, and which treatments are authorized where you live.

Ask your veterinarian:

  • Which test results support CKD, and was the diagnosis confirmed while my pet was stable?
  • What is my pet’s IRIS stage and substage, and which results determined them?
  • How did creatinine and SDMA inform the assessment of kidney function? Were urine protein and blood pressure assessed for substaging?

IRIS bases initial staging on fasting creatinine measured on at least two occasions in a stable patient, then substages using proteinuria and systemic blood pressure. It also recommends using creatinine with SDMA to assess renal excretory function. Read the IRIS Guidelines for its current staging and treatment documents.

Clarify the treatment’s target and expected benefit

A new treatment may address a complication of CKD rather than reverse the underlying disease. Ask what measurable finding prompted the recommendation and what outcome would count as success for your pet.

  • Is the goal to manage blood pressure or urine protein?
  • Is it intended to address phosphate, anemia, potassium, or acid-base balance?
  • Is the main concern nausea, appetite, hydration, or another clinical sign?
  • What change should we expect, and over what period will we assess it?

Blood pressure, urinary protein loss, anemia, phosphate, potassium, acid-base balance, appetite, nausea, and hydration can all matter, but not every pet needs treatment for each. For any proposed adjunct, ask what finding justifies it and how strong the evidence is for a pet like yours. The Cornell Feline Health Center’s CKD overview describes complications that may be addressed as part of feline care.

Check that the option fits your pet and location

Ask why this treatment is appropriate for your pet’s species, stage, laboratory findings, and concurrent conditions. IRIS describes its recommendations as starting points: treatments should be individualized, monitored over time, and adapted to response. Some suggested treatments may not be authorized for cats or dogs in every location, so ask whether the proposed use is approved for your species where you live or is empirical or off-label.

Before agreeing, discuss material risks, contraindications, and interactions with your pet’s other medications or conditions. Ask what alternatives are reasonable if the first option is unsuitable, unaffordable, difficult to administer, or poorly tolerated. Where several approaches are possible, compare their target and supporting evidence, suitability for your pet, risks and interactions, monitoring demands, practical feasibility, cost, and local availability—not just the product name.

Discuss food, phosphate, and maintaining nutrition

A renal diet can be an important part of CKD management, but the right food and transition depend on the patient’s stage, appetite, preferences, and nutritional condition. IRIS advises considering renal diets for cats at Stage 2 and recommends them at Stages 3 and 4, with close nutritional monitoring. For cats: introduce a new diet gradually

The 2026 iCatCare consensus guideline recommends offering small amounts of a new food alongside the existing food over at least 1–2 weeks. Ask how to adjust that transition if your cat refuses the food or eats less, and how appetite, calories, weight, and body condition will be followed. Adequate intake matters; do not let a diet change silently reduce the amount your cat eats.

Ask how phosphate will be managed

For cats, the 2026 iCatCare guideline identifies a phosphate-restricted renal diet as initial management. If serum phosphate remains above the stage target after 4–6 weeks, the veterinarian may consider a phosphate binder and monitor the response. For dogs, the cited IRIS treatment document sets out stage-dependent phosphate management and monitoring. Ask which target applies to your pet and when phosphate will be rechecked; do not choose or dose a binder yourself.

For either species, ask what to do if your pet will not eat the recommended diet or cannot take a medication. Discuss acceptable alternatives and a plan to preserve calories and body weight rather than pressing on with an approach that undermines intake.

Agree on monitoring before treatment begins

Monitoring is part of the treatment, not an optional afterthought. Ask the veterinarian to specify what will be checked, when the first recheck should happen, and which result would lead to a dose adjustment, a different option, or stopping treatment.

  • Blood and urine tests, including the measures relevant to the treatment target
  • Blood pressure, if it is relevant to the diagnosis or plan
  • Weight, appetite, food intake, and body condition
  • Side effects and any interactions or changes in other conditions

IRIS recommends serial monitoring and adapting treatment to the patient’s response; the appropriate schedule is specific to the pet and treatment, so get the timing from your veterinarian. Current IRIS materials identify 2026 revisions, including anemia thresholds and options such as HIF-PH inhibitors; these are veterinary guideline matters, not instructions for owners to start treatment. Ask which current guidance applies to your pet rather than relying on a threshold you found in isolation. See the current IRIS Guidelines.

Get clear instructions for side effects and urgent changes

Before leaving, ask which changes should prompt a routine call, a same-day assessment, or emergency care, and what to do after hours. Request instructions specific to your pet and the proposed medication or diet. Also ask whom to contact if a dose is missed, your pet vomits after a dose, refuses food, or develops a suspected side effect. The right response can depend on the treatment and the patient, so do not assume a generic online list applies.

Plan for practicalities and cost

Ask for an estimate covering the treatment and its expected monitoring, and whether local availability or insurance coverage affects the options. The clinic can provide patient-specific costs; coverage and availability need to be checked for your location and policy.

Before the appointment ends, make sure you have the treatment instructions, recheck timing, contact details, and a written note of the changes that warrant a call. The veterinarian should remain the decision-maker as results and your pet’s response emerge.
