Bexacat (bexagliflozin) and Senvelgo (velagliflozin) can be appropriate for some newly diagnosed diabetic cats, but neither is a general substitute for insulin. They are prescription SGLT2 inhibitors that lower blood glucose by making the kidneys excrete more glucose. Their use is limited to cats that are still eating and drinking, are alert and hydrated, have not received insulin, and have no important renal, hepatic or pancreatic disease or ketoacidosis.
The decision is less about choosing a “safer” brand and more about confirming that the cat fits the label criteria, arranging ketone surveillance and ensuring rapid veterinary care if illness develops. Both medicines carry warnings for diabetic ketoacidosis (DKA), including euglycemic DKA in which blood glucose can look normal.

How SGLT2 inhibitors work in cats
Normally, feline kidneys reclaim filtered glucose and return it to the bloodstream. Bexagliflozin and velagliflozin block the renal sodium-glucose cotransporter 2 (SGLT2), so glucose leaves the body in urine (glucosuria). Blood glucose falls without supplying insulin or correcting an absolute insulin deficiency.
That mechanism explains both the convenience and the risk. A cat may have a reasonable glucose reading while continuing to develop ketones because the medicine does not replace the insulin needed to suppress fat breakdown. Glucose concentration alone therefore cannot confirm that treatment is safe.

Both products are generally given once daily under their respective labels. Bexacat is a tablet; Senvelgo is a liquid oral solution. The formulation that is easier for a particular caregiver to give consistently is relevant, but it does not override medical eligibility or monitoring requirements.
Which cats may be candidates before the first dose?
A veterinarian should establish candidacy at diagnosis, not simply dispense an oral medicine because injections seem difficult. The cat should be alert, active enough to interact normally, eating and drinking, and adequately hydrated. A baseline work-up should include the following:
- Complete history and physical examination, including prior diabetes treatment and current appetite, drinking, urination and weight trend.
- Complete blood count and serum chemistry profile.
- Fructosamine to assess recent average glycemia.
- Urinalysis with ketone evaluation.
- Additional investigation when the examination or laboratory results suggest pancreatic, liver or kidney disease.
Do not start an SGLT2 inhibitor in a cat that has previously received insulin, is currently receiving insulin, or is considered insulin-dependent. FDA labeling also excludes or cautions against treatment when the cat is anorexic, dehydrated or lethargic at diagnosis and when renal, hepatic or pancreatic disease is present. A cat that is already systemically ill needs stabilization and a different treatment plan rather than an attempt to avoid injections.

Timing can change eligibility
Eligibility is not permanently “locked in” on the day diabetes is diagnosed. The Senvelgo label calls for reassessment when initiation is delayed more than one week, because the risk of DKA may rise during that interval. If appetite, hydration, energy or laboratory findings worsen while treatment is being arranged, the veterinarian should re-evaluate the plan before any dose is given.

Bexacat versus Senvelgo: practical differences
| Consideration | Bexacat (bexagliflozin) | Senvelgo (velagliflozin) |
|---|---|---|
| Formulation | Tablet | Liquid oral solution |
| Typical labeled frequency | Once daily | Once daily |
| Who should receive it | Newly diagnosed, non-insulin-treated cats that meet label criteria | Newly diagnosed, non-insulin-treated cats that meet label criteria |
| Core safety concern | DKA or euglycemic DKA, including with apparently stable glucose | DKA or euglycemic DKA, including with apparently stable glucose |
| Monitoring emphasis | Glucose, fructosamine, beta-hydroxybutyrate, pancreatic lipase, liver parameters, cholesterol, triglycerides, weight and clinical signs | Ketonuria checks early and during illness, plus examination, glucose assessment, fructosamine and weight at scheduled rechecks |
| When inadequate control changes the plan | Close review during the first eight weeks; if control remains poor at eight weeks, discontinue and consider insulin | If control is poor at four weeks, discontinue and consider insulin |
Neither label supports declaring one drug universally safer. Choice usually turns on the cat’s examination and laboratory results, the caregiver’s ability to give a tablet or measure a liquid accurately, the ability to check ketones, and how quickly veterinary care can be reached.
What effectiveness and side effects can you expect?
The 2025 International Cat Care consensus guideline reports that field studies found velagliflozin and bexagliflozin effective in about 80–90% of treated cases. “Effective” in those studies meant improved mean blood glucose or fructosamine together with improvement in at least one clinical sign. This is a population estimate, not a probability that a specific cat will respond.
Gastrointestinal effects are common. The same guideline reports diarrhea or loose stools in approximately 38–50% of cats receiving velagliflozin and approximately 38% receiving bexagliflozin. Vomiting was reported in approximately 34% with velagliflozin and 50% with bexagliflozin. These percentages come from field-study reports and should not be interpreted as a prediction for an individual cat.

