Open pyometra can cause bloody or pus-like vaginal discharge; closed pyometra may cause no discharge at all. Both are life-threatening uterine infections and need prompt veterinary care. No discharge does not rule out pyometra.
How open and closed pyometra differ
Pyometra is an infection of the uterus in an unspayed female dog. The difference between the two forms is whether the cervix—the opening between the uterus and vagina—allows infected material to drain. The American College of Veterinary Surgeons (ACVS), VCA Animal Hospitals, and the MSD Veterinary Manual describe both forms as serious illness.

Signs to watch for in either form
Discharge is only one possible clue. Dogs with either open or closed pyometra may become lethargic or depressed, eat less, drink and urinate more than usual, or vomit. Fever may occur. Closed pyometra can also cause abdominal enlargement or pain, but symptoms vary and cannot establish the diagnosis at home.
- Bloody or pus-like vaginal discharge, especially in an unspayed dog.
- Unusual tiredness, listlessness, or reduced appetite.
- Increased thirst and urination.
- Vomiting or diarrhea.
- A swollen or painful-looking abdomen, particularly when there is no discharge.
When pyometra is a concern
Pyometra affects sexually intact female dogs and is more common in older dogs. It often develops in the weeks after a heat cycle. VCA says it typically occurs four to six weeks after heat, though it can occur from two to sixteen weeks afterward. Hormone medications can also contribute to risk. Recent heat is useful context, but timing alone can neither confirm nor rule out pyometra.

What to do if you suspect pyometra
Contact your veterinarian or an emergency veterinary clinic promptly if your dog has possible pyometra signs. Do not wait for discharge to appear: closed pyometra may have none, and both forms can lead to severe infection. The ACVS describes pyometra as a medical emergency requiring rapid intervention to prevent overwhelming infection and death.
How a veterinarian diagnoses pyometra
A veterinarian considers the dog’s history and physical examination alongside tests. Evaluation may include blood tests, urinalysis, and abdominal imaging. Ultrasound can reveal an enlarged, fluid-filled uterus and help distinguish it from pregnancy. Radiographs may show an enlarged uterus, particularly when the cervix is closed; with open drainage, enlargement can be less apparent. No single observation at home can reliably distinguish the forms or rule the disease out.

Treatment and recovery
Veterinary care usually begins with stabilization, followed by surgery to remove the ovaries and uterus (ovariohysterectomy). The ACVS identifies this as the treatment of choice for most pets. It removes the infected uterus and minimizes the risk of recurrence. ACVS reports survival of 80–100% after surgery when abdominal contamination is avoided and systemic infection is treated and responds appropriately; that conditional range is not an individual prognosis or a guarantee.
Medical treatment may be considered for carefully selected breeding dogs, but it is not a safe home alternative. It can take days to work, may fail, and recurrence is possible. With a closed cervix in particular, treatment carries risks including uterine rupture and peritonitis. The veterinary team must weigh these risks for the individual dog. After surgery, follow the veterinarian’s incision-care instructions; ACVS advises protecting the incision from self-trauma during the first two weeks, including with a recovery cone if the veterinarian recommends one.
