Ear polyps in cats are benign inflammatory growths, but their location can make them serious. A polyp may occupy the ear canal, middle ear, auditory tube, throat, nose or nasopharynx, causing chronic ear disease, balance problems, swallowing difficulty or obstructed breathing. A veterinarian must examine the cat—often under sedation or anesthesia—to locate the growth, confirm it with tissue examination and remove it safely. Breathing difficulty is an emergency.

What an “ear polyp” is in a cat

Feline inflammatory polyps are non-cancerous growths arising from mucosal tissue. They most often begin in the lining of the tympanic bulla (the bony middle-ear chamber), the auditory tube that connects the middle ear with the throat, or the pharyngeal lining. As they enlarge, they can project into the external ear canal, middle ear, nose or nasopharynx—the space behind the nasal passages and above the soft palate.

“Ear polyp” is therefore a useful owner term, not a precise statement about where the growth started. A mass visible in the ear may have a deeper attachment in the middle ear, while a mass seen under the soft palate may be connected through the auditory tube. That hidden origin is why pulling on a visible mass at home is unsafe and why a complete veterinary examination matters.

Which cats develop them?

Merck Veterinary Manual describes a predilection for young cats, especially those from 3 months to 5 years of age. No sex or breed predilection has been established. Polyps can occur on both sides; the American College of Veterinary Surgeons (ACVS) reports bilateral polyps in approximately 13%–24% of affected cats.

What causes nasopharyngeal and ear polyps?

The cause remains uncertain. Chronic inflammation of the upper respiratory tract or middle ear has been proposed, but a clear association with specific bacterial or viral infections has not been demonstrated. Merck notes that feline calicivirus and feline herpesvirus have not been identified in polyp tissue, so a virus should not be presented as a proven cause.

A Cornell University College of Veterinary Medicine article quotes James Flanders, DVM, associate professor of surgery, saying: “We believe that this type of polyp is caused by inflammatory changes secondary to infection with a respiratory virus.” That is an attributed hypothesis rather than settled causation. Current professional-reference language continues to describe the etiology as unclear.

Because the initiating event is unknown, there is no proven home measure that prevents a polyp. Treating a respiratory infection or ear infection may be appropriate for that illness, but it does not remove an established growth. Do not put leftover antibiotics, human medicines or ear drops into the ear unless a veterinarian has prescribed them for this cat.

Signs of a polyp and what they mean

Signs depend on the route the growth takes and often become chronic or progressively worse. Some cats show only ear symptoms; others develop nasal, throat, eye or neurological changes.

Nasopharyngeal, throat and nasal signs

  • Noisy, snoring-like or stertorous breathing
  • Sneezing or persistent nasal discharge
  • Difficulty swallowing, gagging or repeated throat-clearing motions
  • Open-mouth breathing or increasing effort to breathe when the passage is obstructed

A cat with a nasopharyngeal polyp may seem to have a recurring “cold,” but a persistent or worsening pattern warrants an examination rather than repeated over-the-counter treatment.

Ear and middle-ear signs

  • Frequent head shaking or scratching at one ear
  • Ear discharge or a recurring ear problem that does not resolve
  • Head tilt, circling, stumbling or other loss of coordination
  • Rapid, involuntary eye movements (nystagmus)
  • Facial-nerve changes, such as an uneven face or difficulty blinking

Middle-ear involvement can affect the balance organs and nearby nerves even when the external ear canal looks relatively normal. A head tilt or new incoordination should be assessed promptly, especially if it appears with vomiting, falling or abnormal eye movements.

Eye and sympathetic-nerve changes

Damage or pressure near the nerves that pass through the middle ear can produce Horner’s syndrome. Typical findings on the affected side include a small pupil, a raised third eyelid, a sunken-looking eye and a drooping upper eyelid. These signs can be alarming, but they also help the veterinarian localize disease.

When breathing difficulty is an emergency

ACVS describes a distressed cat with airway obstruction as potentially extending its neck, panting and appearing anxious. Difficulty breathing requires prompt veterinary care, not watchful waiting. Keep the cat as calm as possible, avoid forcing an examination of the mouth or throat, and transport it for urgent assessment. Do not attempt to pull out a visible mass or give food, water or medication while the cat is struggling to breathe.

How a veterinarian diagnoses a polyp

Diagnosis involves locating the growth, assessing how far it extends and examining tissue. The visible portion alone may not reveal its attachment.

  1. Physical and neurological examination. The veterinarian evaluates breathing, swallowing, facial symmetry, eye position, balance and coordination, then examines both ears rather than assuming the problem is limited to the side showing discharge.
  2. Otoscopy and nasopharyngeal inspection. An otoscope can evaluate the ear canal and eardrum. The nasopharynx may need to be inspected beneath the soft palate. Pain, movement and the cat’s anatomy often make a complete examination impossible while awake.
  3. Sedation or general anesthesia when needed. Sedation or anesthesia allows a thorough look at the ear canal, tympanic membrane and throat while reducing stress and preventing injury. It also permits safe sampling or removal.
  4. Imaging. Radiographs may show soft-tissue changes or increased opacity in the middle-ear bullae. Computed tomography (CT) or magnetic resonance imaging (MRI) can identify a suspected middle-ear mass and map its extension into nearby structures. The choice depends on the suspected location, available equipment and the surgical plan.
  5. Histology. Tissue removed during the procedure is examined microscopically. Histological examination confirms an inflammatory polyp and helps distinguish it from other masses that can resemble one clinically.

