Azathioprine for Cats: Overview, Dosage & Side Effects

Veterinary references advise against azathioprine in cats because of serious bone-marrow and liver toxicity risk. Here is why no feline dose exists, what the side-effect concerns are, and what to do if a cat has received or swallowed it.

Veterinary references advise against using azathioprine in cats. Merck Veterinary Manual says it is not recommended in cats because of their sensitivity to its adverse effects, and the ACVIM consensus statement says it should not be used in cats. Because the drug carries a serious risk of toxicity in this species, there is no routine feline dose to publish, and a cat that has received azathioprine or may have swallowed it needs prompt contact with a veterinarian.

Why cats are handled differently from dogs

Azathioprine is an immunosuppressant used in veterinary medicine in some species and for some indications. Dogs are the species in which it appears most often in veterinary dosing references, and that is the source of a common mistake: a dog dose written on a handbook page is a dog instruction, not a cat instruction.

The ACVIM consensus statement explains the concern in cats. Cats cannot metabolize the drug effectively, which raises the risk of hematological toxicity. That single fact is why feline guidance differs so sharply from canine guidance.

Dosage: why this article gives no feline dose

The authoritative veterinary references reviewed for this article do not support any routine dosage of azathioprine for cats. Each of the following points applies:

  • Merck Veterinary Manual says azathioprine is not recommended in cats for chronic enteropathy, because of their sensitivity to adverse effects.
  • Merck’s guidance on feline colitis says its serious adverse effects in cats preclude its use.
  • Merck’s immunomodulator reference calls azathioprine contraindicated in cats because of rapid, potentially lethal bone-marrow suppression.
  • Dog dosing passages on Merck pages describe dogs only and do not establish any feline amount.

Some older or secondary handbook excerpts mention feline dosing, sometimes described as historical or exceptional use. These passages are not recommendations and should not be followed for a cat. Any dose figure you encounter for azathioprine should be read as specific to one species and one indication, and should be discussed with a veterinarian before anything is given.

Side effects and warning signs

The most consequential risk is myelosuppression, meaning bone-marrow toxicity that can impair the production of blood cells. Hepatotoxicity, or liver toxicity, is also identified as a concern in feline guidance by Merck Veterinary Manual.

Bone-marrow suppression

Merck’s immunomodulator reference describes this effect in cats as rapid and potentially lethal. It is the reason the drug is contraindicated in this species, and it is the main reason veterinary references do not treat feline use as a manageable routine.

Liver toxicity

Merck lists hepatotoxicity among the concerns for azathioprine in cats. The hepatotoxicity figures on Merck’s hepatotoxins page come from dogs and must not be read as feline risk data.

Gastrointestinal and other effects

Veterinary pharmacology references also discuss gastrointestinal effects and other adverse effects. The sources cited here do not give a reliable percentage of cats that develop any of these effects. Dog statistics, including retrospective canine figures, cannot be applied to cats.

The sources also do not list home-observable warning signs that would let an owner catch bone-marrow or liver toxicity early. Detection depends on bloodwork and examination by a veterinarian, which is one more reason the drug is not suited to unsupervised home use in cats.

If a cat has received or may have swallowed azathioprine

The severity warnings in the veterinary references support one clear action: get professional help promptly. The references reviewed do not provide a home overdose threshold or an owner treatment protocol, so the steps below are about getting care rather than treating at home.

  1. Call your veterinarian now. If the clinic is closed or you cannot reach them, contact an emergency veterinary service.
  2. Gather the product name and strength from the label, the number of tablets or amount involved, and the approximate time of exposure. Bring the container with you if you travel to the clinic.
  3. Keep the cat away from any remaining tablets or capsules.
  4. Follow the individualized instructions you are given, including any bloodwork recommended to check blood-cell counts and liver function.

Feline inflammatory bowel disease and alternatives

Azathioprine is sometimes discussed in the context of immune-mediated disease or refractory intestinal inflammation. Its use in that context is indication-specific, and for cats the veterinary references advise against it.

Cornell Feline Health Center says that evaluating feline inflammatory bowel disease can involve baseline bloodwork, fecal examinations, imaging, and tests for alternative diseases. There is no single best treatment. Diet and medication combinations may be tried, and the choice depends on the individual cat.

Merck Veterinary Manual describes prednisolone and chlorambucil as options for feline chronic enteropathy rather than azathioprine. The table below summarizes what the reviewed references say about each option.

Chlorambucil is not suitable for every cat and is not risk-free. The veterinarian chooses a plan based on the diagnosis and the patient’s overall condition. For colitis specifically, Merck Veterinary Manual says azathioprine’s serious adverse effects in cats preclude its use and describes other management approaches.

Questions to ask your veterinarian

  • Has the diagnosis been confirmed, and have infectious and other alternative causes been ruled out?
  • Which treatment options are appropriate for this cat, and what does each one involve?
  • What bloodwork or other monitoring will be needed, and how often?
  • What signs should prompt an immediate call?

How current the guidance is

The veterinary references cited here are Merck Veterinary Manual, Cornell Feline Health Center, and the ACVIM consensus statement. Their guidance on azathioprine in cats is consistent, but treatment decisions for a specific cat must be made by a veterinarian who knows that animal.
