After the drug was stopped, neutropenia and thrombocytopenia had resolved at the seven-day check; anemia persisted then, and all three blood-cell abnormalities had resolved one month after discontinuation. The authors advise cautious use of ilunocitinib in dogs with P-glycoprotein deficiency. This is one case, not an estimate of risk or a basis for a genotype-specific dose.
What happened to the Australian Shepherd?
The dog was a five-year-old spayed female Australian Shepherd with atopy and worsening itch despite immunotherapy. She also had a seizure history controlled with levetiracetam and an MDR1 mutant/normal genotype. The case report is indexed at PubMed.
Because blood-cell abnormalities were a recognized concern in the drug label, clinicians obtained a complete blood count (CBC) before treatment. Neutrophil, platelet, and red blood cell counts were within reference ranges. Ilunocitinib was started. At the one-month follow-up, the dog was described as bright, alert, and responsive, and her pruritus score had improved. Her CBC, however, showed pancytopenia—low counts across multiple blood-cell lines.
How did the blood counts change after stopping ilunocitinib?
Ilunocitinib was discontinued. The recovery was not immediate or uniform across the affected cell lines:

This is the timeline for the individual dog in the report, not a prediction of how quickly another dog would recover.
Why might MDR1 status matter?
MDR1 refers to the gene associated with P-glycoprotein, a transporter involved in limiting drug entry into some tissues and in drug excretion. The authors investigated ilunocitinib’s transporter status and identified it as a canine P-glycoprotein substrate.

They proposed that reduced P-glycoprotein-mediated biliary excretion in a dog with P-glycoprotein deficiency likely allowed ilunocitinib to accumulate, creating a relative overdose. They also suggested that increased drug penetration into hematopoietic stem cells lacking P-glycoprotein may have contributed to disrupted blood-cell production. The accumulation explanation is presented as likely, and the stem-cell pathway as a possible contributor—not as proven mechanisms for all dogs.
What the case can—and cannot—tell dog owners
It documents a possible serious adverse event
Pancytopenia was found after a month of treatment despite a normal baseline CBC. Improvement in itch and an apparently well dog did not rule out the blood-count abnormality.

It does not quantify risk or establish a dose change
The report is a single case, not a controlled study or a group of dogs. It does not establish how often pancytopenia occurs, how much MDR1 status changes risk, or a validated alternative dose for dogs with P-glycoprotein deficiency. AAHA described it on May 19, 2026, as the first reported connection between ilunocitinib-associated bone marrow dyscrasia and the MDR1 mutation in dogs; that wording reflects its coverage at that date, not a guarantee that no later case has been reported. The paper’s practical conclusion is cautious use in dogs with P-glycoprotein deficiency. If your dog has a known MDR1 result, ask the prescribing veterinarian how that result affects the treatment decision and what blood-count monitoring is appropriate. Do not change or stop a prescription without veterinary guidance. If a dog receiving ilunocitinib becomes unwell, contact the veterinarian promptly; a dog appearing comfortable or showing less itch does not by itself establish that blood counts are normal.

The published abstract and AAHA coverage do not provide detailed dose information, laboratory values, or a formal monitoring protocol. The case supports discussing CBC monitoring and genetic status with a veterinarian, but it does not prescribe a specific testing schedule.
