FortiFlora Feline (Enterococcus faecium SF68) has the strongest cat-specific clinical evidence in the products reviewed, but it is not a universal answer to diarrhea. A shelter trial and an antibiotic-associated diarrhea trial found better stool outcomes with SF68 in those particular settings. Proviable-DC is a reasonable second comparison because it has a published feline study, but that trial was open-label and does not prove superiority. Persistent, recurrent, or severe diarrhea needs a veterinary workup rather than supplement experimentation.
Which cat probiotic is best?
There is no evidence-based, universal winner for every cat, cause of diarrhea, or household. Probiotic effects belong to a specific strain, dose, formulation, cat population, and measured outcome. A high colony-forming-unit (CFU) number or a longer list of organisms does not establish that one product is better.

Among the products with directly relevant evidence reviewed here, the clearest case is Enterococcus faecium SF68, the strain named on the US label for Purina Pro Plan Veterinary Supplements FortiFlora Feline. Randomized studies found fewer or less severe diarrhea outcomes in shelter cats and in cats receiving amoxicillin-clavulanate. Those results support SF68 for those use cases; they do not diagnose the cause of a pet cat’s diarrhea or prove benefit for every cat.

Proviable-DC, a proprietary multi-species synbiotic, has been described in a published open-label trial of client-owned cats with chronic diarrhea. That makes it a relevant comparison, not a proven head-to-head competitor. No study reviewed here establishes that it outperforms SF68 or any other feline probiotic.

How the evidence changes the ranking
A useful ranking asks more than “How many strains?” Each product should be judged on the exact strain disclosure, cat-specific clinical evidence, study design, label transparency, and fit with the problem being addressed.
| Product or candidate | What is documented | Evidence setting | Important limitation |
|---|---|---|---|
| FortiFlora Feline (US label) | Enterococcus faecium SF68; 1×108 CFU per 1 g sachet; one packet daily or as directed by a veterinarian | Randomized shelter study; randomized study in cats receiving amoxicillin-clavulanate | Results are strain- and context-specific; the manufacturer’s intended use is not independent proof for every cat |
| Proviable-DC | Proprietary multi-species synbiotic | Published open-label trial in client-owned cats with chronic diarrhea | Open-label design; no head-to-head evidence against SF68 |
The FortiFlora amount above is a current US manufacturer-label figure accessed in 2026. Check the package in hand because country, product variant, and labels can change. Do not transfer a dose from another market or a different formulation.
1. FortiFlora Feline: the best-supported starting point in this comparison
What the label says
The US FortiFlora Feline page identifies a cat-only powder supplement containing Enterococcus faecium SF68 at 1×108 CFU per 1 g sachet. The listed direction is one packet daily or as advised by a veterinarian. The manufacturer describes it as intended to assist management of cats with diarrhea. That is an intended use statement, not a guarantee that the product will work for every cause of loose stool.

In a randomized shelter study, diarrhea lasting at least two days occurred in 7.4% of cats fed SF68 compared with 20.7% of controls (P=.0297). The outcome is meaningful because it measured a defined duration of diarrhea, but the setting matters: shelter cats may experience different exposures, stress, diet changes, and infection risks than a stable household cat.
What the antibiotic study found
A separate randomized study enrolled cats receiving amoxicillin-clavulanate. Across days 1–11, cats given SF68 had lower total diarrhea scores than cats given placebo (P=.0058). Measured microbiome-diversity changes did not differ between groups. In other words, the stool-score improvement does not demonstrate that SF68 works by producing a detectable increase in microbiome diversity.
How to interpret “does FortiFlora work?”
The fairest answer is: SF68 has positive randomized evidence for particular diarrhea outcomes, including a shelter setting and antibiotic-associated signs. That is stronger support than a label claim alone, but it is not proof that every cat with diarrhea will improve. A cat with parasites, inflammatory bowel disease, dietary intolerance, pancreatitis, obstruction, cancer, or another illness may need a completely different treatment.
2. Proviable-DC: a multi-species synbiotic with more limited trial design
What was studied
A published trial described Proviable-DC, a proprietary multi-species synbiotic, in client-owned cats with chronic diarrhea. Because the study was open-label, caregivers and investigators knew which product was being given. That design can describe clinical experience and changes over time, but it is more vulnerable to expectation and other biases than a blinded randomized placebo-controlled trial.

