Hiding, fleeing, freezing, or hissing may signal fear or anxiety, but those signs do not reveal the cause on their own. If the behavior is new, severe, recurring, or escalating, arrange a veterinary assessment; breathing difficulty, especially open-mouth breathing or panting, needs immediate veterinary attention.
What does “panic attack” mean in cats?
People may use “panic attack” to describe a sudden episode in which a cat appears overwhelmed or terrified. Veterinary guidance instead discusses fear, anxiety, phobias, and stress responses; it does not provide validated diagnostic criteria for feline panic attacks. That distinction matters because an episode’s appearance cannot establish whether the cause is fear, anxiety, pain, illness, or something else.

VCA describes fear as an emotion triggered by a perceived threat and anxiety as anticipation of a fear-inducing stimulus. They can produce similar physical and behavioral signs, and anxiety may arise before the trigger is present. The threat may be real or perceived, as the Fear Free guidance explains. VCA: Fear vs. Anxiety in Pets; Fear Free: Fear Free for Cats.
What might a fear or anxiety episode look like?
Possible signs include:
- Hiding, trying to escape, or fleeing
- Freezing, crouching, or keeping the body low
- Ears held back, dilated pupils, or a fluffy tail
- Hissing, arching the back, swatting, or biting, particularly if escape is blocked
- Increased heart or respiratory rate
These are clues to interpret in context, not proof of a panic attack. A cat may freeze rather than run, and defensive behavior can occur when the animal feels trapped. Fear and anxiety can overlap, so the visible behavior alone cannot distinguish them or rule out a medical cause. VCA: Fear vs. Anxiety in Pets; VCA: Fear Free for Cats.

What can trigger a cat’s fear or anxiety?
Potential triggers include travel, veterinary visits, unfamiliar people or animals, children, new objects or surroundings, loud noises, and a frightening past experience. Fear can follow one intense event or develop through repeated exposure. A trigger may be present in the moment, or a cat may become anxious in anticipation of one.
Pay attention to when the behavior happens and what came before it: a carrier, car ride, clinic handling, a visitor, a noise, or a change at home may provide useful context for your veterinarian. A secure cat carrier can help with safe travel, but it is not a treatment for anxiety. VCA: Fear Free Visits; VCA: Fear vs. Anxiety in Pets.

Could a health problem be involved?
Yes. Pain or underlying illness can contribute to fear or anxiety. A new or heightened fear response in an adult or senior cat deserves particular attention rather than being assumed to be purely emotional. Veterinary guidance recommends considering medical issues and using an examination—and, when appropriate, laboratory tests—to help identify possible contributors. VCA: Fear vs. Anxiety in Pets; VCA: Fear Free for Cats.

When is a cat’s breathing an emergency?
Do not assume panting or open-mouth breathing is anxiety. VCA advises immediate veterinary attention for breathing problems, especially open-mouth breathing or panting. Contact a veterinarian or emergency veterinary service promptly rather than waiting to see whether the episode passes. VCA: Recognizing Signs of Illness in Cats.

What should you do during and after an episode?
While your cat is frightened
- Keep the environment calm and let your cat reach a quiet, safe retreat.
- Do not punish, force handling, block an escape route, or deliberately expose your cat to the trigger outside a controlled plan.
- If you can safely reduce or remove the trigger, do so and allow your cat time to recover.
Once your cat is settled
Arrange veterinary assessment if the behavior is new, severe, recurring, or getting worse. Note the circumstances, timing, and signs you observed so you can describe the episode clearly. If a medical contributor is not found or fear persists, ask about a tailored behavior plan. A veterinarian or qualified veterinary behavior professional can advise whether gradual desensitization and counterconditioning are appropriate; moderate or severe fear calls for professional guidance. VCA: Fear vs. Anxiety in Pets; VCA: Fear Free for Cats.
Medication and unavoidable triggers
A veterinarian may consider medication when fear is severe or a trigger cannot be avoided. Do not give sedatives or other medication without veterinary direction. The appropriate plan depends on the cat, the trigger, the severity, and whether the fear is limited to one situation or appears in multiple settings.