Monitoring plan: the first two weeks are the highest-risk period
The American Animal Hospital Association states that clinical hypoglycemia is rare with SGLT2 inhibitors, so routine glucose curves or continuous glucose monitoring are usually unnecessary. The priority is detecting ketosis, particularly during the first two weeks, when DKA is most common.
Senvelgo schedule
- Two to three days after starting: check urine ketones as directed by the prescribing veterinarian.
- About day seven: repeat the ketone check, even if the cat seems well.
- Any time the cat is ill: check ketones and contact the veterinary team for instructions rather than waiting for the next appointment.
- At approximately one week and four weeks: attend the planned examinations. The veterinarian may assess a glucose curve, fructosamine, body weight and clinical signs.
- At four weeks: poor glycemic control should prompt discontinuation and consideration of insulin rather than simply increasing treatment without review.
Bexacat monitoring
The Bexacat label calls for routine review of glucose, fructosamine, beta-hydroxybutyrate, feline pancreatic lipase, liver parameters, cholesterol, triglycerides, body weight and clinical signs. During the first eight weeks, inadequate control requires close veterinary review. If control remains poor at eight weeks, the label recommends discontinuation with insulin considered.
What to record at home
Keep a dated log that includes dose time, appetite, water intake, urination, vomiting or diarrhea, energy, body weight when your veterinarian recommends weighing, and every ketone result. Note whether a dose was vomited or missed; do not automatically repeat it. Bring the log, medication bottle or tablet packaging and ketone supplies to rechecks.

Ketone testing and the euglycemic DKA problem
Ketones are produced when the body uses fat as an alternative fuel. Rising ketones can precede severe acidosis. With an SGLT2 inhibitor, urine glucose loss may keep blood glucose from becoming dramatically high, creating euglycemic DKA. A normal or only mildly increased glucose value therefore does not rule out an emergency.

A veterinarian should select or approve the testing method and teach the caregiver how to obtain and interpret a sample. A blood beta-hydroxybutyrate meter with compatible strips is a practical option to discuss because it measures the principal circulating ketone directly; urine ketone strips may also be used when the veterinary team has provided a specific plan. The meter does not make home treatment safe: the veterinarian must set action thresholds, explain repeat-testing intervals and remain available for escalation.
Do not wait for a “high” glucose reading before acting. A positive or rising ketone result, especially with reduced appetite or any illness sign, should trigger a call to the prescribing clinic for immediate instructions. The clinician may direct repeat testing, examination, laboratory bloodwork, drug discontinuation or hospital care.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When to stop the medicine and seek emergency care
Stop the SGLT2 inhibitor and contact a veterinarian immediately if any of these occur:
- Sudden anorexia, hyporexia or refusal of normal food.
- Vomiting or diarrhea, particularly when repeated or accompanied by weakness.
- Lethargy, unusual quietness, collapse or marked weakness.
- Dehydration or inability to keep water down.
- Unexpected or continued weight loss.
- Breathing difficulty or an abnormal breathing pattern.
- Positive, rising or otherwise unexpected ketones.
These signs can indicate DKA, pancreatitis, dehydration or another serious disorder. DKA treatment requires prompt insulin and supportive care even when blood glucose is normal. Do not give a replacement dose, switch products, or start insulin at home unless a veterinarian has provided explicit instructions for that situation.

When insulin is the better or necessary choice
Insulin remains essential for insulin-dependent cats and is the standard treatment when DKA is present or suspected. It is also used when an SGLT2 inhibitor fails to provide adequate control, when the cat no longer meets eligibility criteria, or when adverse effects make continuation unsafe.
A cat’s need for insulin is not a caregiver failure. Oral treatment removes injections for some carefully selected cats, but it also requires reliable daily observation, ketone testing during the vulnerable period and rapid access to veterinary care. If those conditions cannot be met, the veterinarian may reasonably recommend insulin from the outset.
A veterinarian-guided decision checklist
- Confirm the diagnosis and severity. Review clinical signs, laboratory findings, fructosamine and urinalysis, including ketones.
- Screen for exclusions. Document whether the cat has ever received insulin and assess appetite, hydration, alertness, renal, hepatic and pancreatic status.
- Choose the formulation. Compare tablet administration with liquid dosing, considering the cat’s behavior and the caregiver’s ability to measure every dose accurately.
- Write the monitoring plan before the first dose. Include ketone method, testing dates, weight and examination appointments, and a phone number for urgent advice.
- Define failure criteria. Agree in advance what poor control, persistent ketones or illness will mean for stopping the drug and moving to insulin or hospitalization.
- Reassess promptly. Keep the early appointments even when thirst and urination improve; clinical improvement does not exclude ketosis.
What is still unknown
Current evidence supports short-term effectiveness, eligibility rules, adverse-event patterns and monitoring requirements. It does not establish that either SGLT2 inhibitor provides a long-term remission or survival advantage over insulin for every newly diagnosed cat, nor that one product is superior for all cats. Treatment should therefore be individualized using the cat’s medical status, laboratory screening, caregiver capacity and access to urgent veterinary care.