Proviable-DC belongs on a comparison list because it has feline-specific published evidence and is aimed at a problem readers commonly ask about. The available evidence does not show that it beats SF68, contains a superior combination, or should replace diagnostic testing. A multi-strain or synbiotic label should not be treated as proof of superiority simply because it lists more organisms or ingredients.
What can you give a cat for diarrhea?
First decide whether home monitoring is reasonable or whether the cat needs veterinary care. Diarrhea is a symptom with causes ranging from a short-lived dietary or stress-related episode to serious disease. A probiotic may be discussed as supportive care, but it cannot identify the cause.

Call a veterinarian promptly when warning signs are present
- Diarrhea persists longer than a day or two.
- The cat has poor appetite, lethargy, or vomiting along with diarrhea.
- There is blood, marked weakness, dehydration, abdominal pain, or another sign of systemic illness.
- Episodes are chronic or recurrent rather than a single mild change.
- The cat is very young, elderly, pregnant, or has a known medical condition that increases risk.
Cornell Feline Health Center advises: “If the diarrhea persists for longer than a day or two and the cat is also showing systemic signs, such as poor appetite, lethargy, or vomiting, you should seek veterinary care as soon as possible.” Over-the-counter human medicines can harm cats and should not be substituted for veterinary advice.
What to record before the appointment
- When the diarrhea began and how often it occurs.
- Stool appearance, including watery stool, mucus, or blood.
- Appetite, drinking, vomiting, energy, and urination.
- Recent food changes, treats, scavenging, travel, boarding, or stressful events.
- All medications, dewormers, supplements, and antibiotics, with dates and doses.
These details help a veterinarian decide whether fecal testing, blood work, imaging, a diet trial, or another evaluation is appropriate.
Why probiotics do not replace an IBD workup
Inflammatory bowel disease (IBD) can cause chronic gastrointestinal signs, but its symptoms overlap with other diseases. Cornell notes that diagnosis requires an extensive workup; definitive diagnosis requires an intestinal or gastric biopsy. Treatment may include diet changes and medication, with probiotics considered among newer therapies rather than a stand-alone diagnosis or cure.
That distinction matters when a cat has repeated loose stools. Temporary improvement after a supplement does not establish IBD, rule out parasites, or prove that the underlying trigger has disappeared. Conversely, a lack of improvement does not prove that a probiotic is useless; it may mean the strain, dose, diagnosis, or treatment plan is not the right match.
How to compare a cat probiotic label
1. Look for the exact strain
“Enterococcus faecium” is a species name; SF68 is the strain designation. Strain-level identity matters because clinical results cannot automatically be transferred from one strain to another species, product, or blend.
2. Check the guaranteed amount and serving
Record the CFU amount per serving and the serving size together. FortiFlora’s US label lists 1×108 CFU per 1 g sachet. Do not compare that number with a product that reports CFUs per container, per scoop, or only at manufacture without first putting the figures on the same basis.
3. Ask whether cats were actually studied
Evidence from humans, dogs, laboratory models, or a different product is not direct evidence for your cat. The strongest SF68 results here came from randomized feline studies, while the Proviable-DC evidence came from an open-label feline trial.
4. Match the study population to the problem
Shelter-associated diarrhea, antibiotic-associated diarrhea, and chronic diarrhea in client-owned cats are different clinical contexts. A positive result in one does not prove the same effect in another.
5. Read directions for the exact variant
Use the current package label for the country and formulation you bought. Follow a veterinarian’s directions when they differ from a general label instruction. Never assume a powder, capsule, paste, and veterinary formula are interchangeable.

6. Treat marketing claims as claims to verify
Words such as “supports digestion,” “balances the microbiome,” or “more strains” describe positioning, not necessarily a measured clinical outcome. The useful question is which cats were studied, what comparator was used, and what changed.
How to use a probiotic safely when your veterinarian agrees
- Confirm the product and indication. Ask whether the veterinarian is addressing a short episode, antibiotic-associated signs, chronic diarrhea, or another diagnosed condition.
- Verify the current label. Check the exact strain, serving amount, expiration date, storage directions, and species indication on the package.
- Give only the directed amount. For the US FortiFlora Feline label, the listed direction is one 1 g sachet daily or as advised by a veterinarian. A different product or country may have different instructions.
- Keep prescribed treatment unchanged unless the veterinarian changes it. Do not stop an antibiotic, diet trial, steroid, or other medicine because a supplement was started.
- Track the response. Record stool frequency and consistency, appetite, vomiting, energy, and any adverse change. This gives the veterinarian usable information rather than relying on a vague impression.
- Escalate if the cat worsens or fails to improve as expected. A probiotic should not delay testing when warning signs persist or recur.
Common mistakes when choosing a feline probiotic
Choosing the biggest CFU number
CFU is a quantity, not a universal potency score. A larger number does not establish that the organisms survive storage, reach the intestine, or improve the outcome that matters to your cat.
Assuming more strains means better results
Different strains can have different effects. Without comparative trials, a multi-strain product cannot be declared superior to a single, clinically studied strain.
Using a dog or human product interchangeably
Species indication, excipients, serving size, and evidence may differ. Use a feline product or obtain explicit veterinary guidance before giving another species’ supplement.
Ignoring the reason for diarrhea
Dietary change, stress, infection, parasites, medication effects, IBD, endocrine disease, and other conditions can look similar at home. A supplement may alter stool without treating the cause.

