Official UK and US breed-health guidance does not establish a ranked list of exactly 11 common diseases in Cavalier King Charles Spaniels. It does identify eight distinct concerns or concern categories: mitral valve disease, syringomyelia, inherited eye disease, hip dysplasia, patellar luxation, episodic falling, dry eye–curly coat syndrome, and middle-ear infections. They differ in cause, severity, age of onset and quality of evidence, and a breed predisposition is not a diagnosis.
The practical response is veterinary care for symptoms and documented health screening when choosing a puppy or planning a litter. No screening result guarantees a disease-free Cavalier, especially when a condition can appear late.

What “common” means for this breed
The phrase “common health problems” can be misleading. The Royal Kennel Club, the Cavalier King Charles Spaniel Club and the American parent-club health statement identify recurring breed concerns, but the sources do not provide one current, population-level prevalence dataset or a frequency ranking. The eight concerns below are therefore presented in a non-ranked order. Some are inherited disorders with specific tests; others are problems that can affect many dog breeds but are included in breed-health guidance.

The Cavalier King Charles Spaniel Club explicitly warns: “The impression should not be given that all Cavaliers have these health conditions – they do not.” Many Cavaliers live long lives. A risk associated with the breed changes what owners and breeders should watch for; it does not predict what will happen to an individual dog.

Eight documented Cavalier health concerns
1. Mitral valve disease (MVD)
Mitral valve disease is degeneration of the valve between the heart’s left atrium and left ventricle. The Royal Kennel Club notes that MVD is common in older dogs generally but tends to begin earlier in Cavaliers. A heart murmur can be detected in a younger dog, although the timing and severity vary.
For breeding dogs in the United Kingdom, the Kennel Club heart scheme is used to grade cardiac findings. A single normal examination cannot rule out disease that develops later, so the age of the dog and the scheme’s repeat requirements matter. If a veterinarian hears a murmur, ask what follow-up examination is appropriate; do not try to interpret a murmur at home.
2. Syringomyelia (SM)
Syringomyelia occurs when fluid-filled cavities, called syrinxes, form in the spinal cord. The Royal Kennel Club describes scratching in the air near the neck—often when a dog is excited or on a lead—as a possible sign. Some dogs show few or no obvious signs, while others develop pain or neurological problems.
The cited breed guidance identifies magnetic resonance imaging (MRI) as the diagnostic scan. An unusual scratching pattern, neck sensitivity, yelping, weakness or a change in coordination warrants veterinary assessment rather than a home diagnosis. The Kennel Club is collaborating with the breed and the British Veterinary Association on a screening project; recommendations and access to MRI can differ by country.

3. Inherited eye disease
UK breed guidance names congenital and juvenile cataracts and multifocal retinal dysplasia among inherited eye concerns in Cavaliers. Cataracts affect the lens, while retinal dysplasia involves abnormal development of the retina. The source material does not establish a current prevalence estimate for either condition.
Reputable breeders have breeding stock examined under an accepted eye-testing program. Ask to see the actual certificates, the examination date and the dog’s registered identity rather than relying on a verbal assurance. Cloudiness, bumping into objects, reluctance in dim light or other vision changes should be assessed by a veterinarian; these signs have multiple possible causes.
4. Hip dysplasia
Hip dysplasia is abnormal development or looseness of the hip joint. The US parent-club health statement describes screening with radiographs, including an Orthopedic Foundation for Animals (OFA) evaluation or a PennHIP assessment. These are professional assessments of joint structure and laxity, not tests an owner can perform.

If a dog shows difficulty rising, an altered gait, reluctance to jump or exercise intolerance, a veterinarian can determine whether the hips—or another structure—are involved.

5. Patellar luxation
Patellar luxation means the kneecap moves out of its normal track. A veterinarian assesses it by physical examination; radiographs can also document the condition or associated bone changes. The severity and consequences range from an incidental finding to recurrent lameness, so an examination is needed before deciding what it means for a particular dog.
Brief skipping, carrying a hind leg, stiffness or repeated episodes of lameness should not be assumed to be “just a loose kneecap.” Several orthopedic and soft-tissue problems can look similar.
6. Episodic falling
Episodic falling is described by both UK and US breed-club sources as an inherited condition. The UK Kennel Club lists a breed DNA test among recommended pre-breeding schemes, and the US parent-club statement lists laboratories that offer testing.
An episode is not automatically ordinary fainting. Collapse, loss of balance, stiffening or abnormal movement can have cardiac, neurological, metabolic or other causes. Keep the dog safe from stairs and traffic, record what happens if you can do so without delaying care, and contact a veterinarian promptly—urgently if the episode is prolonged, repeated or accompanied by breathing difficulty.

