There is no universal point at which concern about a cat’s health becomes “too much.” A useful guide is whether you have noticed a meaningful change from your cat’s normal behavior—and whether the sign could affect an essential function such as breathing or urination. Trust your observation, write down what has changed, and contact a veterinarian for guidance. If your cat is struggling to breathe, cannot urinate, collapses, becomes unresponsive, has a seizure, suffers serious trauma, or is vomiting severely or for a prolonged period, seek emergency veterinary care.

What makes concern worth acting on?
Compare what you see with your individual cat’s usual habits, rather than with an idea of what all cats are like. Cats may show subtle changes, and being familiar with a cat’s baseline can help caregivers and veterinarians recognize a problem earlier. The 2021 AAHA/AAFP Feline Life Stage Guidelines support understanding a cat’s normal behavior as part of care.

Changes worth discussing with a veterinarian can include increased hiding, altered eating or sleeping, litter-box changes, excessive grooming, more vocalizing, withdrawal, or lethargy. As the American Animal Hospital Association (AAHA) puts it, “If you notice that your cat is hiding more frequently than usual, that’s one clue.” The clue is a change to pay attention to, not a diagnosis on its own. See AAHA’s guide to cat anxiety signs.

A single unusual moment may be hard to interpret. What matters for a veterinary conversation is the whole picture: what changed, when it began, whether it has returned, and how different it is from your cat’s normal pattern. You do not need to prove that something is wrong before asking a clinic for advice.
When is it an emergency?
Some signs call for immediate veterinary attention rather than routine observation. Cornell University’s emergency-care guidance identifies difficulty breathing, inability to urinate, collapse, unresponsiveness, seizures, trauma, and severe or protracted vomiting as emergency examples.

Repeated straining to urinate while producing little or no urine is particularly urgent. A urinary obstruction can be life-threatening; Cornell’s feline lower urinary tract guidance describes suspected urethral obstruction as an absolute emergency. Do not wait for a home observation period when this or another emergency sign is present; contact an emergency veterinary service.

If the change is not an emergency, contact your veterinarian when it persists, recurs, or is materially different from your cat’s baseline. A change in eating, sleeping, hiding, grooming, vocalizing, energy, or litter-box habits is useful to report even when you cannot tell what caused it. AAHA’s overview of observable behavior changes can help you describe what you are seeing, but a veterinarian must assess the individual cat.

There is no validated universal threshold in these sources for when a caregiver is “too concerned,” and the examples above are not a complete triage algorithm. The right next step depends on the sign and the cat’s clinical context. If you are unsure whether something is urgent, call a veterinary clinic and describe the change; let the veterinary team advise you rather than relying on reassurance from a general checklist.

Brief written notes can make it easier to explain a change accurately, especially when details are difficult to recall during a stressful visit. Cornell’s Urgent Care guidance advises bringing written symptom notes that include when signs began and how they have changed.

- Date and time the sign began or was observed.
- What you noticed, using specific descriptions rather than a guess about the cause.
- How long it lasted, whether it recurred, and what has changed since it began.
- Appetite and litter-box observations when relevant, including straining or changes in urine output.
If a sign may be an emergency, seek care first. Notes can help you communicate, but gathering them should not postpone contacting a clinic.

What if the vet visit itself is stressful?
Tell the clinic in advance if your cat becomes very anxious during visits. The team can discuss preparation and whether veterinarian-guided medication is appropriate for that individual cat. In an AAHA article about veterinary visits, veterinary behavior expert Ilona Rodan suggests asking the clinic: “Can I get a drug, a medication to relieve the anxiety so that we can have a much happier visit?” That is a question to discuss with the veterinary team, not an instruction to give medication without veterinary guidance. See AAHA’s advice on helping cats cope with veterinary visits.