| Finding | What it supports | What it does not establish |
|---|---|---|
| 7.4% versus 20.7% of shelter cats had diarrhea lasting at least two days with SF68 versus controls (P=.0297) | SF68 was associated with a lower frequency of that defined outcome in that randomized shelter study | A benefit for every household cat, every diarrhea cause, or every product containing a different strain |
| Lower total diarrhea scores over days 1–11 in cats receiving amoxicillin-clavulanate plus SF68 (P=.0058) | SF68 may reduce antibiotic-associated diarrhea scores in that studied context | That microbiome diversity must increase, or that SF68 treats an unrelated gastrointestinal disease |
| Open-label Proviable-DC trial in client-owned cats with chronic diarrhea | The product has published feline clinical experience worth considering | That it outperforms SF68, that the effect was placebo-controlled, or that it works for all chronic diarrhea |
A practical decision guide
If the cat is otherwise well and diarrhea is brief
Contact your veterinary practice for product and monitoring advice, especially if the cat is on medication or has a medical history. Keep the diet consistent, avoid unapproved human medicines, and monitor closely for worsening or persistence.
If diarrhea began during or after antibiotics
Ask the prescribing veterinarian whether SF68 is appropriate as supportive care. The randomized antibiotic-associated study is relevant to this situation, but it does not authorize changing or stopping the antibiotic.
If diarrhea keeps returning
Prioritize a diagnostic plan over repeated supplement trials. Chronic or recurrent disease may require fecal testing, diet evaluation, imaging, medication, or biopsy depending on the findings.

Bottom line on the two leading candidates
FortiFlora Feline is the evidence-led first candidate in this comparison because its SF68 strain is identified on the US label and has two randomized feline study contexts with favorable diarrhea outcomes. Use that conclusion narrowly: the evidence supports a strain and situations, not a guarantee for an individual cat.
Proviable-DC is a legitimate comparison option with a published feline study, but its open-label design and lack of head-to-head testing leave its relative ranking uncertain. The best choice for a particular cat depends on the diagnosis, concurrent treatment, label variant, and veterinarian’s plan.
Frequently asked questions
Can I give my cat a probiotic without seeing a veterinarian?
A veterinarian should guide use when diarrhea is persistent, recurrent, severe, or accompanied by poor appetite, lethargy, vomiting, blood, or another illness sign. Even during a mild episode, confirm the product and serving with your veterinary practice if the cat takes medication or has an underlying condition.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Does a higher CFU count make a cat probiotic better?
No. CFU count alone does not account for strain identity, feline clinical evidence, formulation, storage, or the outcome studied. Compare the exact strain and evidence instead.
Is SF68 the same as every Enterococcus faecium product?
No. SF68 is a particular strain. Results from SF68 should not automatically be applied to an unnamed or different Enterococcus faecium strain.
When should I call a vet about cat diarrhea?
Call as soon as possible when diarrhea lasts longer than a day or two with poor appetite, lethargy, or vomiting, and seek prompt care for blood, dehydration, severe weakness, pain, or rapid deterioration. Recurrent or chronic diarrhea also warrants assessment even if the cat appears comfortable between episodes.
Frequently Asked Questions
Can I give my cat a probiotic without seeing a veterinarian?
Veterinary guidance is important for persistent, recurrent, severe, or illness-associated diarrhea, and for cats taking medication or managing another condition.
Does a higher CFU count make a cat probiotic better?
No. Strain identity, feline clinical evidence, formulation, storage, and the studied outcome matter more than CFU count alone.
Is SF68 the same as every Enterococcus faecium product?
No. SF68 is one specific strain, so its results cannot automatically be applied to other Enterococcus faecium products.
When should I call a vet about cat diarrhea?
Seek veterinary care when diarrhea lasts longer than a day or two with poor appetite, lethargy, or vomiting, or sooner for blood, dehydration, severe weakness, pain, or rapid deterioration.
