Constipation in cats means passing stool less often or with difficulty, usually producing hard, dry feces. Repeated litter-box trips, straining, or small hard stools can be clues—but straining can also signal a urinary blockage, which is an emergency. Contact a veterinarian promptly if your cat seems ill or painful, and call if no bowel movement has occurred within 48 to 72 hours.
What constipation looks like in cats
The Cornell Feline Health Center defines constipation as “infrequent or difficult emission of hard, dry fecal matter.” A cat may make repeated attempts to defecate, strain or vocalize, pass small, hard stools, or go outside the litter box. Some cats pass a little liquid stool or mucus around retained hard feces, which can be mistaken for ordinary diarrhea. Cornell Feline Health Center and Merck Veterinary Manual describe these signs.

More serious cases may also involve reduced appetite, lethargy, vomiting, or abdominal discomfort. These signs warrant veterinary advice rather than watchful waiting.
Constipation, obstipation, and megacolon are different
Routine constipation is not the same as obstipation or megacolon. Merck describes obstipation as intractable constipation in which retained hard feces cannot be evacuated. Megacolon is abnormal enlargement and reduced movement of the large intestine; chronic cases that do not respond to medical treatment may need surgery. First rule out a urinary emergency
Straining in the litter box can look similar whether a cat is trying to pass stool or urine. If you cannot tell whether urine is coming out, do not assume constipation. Cornell identifies urethral obstruction—the inability to pass urine—as a true medical emergency requiring immediate veterinary attention. What can cause constipation?
Constipation has several possible causes, and sometimes a veterinarian cannot identify one specific cause. Possibilities include dehydration or insufficient water intake; hair, bones, litter, or other poorly digestible material; and pain or reluctance to defecate. Stress, an unsuitable or dirty litter box, or difficulty reaching the box can contribute to avoidance. Other causes include medication effects, an intestinal obstruction, tumors or strictures, pelvic injury, neurologic disease, and impaired movement of the colon. Older or overweight cats, cats with chronic kidney disease, and cats with previous episodes may be at increased risk, according to Merck’s clinical review. Kidney disease can contribute to chronic dehydration. These factors raise concern; they do not establish a diagnosis in an individual cat.

How a veterinarian checks for constipation
The veterinarian will usually ask about the cat’s litter-box habits and general health, then perform a physical examination. Abdominal palpation may reveal retained feces in the colon. X-rays can help assess stool buildup and look for an obstruction. Depending on what the examination suggests, the veterinarian may also recommend a rectal examination, blood or urine tests, ultrasound, or other procedures to investigate an underlying disease or mass. Information to share at the appointment
- When your cat last passed a normal stool, and what recent stools looked like.
- How often your cat is trying to use the box, and whether urine is passing.
- Any change in appetite, water intake, vomiting, activity, pain, or mobility.
- Current medicines, previous constipation episodes, and possible access to bones, litter, or other objects.
How constipation is treated
Treatment depends on severity, hydration, the suspected cause, and whether the problem is recurrent. A veterinarian may correct dehydration, address an underlying problem, prescribe an appropriate laxative or medication that changes intestinal movement, or recommend a diet change. Severe impaction may require an enema or manual removal of feces in a veterinary setting; removal may require anesthesia and supportive fluids. For chronic constipation or megacolon that fails medical management, surgery may be considered. These approaches are described by Cornell, Merck, and VCA.
For some mild cases, a veterinarian may discuss easier access to water, a clean and accessible litter box, or a diet with an appropriate amount and type of fiber. Fiber is not a universal fix: Cornell notes that too much soluble fiber can make stool overly liquid and affect nutrient absorption, while insoluble fiber can reduce water in stool. Ask the veterinarian before changing food or adding fiber.

What not to try without veterinary guidance
- Do not give a human laxative. Merck warns that laxatives made for people can be dangerous for pets.
- Do not attempt an enema or manually remove stool at home.
- Do not change a prescribed treatment plan or give additional medication without consulting the veterinarian. VCA advises against altering a treatment protocol without veterinary guidance.
When to call the veterinarian
VCA advises contacting a veterinarian if your cat has not passed stool within 48 to 72 hours of the previous bowel movement. Contact one sooner if your cat repeatedly strains, appears painful or unwell, stops eating, becomes lethargic, vomits, or passes only small amounts of liquid stool when hard stool may be retained. Do not wait for the time threshold if your cat is showing concerning signs.

Why treatment is not one-size-fits-all
The right approach depends on how severe the stool buildup is, whether an obstruction or impaction is suspected, the cat’s hydration and likely underlying cause, and whether this is a first episode or a chronic problem. Mild cases may respond to veterinarian-directed changes, while severe impaction can require procedures. If the colon has become irreversibly enlarged and medical management fails, the veterinarian may discuss surgery. A veterinary assessment helps distinguish these situations and avoid an unsuitable home treatment.