These steps also help the surgeon decide whether a growth can be removed through the ear or mouth or whether the middle-ear cavity must be opened to reach its base.

Treatment: removing the growth and its attachment

The definitive treatment is surgical removal. Medication may address a concurrent infection or inflammation, but it does not eliminate the polyp itself. The technique is chosen for the individual cat according to the stalk’s location, the extent of disease and the likelihood that the base can be removed completely.

Approach What it involves When it may be considered Main trade-off
Traction avulsion The surgeon gently removes the mass by traction under general anesthesia when it extends into the external ear canal or beneath the soft palate. A visible, accessible polyp whose attachment can be reached safely. Less invasive, but leaving the stalk or base behind increases the chance of regrowth.
Ventral bulla osteotomy The surgeon opens the bony middle-ear cavity through an approach under the jaw and removes tissue at its origin. When traction alone cannot reach the base, disease is extensive, or recurrence is a major concern. More invasive middle-ear surgery, with temporary balance or nerve effects possible.
Traction combined with ventral bulla osteotomy The accessible portion is removed and the middle-ear origin is addressed during the same treatment plan. When complete removal requires both routes. More involved than traction alone, but offers better control of the attachment.

Why the base matters

Removing only the part that protrudes can leave the polyp’s origin in the middle ear or auditory tube. Merck reports rapid regrowth and return of signs in 15%–50% of cats when traction avulsion leaves the base. ACVS reports recurrence below 2%–10% after traction combined with ventral bulla osteotomy, compared with 30%–50% after traction alone. These are source-specific ranges, not a guarantee for an individual cat; the surgeon’s ability to identify and remove the attachment is the key factor.

A surgeon may recommend imaging before an operation when the attachment is not clear, when both ears may be involved or when signs suggest disease beyond the visible mass. Ask which part of the polyp is accessible, whether the base can be removed and what neurological or balance effects are expected with the proposed approach.

Recovery and aftercare

ACVS describes most cats as recovering rapidly after treatment. Recovery still depends on the procedure and on whether infection or nerve involvement was present before surgery.

Expected temporary effects

After middle-ear surgery, a cat may temporarily develop Horner’s syndrome, a head tilt or balance problems. ACVS reports that about 80% of cats develop Horner’s syndrome after ventral bulla osteotomy; it is usually temporary and generally resolves within a month. The organization describes the overall prognosis as excellent, including for cats that experience these temporary signs.

Medications, food and incision protection

The surgeon may prescribe pain relief and may recommend antibiotics when a concurrent infection is suspected. Follow the exact dose and duration rather than stopping early or substituting a human product. Wet food may be advised during healing if swallowing is uncomfortable or the throat has been manipulated.

If an incision needs protection, the veterinarian may recommend an Elizabethan or other recovery collar. Its purpose is to prevent scratching or licking of the surgical site; it does not treat a polyp or prevent one from forming. Use it for the period and fit specified by the veterinary team.

What to monitor at home

  • Appetite, water intake and ability to swallow
  • Breathing effort and any return of noisy breathing
  • Head tilt, falling, circling or worsening eye movements
  • Incision redness, swelling, discharge or self-trauma
  • New ear discharge, facial weakness or a return of the original signs

Contact the surgeon or primary veterinarian promptly if the cat is not eating after surgery, develops increasing breathing effort, or shows deterioration rather than gradual improvement. Do not flush the ear, probe the canal or apply non-prescribed drops while the tissues are healing.

Prognosis and the possibility of recurrence

Inflammatory polyps are benign, so the concern is obstruction, nerve dysfunction and recurrence rather than malignant spread. When the growth and its base are removed, the outlook is generally excellent. Cats with nasopharyngeal involvement often regain comfortable breathing and swallowing after the obstruction is relieved.

Recurrence is most closely linked to residual tissue at the attachment. A cat can appear well immediately after a traction procedure and later redevelop discharge, noisy breathing, head shaking or a head tilt if the base remains. A return of signs should prompt a recheck; it does not prove cancer, but it may indicate regrowth or another middle-ear problem.

Because bilateral polyps are reported in 13%–24% of affected cats, the veterinarian may assess the opposite ear even when only one side is symptomatic. Long-term follow-up is individualized and may include repeat examination or imaging if signs persist.

Practical questions to discuss with your veterinarian

  • Where does the polyp appear to originate, and which spaces does it enter?
  • Will the examination require sedation or general anesthesia?
  • Would radiographs, CT or MRI change the surgical plan?
  • Can the stalk or base be removed with traction, or is a ventral bulla osteotomy more appropriate?
  • What temporary head tilt, balance or eye changes should be expected?
  • Which pain medicines, antibiotics, food and collar instructions apply at home?
  • What specific signs require an immediate call or emergency visit?
