If your dog may have it, contact a veterinarian or emergency clinic immediately; do not wait for discharge or try to monitor the illness at home. The infection can occur with or without visible vaginal discharge, and prompt veterinary treatment is important.
What is pyometra in dogs?
Pyometra is an infection in the uterus that can make a dog severely ill. It most commonly develops in an intact female after a heat cycle. Hormonal changes during the period after heat can affect the uterine lining and its defenses, allowing bacteria to multiply. The timing varies: UC Davis describes a typical interval of two to ten weeks after a dog comes out of heat, but timing alone cannot confirm pyometra. UC Davis Veterinary Medicine and Cornell University College of Veterinary Medicine explain the condition and its signs.
What are the symptoms of pyometra in dogs?
Possible signs include:
- Vaginal discharge that may be cream-colored or bloody
- Lethargy or weakness
- Poor appetite
- Drinking and urinating more than usual
- A swollen or painful abdomen
- Vomiting or fever
- Pale gums or collapse
Open-cervix and closed-cervix pyometra
With open-cervix pyometra, infected material can drain through the vagina, so discharge may be visible. With closed-cervix pyometra, the cervix is closed and discharge may be absent even as the dog becomes very ill. A lack of discharge does not rule out the disease. If your dog has concerning signs—especially after a recent heat—seek urgent veterinary care rather than waiting for symptoms to worsen.
How is pyometra diagnosed?
A veterinarian considers the dog’s reproductive history and physical examination findings, including any discharge or abdominal changes. Ultrasound or radiographs may show an enlarged, fluid-filled uterus. Blood tests and urinalysis help assess the dog’s condition and the severity of illness; vaginal cytology may also be used in some cases. The veterinarian interprets these findings together and may use testing to assess the dog’s readiness for anesthesia and surgery. No single sign or test that an owner can perform at home establishes the diagnosis.

How is pyometra treated?
Most dogs need veterinary stabilization followed by ovariohysterectomy, an operation that removes the uterus and ovaries. The veterinary team may provide supportive care and intensive monitoring depending on how sick the dog is. Emergency surgery can be more complicated than an elective spay, but it removes the infected uterus and prevents pyometra from recurring in that uterus.
Surgery: the usual treatment
For most patients—particularly dogs that are critically ill or are not intended for breeding—surgery is the usual approach. The American College of Veterinary Surgeons (ACVS) states: “The prognosis for survival with ovariohysterectomy can be as high as 80–100% if abdominal contamination is avoided and the systemic infection is treated and responds appropriately.” This is a conditional estimate, not a guarantee for every dog. Medical management: only for selected cases
Medical treatment may be considered for a relatively stable dog with important breeding value when preserving fertility is a priority. It takes days, requires close veterinary monitoring and antibiotics, and can fail or cause adverse effects. The infection can recur, and uterine rupture is a concern. Under Merck Veterinary Manual guidance, medical management is not indicated for systemically ill or endotoxemic patients. Merck advises emergency ovariohysterectomy if a dog worsens or does not show major improvement within 48 hours of medical management. This is veterinary clinical guidance—not a home-treatment plan. Ask the treating veterinarian whether this option is appropriate; it is not a safe substitute for urgent assessment.
Can a dog survive pyometra?
Many dogs can survive with prompt veterinary care, but prognosis depends on the dog’s condition and response to treatment. The ACVS estimate above applies only under its stated conditions: abdominal contamination is avoided, and systemic infection is treated and responds appropriately. A seriously ill dog may need intensive treatment, and complications can worsen the prognosis.

What does recovery after pyometra surgery involve?
Follow the treating veterinary team’s discharge instructions, give prescribed antibiotics as directed, and protect the incision from licking or other self-trauma. The surgeon can advise whether a recovery collar or another protective option is suitable and how it should fit.
ACVS describes recovery care as generally similar to routine spay recovery and advises restricting normal play, stairs, and off-leash activity for the first two weeks. The individual surgeon’s instructions take precedence. ACVS recovery guidance provides additional details.

Can pyometra happen in a spayed dog?
Spaying before infection develops prevents ordinary pyometra. A rare infection involving uterine tissue left behind after spay—called stump pyometra—is possible, but the term is sometimes confused with a stump granuloma, which may not involve bacterial infection. If a spayed dog has relevant signs, a veterinarian can investigate; ultrasound may help distinguish stump conditions. Treatment for stump pyometra includes antibiotics and surgery to remove infected tissue and any remaining ovarian material when present. See UC Davis Veterinary Medicine’s discussion of pyometra.