7. Dry eye–curly coat syndrome
Dry eye–curly coat syndrome is an inherited condition covered by a UK Kennel Club DNA test and described in the US parent-club statement. The name reflects two components: inadequate tear production and an abnormal coat texture. A DNA result indicates genetic status for the tested variant; it does not replace an eye examination or establish how an individual dog is currently affected.
Redness, squinting, thick ocular discharge or obvious eye discomfort needs veterinary attention. Eye problems can worsen quickly, and treatment decisions depend on an examination of tear production and the eye surface.
8. Middle-ear infections
The American Kennel Club’s Cavalier overview includes middle-ear infections among conditions that may affect the breed. The supplied guidance does not give a frequency estimate or a detailed diagnostic protocol. Middle-ear disease can be painful and may affect hearing or balance, so head shaking, ear scratching, odor, discharge, pain when the ear is touched or a head tilt should be evaluated by a veterinarian.

| Concern | Method named in guidance | What the sources establish |
|---|---|---|
| Mitral valve disease | Veterinary cardiac examination and the UK Kennel Club heart scheme | Heart screening is recommended for breeding dogs in the UK; late onset can complicate screening. |
| Syringomyelia | MRI | The Royal Kennel Club identifies MRI as the diagnostic scan and describes ongoing screening collaboration. |
| Inherited eye disease | Formal eye examination | UK best-practice guidance includes eye testing; no current prevalence figure is supplied. |
| Hip dysplasia | Radiographs, including OFA evaluation or PennHIP assessment | These methods are described by the US parent-club statement; no Cavalier prevalence figure is supplied. |
| Patellar luxation | Veterinary orthopedic examination, with radiographs when indicated | The condition can be assessed clinically and documented radiographically. |
| Episodic falling | DNA test for breeding decisions | The UK and US sources identify inherited status and testing laboratories. |
| Dry eye–curly coat syndrome | DNA test for breeding decisions | The UK Kennel Club recommends testing before breeding; the US statement describes inheritance. |
| Middle-ear infections | Veterinary ear examination | The AKC overview names the problem but does not specify a breed screening scheme. |
A “clear,” “normal” or favorable result is specific to the test performed. It does not certify that a dog has no other disease, and it cannot guarantee that a late-onset disorder will never develop.
Questions to ask a Cavalier breeder
Ask for records for both parents, not just a statement that the parents were “health tested.” The Cavalier Club advises prospective buyers to request proof of recommended tests and cautions that testing and adherence to breeding guidance cannot guarantee a puppy will be free of genetic problems.
- Which organization issued each result—the UK Kennel Club, a US parent-club program, OFA, PennHIP or another recognized scheme?
- What was the examination date, the dog’s registered identity and the evaluator’s qualification?
- For cardiac and eye examinations, was a repeat schedule required, and has it been followed?
- Were DNA results reported as clear, carrier or affected under the laboratory’s terminology?
- Were hip and patella findings recorded in the relevant registry?
- What is known about the parents, grandparents and older relatives, especially for late-onset MVD or syringomyelia?
- Will the breeder provide copies of certificates and explain any limitations rather than promising a guarantee?
Late-onset MVD and syringomyelia can become evident after a dog has been used for breeding. Family history and transparent records therefore add context, but they still cannot predict an individual puppy with certainty.
When an owner should call the veterinarian
Arrange a veterinary appointment for a new murmur, repeated coughing or reduced stamina; air-scratching near the neck, pain or neurological change; vision changes; persistent lameness or difficulty rising; recurrent collapse or unusual episodes; red or painful eyes; or ear pain, discharge, head shaking or balance changes. Seek urgent care for breathing difficulty, prolonged collapse, severe pain, sudden inability to walk or rapidly worsening neurological signs.
Bring a timeline, medication list and video of an episode when safe. A video can help a veterinarian distinguish movement patterns, but it is not a substitute for examination or testing.
Keeping the risk information in perspective
These eight concerns are useful for informed ownership and responsible breeding, not for labeling every Cavalier as sick. The official club guidance emphasizes that many dogs do not develop the listed conditions. Work with a veterinarian who can tailor examinations to the dog’s age, history and findings, and use the screening program recognized in your country when evaluating breeding stock.
No named source establishes a verified, comparable prevalence ranking for exactly 11 Cavalier diseases. Treat online lists that present precise percentages without a defined population, date and method with caution. A documented test result, a complete family history and prompt veterinary attention to symptoms are more actionable than an unsupported number.
