Urinary incontinence is urine leaking involuntarily, without a cat being able to control or notice it. You may find a wet patch where your cat was resting, see dribbling while the cat walks, or notice damp fur or irritated skin. Leakage has several possible causes, and other urinary problems can look similar, so arrange a veterinary assessment rather than assuming your cat is misbehaving or treating the symptom at home. If your cat is straining but passing little or no urine—or cannot urinate—seek emergency veterinary care immediately.
What urinary incontinence looks like
The Merck Veterinary Manual defines urinary incontinence as the inability to prevent or control urination. In an incontinent cat, urine may leak constantly or intermittently without the cat realizing it. Signs can include:

- A wet patch or pool of urine where the cat has been lying or sleeping.
- Dribbling while walking or moving.
- Wet fur around the genitals.
- Skin irritation where urine repeatedly contacts the skin.
These signs suggest leakage, but they do not identify its cause. Keep urine-wet skin clean and dry, and ask your veterinarian how to limit irritation.

How leakage differs from other urinary signs
Urinating outside the litter box is not, by itself, proof of incontinence. A cat may deliberately urinate in an unusual place, or may repeatedly visit the box, strain, pass small amounts, cry out, or have blood in the urine because of a lower urinary tract problem. Behavioral urination and painful or difficult urination can both be mistaken for leakage.

Note what you actually observe: whether urine comes out while your cat is resting or walking, whether your cat appears aware of it, how often the cat tries to urinate, and whether each attempt produces a normal amount. A veterinarian can use that history and an examination to help distinguish involuntary leakage from other urinary or behavioral problems.
Why a cat may leak urine
Urination depends on the bladder storing urine and then emptying it in a coordinated way. Problems with either process can cause urinary signs. Possibilities include:

- Storage dysfunction: The urethral sphincter may not close properly, or bladder irritation may create urgency and leakage.
- Congenital differences: A urinary tract malformation, such as an ectopic ureter, can cause leakage.
- Injury or neurologic disease: Damage to urinary structures or the nerves involved in urination can interfere with storage or emptying.
- Incomplete emptying and overflow: A weak bladder contraction or an obstruction may prevent the bladder from emptying. An overfull bladder can then leak.
- Other lower urinary tract disorders: Cystitis, stones, infection, urethral plugs, and tumors can cause signs that resemble incontinence.
Feline idiopathic cystitis is a diagnosis used for lower urinary tract signs after other causes have been excluded. It is not the same as confirming urine-storage incontinence, and its cause is unknown. The visible sign alone cannot establish which problem is present.

Repeated straining with little or no urine, or inability to urinate, may indicate a urethral obstruction. Obstruction can be life-threatening and requires immediate veterinary attention. Do not wait at home to see whether straining or apparent leakage stops.

How a veterinarian investigates leakage
The diagnostic plan depends on your cat’s signs and examination; there is no single fixed test panel that identifies every cause. Your observations can help. Be ready to describe when leakage occurs, its frequency and amount, urine appearance, drinking and appetite, changes in weight, medications and diet, prior illness or injury, and what happens during litter-box visits.
The veterinarian may examine the kidneys, bladder, and genital area. A neurologic examination may be appropriate if nerve disease or difficulty emptying the bladder is suspected. Testing may include:

- Urinalysis to assess the urine. Fresh urine is important for accurate results.
- Urine culture when infection is suspected. Merck notes that cystocentesis is the preferred collection method for culture.
- Blood tests or blood-pressure measurement when the findings call for them.
- Imaging such as x-rays, contrast studies, or ultrasound to investigate the urinary tract.
- Further procedures or tests such as biopsy, cystoscopy, or neurologic testing when needed.
Not every cat needs every test. The veterinarian selects the next steps based on the history, examination, and initial findings; looking at urine at home cannot determine the underlying cause.

Treatment depends on the diagnosis
There is no general incontinence cure that is appropriate for every cat. Treatment may involve medication for a particular storage, urgency, or bladder-contraction problem, while other causes may need different medical or surgical management. The right approach depends on what is preventing normal storage or emptying, so do not give medication or supplements without veterinary direction.
If a veterinarian diagnoses feline idiopathic cystitis, management is aimed at reducing the severity and frequency of episodes. Depending on the cat, a plan may include environmental and litter-box changes, reducing stress between cats, dietary adjustments such as canned food, pheromones, and veterinarian-directed pain relief. These measures address that diagnosis; they are not a proven treatment for urinary incontinence in general.
What you can do at home while arranging care
If urine pools where your cat rests, a washable absorbent pad can protect bedding or a resting surface. It is a containment aid only: it does not diagnose or treat the cause. Clean and dry wet fur or skin, and contact your veterinarian for guidance if irritation develops. Do not let cleanup measures delay an assessment or emergency care when your cat cannot urinate.
