A hairball episode usually ends with your cat bringing up a damp wad of swallowed hair. A cough may look like hacking or an attempt to bring something up, but often produces no hairball. Because both can involve crouching and stretching the neck, posture alone cannot tell you which is happening. Watch what comes up and how your cat is breathing—and seek urgent veterinary care for open-mouth or difficult breathing.
Hairball retching or coughing? Compare what happens
The most useful clue is whether material is produced. A hairball is swallowed, undigested hair mixed with digestive fluids. When brought up, it is often elongated like a cigar or sausage rather than round. A cough is a forceful expulsion of air from the lungs; it may be followed by gagging, which can make it look like retching, but no hairball may appear. VCA notes that “Cats do cough, but not nearly as often as other animals.” VCA’s guide to coughing in cats describes coughs and the signs that warrant veterinary advice.

| Clue | More consistent with hairball retching | More consistent with coughing |
|---|---|---|
| What comes up | A damp wad of hair may be brought up, often with bile or digestive fluid. | Often no hairball appears; the episode is an expulsion of air. |
| Movement and sound | Retching or vomiting culminates in bringing up material. | A dry or harsh cough may involve crouching and neck extension, then end in a gag. |
| What the clue tells you | A visible wad supports that hair was regurgitated, but repeated episodes still deserve attention. | Posture and sound may suggest a cough, but cannot identify its cause. |
| Urgency | Repeated unproductive retching, lethargy, or refusing food warrants prompt veterinary assessment. | Breathing difficulty or open-mouth breathing is urgent; persistent or recurring cough merits veterinary advice. |
These are observation clues, not a reliable home diagnostic test. A crouched posture, an extended neck, or a gagging sound by itself cannot distinguish the two. Veterinary educational sources do not provide a sensitivity or accuracy figure for making this distinction by observation.

Cats swallow loose hair as they groom. Hair that is not digested can collect with digestive fluids and later be brought up. The result is often a damp, elongated wad—not necessarily a neat round ball. A visible wad of hair makes a hairball episode more plausible, but it does not mean that every repeated bout of retching is harmless. Pay attention to the overall pattern and to whether your cat seems well between episodes.

A cat may crouch and extend its neck while retching, but that posture is not exclusive to hairballs. The episode’s outcome matters: retching or vomiting that culminates in bringing up material is different from a cough that forcefully expels air and may end in gagging. Even so, observing an episode cannot establish its cause with certainty.
What a cough can look like
A cough is an expiratory action: the cat forcefully expels air from the lungs. It can sound dry or harsh and may be followed by gagging, which is one reason owners can mistake it for an attempt to bring up a hairball. A cat may crouch and stretch its neck during a cough, too. If nothing resembling a hairball appears, coughing is one possibility to discuss with a veterinarian, but the absence of hair alone does not identify the underlying problem.
VCA’s observation that cats cough less often than other animals is not a rule for diagnosing an individual cat. A cough should not automatically be dismissed as a hairball simply because the movement looks similar. In particular, a cough that persists, is severe, produces material, or recurs consistently deserves veterinary advice.
What to do during an episode
- Check breathing first. If your cat is struggling to breathe or breathing with its mouth open, seek urgent veterinary care. Do not wait to see whether a hairball appears.
- Notice whether anything is produced. If it is safe to do so, note whether you see hair, fluid, vomit, or no material. Do not put your fingers in your cat’s mouth or try to force anything out.
- Record a short video if you can do so without delaying care. A clip can help the veterinary team see the movement and sound. Also note how long the episode lasts, how often it happens, and whether breathing, appetite, or energy has changed.
- Share the pattern and your observations with the veterinary team. VCA notes that the onset and progression of a cough, the cat’s environment, and other signs can all matter in assessing it.
Useful notes are straightforward: when the episodes began, whether they are becoming more frequent or severe, how long each lasts, and whether anything is brought up. Record any changes in breathing, appetite, or energy as well. This history does not diagnose the problem, but onset, progression, environment, and other signs can help the veterinary team assess a cough. A video may preserve details of the sound and movement that are difficult to describe after the episode has passed.
Do not spend time filming or taking notes if your cat is having difficulty breathing. In that situation, urgent care comes first. Similarly, seeing a hairball once does not make later repeated or unproductive episodes automatically safe to monitor at home.
When should you contact a veterinarian?
Get urgent help for breathing difficulty
Open-mouth breathing or difficulty breathing requires urgent veterinary care. In an asthma attack, blue or gray gums or tongue are emergency signs. Do not delay care to find out whether the episode ends in a hairball. VCA’s coughing guidance identifies breathing difficulty and open-mouth breathing among the signs that need veterinary attention.

