A bot fly lesion in a cat—usually caused in North America by a Cuterebra larva—needs veterinary assessment. A lump with a small breathing hole may be a warble, but abscesses and foreign bodies can look similar. Do not squeeze the swelling or try to remove a larva yourself: rupture can damage tissue and trigger complications.
What a bot fly is and how cats get one
In North America, “bot fly” in cats usually means a rodent or rabbit bot fly in the genus Cuterebra. Its larvae normally parasitize small mammals; cats are accidental hosts. Adult flies place eggs near burrows, runways, or nearby vegetation and objects. A larva can enter a cat as it passes through the area, including through the mouth or nose during grooming, or less commonly through an open wound. Hunting and exploring around rabbit or rodent habitat increase opportunities for exposure.

Infestations are most common in late summer or early autumn according to the MSD Veterinary Manual, but timing varies by location. The manual says larvae leave the skin after approximately 30 days; this is an approximate life-cycle duration, not a reliable countdown for an individual cat. How to recognize a possible warble
A developing larva may form a distinct swelling beneath the skin, often on the head, neck, or trunk. A small central breathing pore may be visible. The fur can become matted, a cat may groom the area intently, and discharge or pus can occur, particularly if there is a secondary infection. Pain varies.

A lump or draining sore is not enough to diagnose a bot fly. An abscess or a foreign body can produce a similar-looking lesion, so a veterinarian needs to examine it. The MSD Veterinary Manual identifies a soft, fluctuant swelling with a fistula as typical and says it should be explored for parasitic larvae. MSD Veterinary Manual: Flies and Mosquitoes of Cats
When signs may mean migration
Less often, a larva moves away from the skin and affects the eye or surrounding tissues, nose, larynx, trachea, or central nervous system. These presentations can be serious. Seek urgent veterinary care for signs such as seizures, blindness, unusual behavior or mentation, marked lethargy, abnormal temperature, or respiratory signs such as sneezing. These symptoms have many possible causes and do not, by themselves, prove a bot fly infestation. A history of sudden, intense sneezing weeks or months earlier may be relevant when a cat later develops neurologic signs, but it is not diagnostic.

How veterinarians diagnose and treat a skin lesion
Diagnosis
Diagnosis is confirmed by finding and identifying the larva. For a visible lesion, the veterinarian may carefully explore the breathing pore. When there is no visible warble but signs suggest respiratory or neurologic migration, imaging may help guide diagnosis.

Removal and wound care
For a skin lesion, treatment generally involves careful removal by a veterinarian, followed by cleaning and flushing the wound. Debridement may be needed depending on tissue damage; healing can take weeks to months. If there is a secondary infection or more extensive injury, additional veterinary treatment, medication, or surgery may be considered. The appropriate care depends on the cat and the lesion, rather than following one universal protocol. VCA Animal Hospitals: Cuterebra (Botfly) or Warbles in Cats
Why not to squeeze or extract it at home
Do not press on the lump, probe the pore, or pull at a suspected larva. Squeezing can rupture the larva and provoke a chronic foreign-body reaction or secondary infection. The MSD Veterinary Manual also notes anecdotal reports of anaphylaxis after rupture. Arrange veterinary assessment instead; wound flushing described in veterinary care is not a reason to open or manage an intact lesion at home.

Neurologic disease and outlook
Central nervous system cuterebriasis is a separate, serious presentation; the MSD Veterinary Manual’s owner guidance describes treatment as experimental. Do not attempt medication treatment at home. The professional manual notes that ivermectin use for this purpose is extralabel in the United States, so treatment decisions belong with a veterinarian.

Reducing a cat’s exposure
- Where practical, limit hunting and access to rodent or rabbit burrows.
- Check cats with outdoor access regularly for new swellings, matted fur, or a small breathing pore, and contact a veterinarian if you find a suspicious lesion.
- Ask a veterinarian about an individual cat’s risk and health; the reviewed veterinary guidance does not establish a guaranteed preventive medication for Cuterebra.
