Ear mites in cats are caused by the parasite Otodectes cynotis. The typical pattern is intense itch, inflamed outer ears, repeated scratching or head shaking, and dark, gooey wax mixed with debris. Those signs warrant a veterinary examination rather than a home diagnosis: infections and other ear diseases can look similar, and dark material alone does not prove mites.
A veterinarian can inspect the canal, examine discharge under a microscope, and select an appropriate antiparasitic. Treatment may include a prescribed topical, oral, or other systemic medicine plus veterinarian-directed cleaning. Because mites spread readily between animals, the vet may also advise checking or treating in-contact pets and cleaning bedding and grooming items.

What ear mites are and why they matter
Otodectes cynotis lives in the ear canal and feeds on surface debris. Its activity causes inflammation and severe irritation. Cornell’s feline guidance describes the classic presentation as inflamed outer ears, near-constant scratching, frequent head shaking, and a dark, gooey accumulation of wax and mite debris.
Mites are extremely contagious between animals during close contact. An outdoor cat can encounter them while sharing space with, grooming, or fighting with other cats. A cat does not need to roam widely to be exposed if another infested animal enters the household.

Persistent scratching can tear the skin around the ears and face. Irritation may be followed by a secondary infection or otitis externa (inflammation of the external ear canal). If infection progresses toward the middle or inner ear, hearing and balance can be affected. Early veterinary attention is safer than waiting for those complications.
Signs that should prompt an examination
Common signs of a possible infestation
- Frequent or forceful scratching at one or both ears
- Repeated head shaking or tilting
- Redness, swelling, heat, or obvious tenderness around the ear opening
- Dark, sticky or crumbly wax mixed with debris
- Rubbing the head on furniture or the floor
- Scabs, hair loss, or scratches on the ear flap or nearby face from self-trauma
Some cats show only a portion of this pattern. One quiet-looking ear does not rule out mites, particularly when another pet in the home is itchy.
Why appearance is not enough
Dark discharge is suggestive but not diagnostic. Bacterial or yeast-related disease, other inflammation, injury, and additional ear problems can produce overlapping redness, itch, odor, or discharge. Using an over-the-counter product solely because debris looks dark can delay the right treatment or aggravate a painful ear. Diagnosis should be based on an examination and, when needed, testing of the material in the canal.

How a veterinarian confirms ear mites
Otoscope examination
The veterinarian may use an otoscope to view the ear canal and eardrum, assess swelling and debris, and look for damage or another cause of pain. A very inflamed canal can make this difficult. VCA notes that sedation may be needed when a cat cannot tolerate the examination or when the ear must be cleaned and treated thoroughly.
Microscopic examination of ear debris
The clinician can gently collect discharge and inspect it under a microscope for mites or their eggs. Cornell describes this approach when a cat will not tolerate an otoscope. Microscopy also helps distinguish mites from other causes of discharge, so treatment is based on evidence rather than color alone.
Information to bring to the appointment
- When scratching, shaking, or discharge began
- Whether one or both ears are affected
- Names of any ear cleaners, drops, flea products, or home remedies already used
- Whether other cats or dogs are itchy or have ear discharge
- Recent boarding, grooming, rescue, outdoor, or close-contact exposure
How ear mites are treated
There is no single safe dose or product that can be chosen from a general article. The veterinarian selects a medicine according to the cat’s age, weight, health, examination findings, local approvals, and whether infection or eardrum damage is also present. Follow the exact label and veterinary directions; products and routes are not interchangeable.

Cleaning as an adjunct
Wax and debris can shield mites from a topical medicine. Merck Veterinary Manual states that, as a general rule, cleaning with an appropriate ceruminolytic agent is part of treatment. Cleaning is an adjunct, not a mite-killing drug by itself.

Do not probe a sore ear or force liquid into a canal that has not been examined. Pain, marked swelling, infection, or concern about the eardrum changes what can be safely put in the ear. A retail cleaner should never be presented as a cure unless its label and the veterinarian specifically support that use.
Medication routes veterinarians may consider
| Route or format | How it may fit | Important qualification |
|---|---|---|
| Topical medicine placed in the ear | Can act at the site of the infestation after the canal is assessed and, when appropriate, cleaned. | The product must be appropriate for the cat and the condition; a painful or damaged ear may require a different plan. |
| Topical medicine applied to the skin | Some systemic-acting products treat ear-mite infestations without drops being placed down the canal. | Application site, repeat schedule, age and weight limits, and species eligibility come from the specific label. |
| Oral or other systemic medicine | May be selected when the veterinarian considers a non-ear route suitable or when additional disease is present. | Do not substitute a human, dog, livestock, or previously prescribed product. |
Selamectin and other approved options
In the United States, selamectin is labeled for treatment and control of Otodectes cynotis infestations in cats. U.S. regulation restricts it to use by or on the order of a licensed veterinarian. The approved age, weight bands, application instructions, and repeat directions depend on the particular product, so a general article cannot supply a dose.

