There is no one-size-fits-all L-carnitine dose for dogs. A UC Davis Stern Lab document gives a dose for dogs with dilated cardiomyopathy (DCM), but that disease-specific protocol is not a general wellness dose. Your veterinarian should decide whether your dog needs L-carnitine and, if so, prescribe the amount.
What L-carnitine dose is reported for dogs with DCM?
A UC Davis Stern Lab clinical guidance document, updated August 20, 2018, reports approximately 50 mg/kg by mouth with food every 8 hours for dogs with DCM. This is a condition-specific practice recommendation, not an individualized prescription or a dose established for healthy dogs. The document directs owners to consult a veterinary cardiologist or family veterinarian to determine the best dose for their dog.
Do not use that figure to calculate a dose for your dog. A veterinarian needs to assess the diagnosis, the reason for supplementation, and the product being considered.
Why isn’t L-carnitine routinely recommended for every dog?
L-carnitine is involved in fatty-acid metabolism and energy production. Although dogs with DCM may have low carnitine in heart muscle, the Merck Veterinary Manual says that, in most dogs, this deficiency is more likely a consequence of DCM than its cause. It also notes that supplementation in other breeds has had little success and that diagnosing myocardial carnitine deficiency is difficult: it requires an endomyocardial biopsy.

Merck says routine carnitine supplementation is not recommended for canine cardiomyopathy because its role is not well established. The UC Davis Stern Lab’s document makes the related point directly: “The Stern lab does not recommend the empirical supplementation of taurine or l-carnitine to dogs without evidence of DCM and/or significant deficiency.”
What do the canine DCM reports show?
The published reports do not establish that L-carnitine reliably treats DCM, or that a higher dose is better. Their designs and outcomes differ:

How should you decide whether to give it?
- Ask what diagnosis or deficiency the supplement is meant to address. Do not start L-carnitine empirically; the cited UC Davis guidance advises against it without evidence of DCM or significant deficiency.
- Have your veterinarian set the dose and schedule. The DCM protocol above is not a substitute for an assessment of your dog or a prescription from the attending veterinarian.
- Review the exact product with your veterinarian. Check the label for the amount of L-carnitine per unit, formulation, inactive ingredients, and intended species, and confirm that the veterinarian approves it. The cited evidence does not compare commercial products or establish the suitability of human-labeled tablets for dogs.
What is known about safety?
The cited sources do not establish a comprehensive canine adverse-effect profile or a universal safe dose. They therefore do not support assuming that supplementation is harmless, necessary, or appropriate for a particular dog. Ask your veterinarian about the reason for use, the specific product, and the prescribed dose before giving it.