Breathing changes deserve more weight than the resemblance of an episode to retching. A cat with an apparent cough and difficult breathing should not be watched to see whether it eventually produces hair. Contact a veterinary clinic urgently and follow its guidance.
Arrange an assessment for a concerning cough
VCA advises consulting a veterinarian if a cough lasts more than a few days, is severe or productive, recurs consistently, or comes with difficulty breathing, open-mouth breathing, signs of illness, or weight loss. A productive cough is not the same as a hairball: the appearance of material does not establish the cause. Tell the clinic what you observed rather than assuming the episode was harmless retching.
“More than a few days” is a reason to consult, not a reason to wait when a cough is severe or breathing is difficult. The other listed concerns—consistent recurrence, signs of illness, or weight loss—also make it sensible to contact a veterinarian. Describe the episodes as accurately as you can: whether the cat forcefully expels air, whether anything comes up, how often it occurs, and whether the cat’s breathing or general condition has changed.
Do not dismiss repeated unproductive retching
Repeated attempts to bring something up without producing a hairball can be concerning, including for an obstruction or another illness. Cornell advises prompt veterinary attention if a cat is lethargic, refuses food for more than a day or two, has repeated unproductive retching, or is truly vomiting. See Cornell’s guidance on hairballs for these warning signs.
In particular, do not assume that repeated dry retching means your cat is simply working on a hairball. If nothing is produced and the behavior repeats, contact a veterinarian. Lethargy, refusing food for more than a day or two, and true vomiting are additional reasons Cornell identifies for prompt attention. The same caution applies even if your cat has brought up hairballs in the past: a familiar history does not explain every new episode.

Could a cough be asthma?
Feline asthma can cause coughing or hacking, wheezing, rapid or difficult breathing, and open-mouth breathing. Those signs do not prove asthma: other respiratory diseases can look similar, and a cough alone cannot establish the cause. Cornell estimates that feline asthma affects between 1 and 5% of cats; this is the Cornell Feline Health Center’s general estimate, not an individual cat’s odds. Cornell’s feline asthma guide explains that “There is no single, specific test that can definitively diagnose feline asthma.”

Instead, veterinarians assess a cat’s history alongside test results. The workup may include imaging, evaluation of airway secretions, and sometimes blood or allergy testing. The specific approach depends on the cat’s signs and clinical assessment; a video and clear notes about the episodes can help provide context.
The range of possible tests is one reason not to diagnose asthma from a cough at home. Coughing, wheezing, rapid breathing, and open-mouth breathing can fit asthma, but other respiratory diseases may look similar. A veterinarian considers the history and test results together rather than relying on one symptom or a single definitive test. The 1–5% estimate describes asthma across cats generally; it does not tell you whether a particular cat’s cough is asthma.
Can you reduce hairballs?
Hairballs form when a cat swallows loose hair while grooming. Cornell recommends daily brushing and combing to minimize them. Kittens and young cats are less prone than older cats, while long-haired breeds and shedding seasons can raise the likelihood. Regular grooming can reduce loose hair available to swallow, but it does not treat coughing or breathing problems.
A grooming brush or comb is a practical tool for making regular brushing easier. The aim is to remove loose hair from the coat, not to treat a cough or prevent a respiratory problem. If hairballs are a recurring concern, mention their frequency to your veterinarian rather than relying on grooming alone to explain every episode. Age, coat length, and shedding season can affect how likely hairballs are, but none of those factors proves that a coughing-looking episode is a hairball.
Digestive lubricants and hairball gels are supplements used to help prevent or eliminate hairballs, according to VCA’s guide to digestive lubricants and hairball gels. Ask your veterinarian before giving a product, particularly if episodes are frequent or unproductive. Cornell cautions against giving laxatives or commercial hairball diets without veterinary approval and supervision. A hairball product is not a treatment for cough or asthma.
Do not use a hairball supplement as a test of whether an episode is respiratory or digestive, and do not let trying a product delay care for repeated unproductive retching or breathing difficulty. The available guidance supports discussing digestive lubricants or gels with a veterinarian; it does not support using them to treat coughing, asthma, or an undiagnosed breathing problem.

