California’s First Human Bird Flu Cases: What Happened to Dairy Workers in 2024

California’s first confirmed human H5N1 cases involved two dairy workers at separate Central Valley farms in October 2024. Later surveillance documented additional cases, mostly among workers exposed to sick cows.

California’s first confirmed human H5N1 bird flu cases were announced on October 3, 2024, and involved two dairy workers at separate Central Valley farms—not one worker. Both had conjunctivitis, and one also reported a fever. The initial investigation found no known link between the workers; later surveillance recorded many more California cases, mostly among dairy workers exposed to sick cows.

What happened to the California dairy workers?

On October 3, 2024, the CDC confirmed the first two human H5N1 cases in California. The workers were employed at different dairies where infected cows had been detected. Both experienced conjunctivitis, and one also reported fever. Officials reported no known epidemiologic link between the two workers, so the initial report did not establish that they infected one another. CDC MMWR; CDC announcement

On October 4, 2024, the California Department of Public Health alerted clinicians to consider H5N1 in people with symptoms after exposure to animals confirmed or suspected to have bird flu. The alert described the two workers as employed at dairy facilities with outbreaks in the Central Valley. CDPH alert

How many H5N1 cases were later reported in California?

The first two cases were an initial October 2024 milestone, not California’s final tally. A CDC investigation later documented 38 positive test results in the state between September 30 and December 24, 2024. Of those patients, 37 were dairy workers with occupational exposure to sick cows, and one was a child whose exposure could not be determined. This later count should not be confused with the two cases confirmed on October 3. CDC MMWR

All 38 patients in that reported series had mild illness; none was hospitalized or died, and all recovered. Among patients with available symptom data, 97% reported eye irritation or redness. These outcomes describe the reported California patients in that period; they are not a prediction for every person exposed to H5N1.

Can bird flu spread from cows to people?

The California cases were associated with occupational exposure to infected or sick dairy cows. The CDC MMWR states: “Persons with occupational exposure to highly pathogenic avian influenza (HPAI) A(H5N1) virus–infected dairy cattle are at increased risk for infection.” That finding concerns exposure to infected cattle at work; it does not quantify the risk from other kinds of contact. California worker guidance describes possible routes of exposure as splashes or aerosols containing virus, or touching contaminated surfaces and then touching the eyes, nose, or mouth. It is occupational, task-specific guidance—not a general consumer protection checklist. California DIR worker guidance

What protection does California recommend for exposed workers?

When bird flu is detected in animals, California DIR says employers must provide and ensure that exposed employees wear N95 respirators, goggles or face shields, gloves, full-body coveralls, and any other PPE needed for the task. The guidance also directs workers to follow written safety procedures, report symptoms, and obtain medical attention through their employer. Protection is based on the work and exposure conditions; no single item should be treated as sufficient by itself. California DIR worker guidance

Was there evidence of human-to-human spread?

The CDC investigation did not identify human-to-human transmission in the United States as of the report’s publication. It also found no viral adaptations indicating efficient human-to-human transmission among the sequences it assessed. Those are dated findings from that investigation, not proof that transmission between people is impossible or a guarantee about future events. The report notes that PPE-use information was incomplete and that some symptomatic exposed people might not have been reported. CDC MMWR