Merck Veterinary Manual describes approved systemic therapies including topical selamectin and moxidectin. Approvals and availability differ by country, and a product authorized for one species, age, or region may not be suitable for another.
Cornell dermatologist William Miller Jr., VMD, explains that “There are many topical, oral, and systemic agents,” and that “most—such as ivermectin—are highly effective.” This is a general description of available veterinary agents, not permission to give ivermectin or any other drug without veterinary direction. Concentration, formulation, route, interactions, and toxicity risk matter.

Because mites pass between animals, tell the veterinarian about every cat and dog that has close contact with the affected cat. Merck advises treating other cats and dogs in the home when indicated. A pet that looks normal may still need an examination or a preventive treatment decision from the clinician.

- Keep the affected cat’s appointment and ask whether each in-contact pet should be examined or treated.
- Use each medicine only on the species and animal for which it was prescribed or labeled.
- Wash or otherwise clean bedding, blankets, brushes, carriers, and other items that contact the animals, following the veterinarian’s instructions.
- Reduce close contact with untreated animals until the veterinarian says it is appropriate.
- Complete the prescribed course and attend any recommended recheck, even if scratching improves quickly.
Mites can survive away from the host for some time, so cleaning household items is sensible veterinary advice. No particular cleaning schedule guarantees that reinfestation will not occur; the plan should match the animals and environment in your home.
Safe care while you arrange veterinary help
- Arrange a prompt appointment if scratching, head shaking, redness, or dark discharge persists.
- Prevent further self-trauma as advised by the clinic; a cat that is ripping the skin may need immediate care.
- Do not insert tools into the ear canal or use leftover medication from another pet.
- Tell the clinic about pain, foul odor, swelling, bleeding, a head tilt, stumbling, or apparent hearing change.
- If the cat becomes markedly unsteady, cannot keep its balance, or has severe pain, seek urgent veterinary attention because deeper ear disease may be involved.
How ear mites differ from ordinary wax or another ear problem
Normal wax is usually limited and does not cause relentless scratching. Mite-related debris is often dark and abundant, but that visual clue is not specific. A veterinarian’s otoscope and microscopic examination answer questions that home inspection cannot: whether mites are present, whether the canal is damaged, and whether bacteria, yeast, or another condition also needs treatment.
That distinction matters because an antiparasitic alone will not correct every cause of ear inflammation. Conversely, repeatedly treating for infection without identifying mites can leave the infestation active. The safest sequence is diagnosis, a targeted prescription, and follow-up if signs do not resolve as expected.

Preventing a repeat infestation
Keep newly acquired or recently exposed animals separate from resident pets until a veterinarian has assessed them when exposure is plausible. Ask before introducing an animal that has intense ear itch or dark discharge into a shared sleeping or grooming area. Outdoor access increases opportunities for close contact with other cats, so discuss exposure reduction with the veterinarian if infestations recur.
Do not use a household insecticide, dog product, or human medicine as a shortcut. Prevention depends on identifying every in-contact animal and using products according to their veterinary label, not on treating only the cat that shows the most dramatic debris.
Questions to ask at the appointment
- Were mites seen on microscopy, or is another ear condition more likely?
- Is the eardrum visible and intact?
- Does this cat need ear cleaning, and which cleanser and technique are safe?
- Which medication, route, dose, and repeat schedule apply to this cat’s age and weight?
- Should the other cats or dogs in the household be examined or treated?
- What signs mean the cat needs a recheck or urgent care?
Bottom line for cat owners
Ear mites are a contagious parasitic cause of severe ear itch and dark debris, but the same signs can come from other disease. Have a veterinarian confirm the cause, use a labeled treatment selected for that individual cat, clean only as instructed, and address exposed pets and household items. Prompt care limits scratching injuries and the risk that external-ear disease will progress deeper.
