A cataract is a true opacity in the lens of the eye. It scatters light and can reduce vision as more of the lens is affected. A cloudy-looking eye is not enough to diagnose one. Older dogs often develop a harmless-looking haze called nuclear sclerosis, and only an eye examination can tell the two apart. If you notice cloudiness or a change in how your dog sees, book a veterinary eye exam. Surgery is the only known way to remove a cataract, and no drop or supplement has been shown to reverse one.
What a cataract is
The lens sits behind the pupil. It is normally transparent and focuses light onto the retina. A cataract is an abnormal opacity in that lens, so light is scattered instead of focused (American College of Veterinary Ophthalmologists, ACVO; VCA Animal Hospitals).
Signs and appearance
A cataract may look like a small spot, a streak, a diffuse haze or extensive whiteness. Some begin in a limited area and spread. Progression is hard to predict and varies by dog and by cause.
- Visible cloudiness or whiteness in the pupil area.
- Changes in visual behavior, such as hesitation or bumping into things.
Neither sign confirms a cataract. Cloudiness has other explanations, and vision changes can come from retinal or other eye disease.

Cataract versus nuclear sclerosis
Nuclear sclerosis is an age-related change in the lens that can look blue-gray or hazy. It is commonly mistaken for a cataract, and it is not generally thought to cause significant vision impairment (ACVO; VCA). A veterinarian can examine the lens, sometimes after dilating the pupil, to distinguish them.
| Question | Cataract | Nuclear sclerosis |
|---|---|---|
| True lens opacity? | Yes | No, an age-related change in the lens |
| Significant effect on vision? | Can be, depending on how much of the lens is involved | Generally not |
| Expected to progress? | Varies by type, breed and cause | Not generally thought to impair sight |

Causes
ACVO identifies inherited disease as a common basis for canine cataracts and diabetes as the most common disease associated with them. Other recognized causes include:

It can be difficult to name the specific cause in an individual dog. VCA notes that diabetic cataracts can progress quickly. So a dog with new cloudiness or sudden vision changes needs prompt evaluation. Do not try to diagnose diabetes or cataracts by looking at the eye.
How cataracts are diagnosed
Diagnosis needs a veterinary ophthalmic examination. VCA describes examining the internal structures of the eye with an ophthalmoscope. ACVO recommends early evaluation by a board-certified veterinary ophthalmologist when cataracts are suspected. A specialist can assess the retina and other structures before a mature opacity makes that harder.

Stages and what they mean for vision
VCA describes incipient, immature, mature and hypermature stages. Its overview offers illustrative thresholds. Cataracts covering under 30% of the lens, or affecting only one lens, rarely reduce vision. More extensive opacity is more likely to impair sight, and a fully opaque lens can blind that eye. These are clinical descriptions, not a prognosis for your dog. Progression depends on type, breed and other factors, and ACVO cautions that not all cataracts progress to full vision loss.
Will my dog go blind?
It depends. Neither appearance nor age predicts it reliably. An examination is the way to find out how much of the lens is affected and whether the retina is healthy.

Complications that need prompt attention
Advanced cataracts can cause lens-related inflammation, which can contribute to complications including glaucoma. Contact your veterinarian promptly if you see a painful red eye, apparent discomfort or sudden worsening. Do not self-medicate. Prescribed anti-inflammatory drops may control inflammation and lower the risk of complications, but they do not treat the cataract itself (VCA).

ACVO’s Dr. Nancy Bromberg, DVM, DACVO, states: “There is no proven medical treatment known to reverse or slow the progression of, or prevent the formation of a cataract.” Over-the-counter drops and supplements should therefore not be expected to clear a cataract.

Surgery
Surgery is the only known treatment that removes the cataract and can restore functional vision. VCA says: “Veterinary ophthalmologists can surgically remove cataracts and restore failing vision caused by cataracts in your pet.” ACVO’s lens overview describes phacoemulsification performed by veterinary ophthalmologists, usually with an artificial lens placed in the capsule. Pre-operative testing helps determine suitability.
Candidacy is individual. The specialist weighs:
- vision and retinal health
- other eye findings
- any underlying disease, such as diabetes
- overall health and anesthesia risk
ACVO recommends considering evaluation early. No standard recovery, success rate, cost or timeline is established for an individual dog in these sources, so ask the surgeon for figures specific to your dog.
Diabetic cataracts
VCA discusses some specialized medications for diabetic cataracts. They must be prescribed and supervised by a veterinarian and are not an over-the-counter option.
If surgery isn’t pursued
Not every dog is a candidate, and some owners decline surgery. Talk with your veterinarian about monitoring the eye, managing inflammation and watching for glaucoma.