Questions owners often ask
Can a cat cough and gag without bringing anything up?
Yes. A cough forcefully expels air from the lungs and may be followed by gagging, while producing no hairball. That can resemble retching. If the cough persists, is severe, productive, recurs consistently, or occurs with signs of illness or weight loss, VCA advises consulting a veterinarian. Difficulty or open-mouth breathing needs urgent attention.
Does a crouch or stretched neck mean it is a hairball?
No. Crouching and extending the neck can occur with either coughing or hairball retching. These movements may help you describe the episode, but they cannot identify its cause. Note whether anything is produced and how your cat is breathing, then share the pattern with your veterinary team.
When is repeated retching an emergency or a reason to call?
Open-mouth or difficult breathing calls for urgent veterinary care. Repeated unproductive retching is also concerning and warrants prompt veterinary attention, particularly if your cat is lethargic, refuses food for more than a day or two, or is truly vomiting. Do not wait for a hairball to appear if your cat is struggling to breathe.
How do veterinarians assess a possible asthma cough?
There is no single, specific test that definitively diagnoses feline asthma. A veterinarian evaluates the cat’s history and test results; assessment may include imaging, examination of airway secretions, and sometimes blood or allergy testing. Other respiratory diseases can resemble asthma, so a cough or wheeze alone is not proof.
What information should I bring to the appointment?
If it is safe and does not delay care, bring a short video and note when the episodes began, how often they happen, how long they last, and whether anything is produced. Also note changes in breathing, appetite, or energy. VCA identifies onset, progression, environment, and other signs as useful parts of a cough history.
Can grooming or hairball gel stop a cough?
No. Daily brushing and combing can help minimize hairballs by removing loose hair before it is swallowed. VCA describes digestive lubricants and hairball gels as supplements for hairballs, not cough or asthma treatments. Ask a veterinarian before using products, and get advice for frequent or unproductive episodes.

Can a cough mean my cat has asthma?
Not necessarily. Asthma can cause coughing or hacking, wheezing, rapid or difficult breathing, and open-mouth breathing, but other respiratory diseases can look similar. A veterinarian assesses history and test results; no single test definitively diagnoses asthma.
What should I record before a veterinary visit?
If doing so will not delay care, record a short video and note when episodes began, their duration and frequency, whether anything comes up, and changes in breathing, appetite, or energy. The cat’s environment and the progression of signs can also matter.
Will brushing or hairball gel treat coughing?
No. Daily brushing and combing can help minimize hairballs, while digestive lubricants and hairball gels are supplements used for hairballs. They do not treat cough or asthma. Ask your veterinarian before giving a product, especially for frequent or unproductive episodes.
The Bottom Line
A damp, often elongated wad of hair supports a hairball episode; a cough may look similar but often brings up no hairball. Crouching, neck extension, and gagging are not enough to tell the difference or identify a cough’s cause. Record the pattern and share it with your veterinarian. Seek urgent care for open-mouth or difficult breathing, and arrange veterinary advice for persistent, severe, or consistently recurring cough or repeated unproductive retching.
