Cholangitis/Cholangiohepatitis Syndrome in Cats: Causes, Symptoms, and Treatment

Cholangitis is inflammation of the bile ducts; cholangiohepatitis means inflammation has also extended into nearby liver tissue. In cats, these terms describe a group of related disorders—not one uniform disease—and symptoms alone cannot tell which form a cat has. Jaundice, repeated vomiting, refusal to eat, or marked lethargy warrant prompt veterinary assessment.

What is feline cholangitis/cholangiohepatitis syndrome?

Feline cholangitis/cholangiohepatitis syndrome (CCHS) covers inflammatory disorders affecting the bile ducts and, in some cases, surrounding liver tissue. The Merck Veterinary Manual describes it as the most common acquired inflammatory liver disease in domestic cats, but that umbrella term includes different patterns with different likely causes and treatment approaches. The two broad patterns owners are most likely to hear about are acute suppurative (often called neutrophilic) disease and chronic nonsuppurative (often called lymphocytic) disease. These labels describe the type of inflammation found in tissue; they are not diagnoses that can be made from symptoms alone.

What causes CCHS, and how do the main forms differ?

Form Typical pattern and context General treatment emphasis
Acute suppurative/neutrophilic Often associated with bacterial infection involving the biliary system and can cause sudden, overt illness. Stagnant bile or a blockage can raise concern for infection. Veterinary care may include antibiotics and supportive treatment; suspected obstruction may also need attention.
Chronic nonsuppurative/lymphocytic May develop slowly or cause fluctuating, subtle signs. An immune-mediated component may be involved. May involve immune-modulating medication and longer-term monitoring.

These are broad patterns, not rules for every cat. Bacterial culture and cytology can help guide decisions when infection is suspected, while tissue evaluation may be needed to identify the inflammatory pattern. Cornell also notes that inflammation of the pancreas and intestines may occur alongside biliary disease. In some tropical or subtropical regions, liver flukes are another possible context: free-roaming cats that eat lizards, geckos, or toads may be exposed where those parasites occur. What symptoms should cat owners watch for?

  • Reduced appetite or refusal to eat
  • Vomiting, which may be intermittent or repeated
  • Weight loss or listlessness
  • Fever, dehydration, or abdominal discomfort
  • Jaundice—a yellow tinge to the skin, gums, or whites of the eyes

Acute suppurative disease can make a cat suddenly and seriously ill. Lymphocytic disease may begin more subtly, fluctuate, or progress gradually. These signs also occur with other illnesses, so they cannot confirm CCHS or distinguish its form.

When to contact a veterinarian urgently

Seek prompt veterinary care if your cat is jaundiced, repeatedly vomiting, not eating, markedly lethargic, or acutely unwell. Severe illness may require hospitalization. How do veterinarians diagnose cholangitis or cholangiohepatitis?

Evaluation commonly begins with a physical examination, blood tests, and abdominal ultrasound. Bloodwork may show liver-enzyme changes or elevated bilirubin, but findings vary and are not specific to one inflammatory form. Ultrasound can help assess the liver, gallbladder, and bile ducts, look for possible obstruction, and identify concurrent disease. These tests can support a working diagnosis without settling exactly what type of inflammation is present. Liver biopsy with histopathology is identified by Merck as the definitive diagnostic method. A veterinarian may instead make a presumptive diagnosis from the cat’s signs, bloodwork, and ultrasound if biopsy is not feasible. Whether to pursue tissue sampling depends on the likely benefit of a clearer diagnosis and the cat’s health and procedural risks; it is a decision to discuss with the veterinary team, not a test every cat can automatically undergo. What treatment might a cat need?

Care depends on the suspected form, test results, severity, and whether obstruction or another illness is present. Treatment is veterinary-directed; there is no single medication or home-care plan that suits every cat.

Acute suppurative disease

A seriously ill cat may need hospitalization, fluids and electrolyte management, anti-nausea treatment, nutritional support, and antibiotics. The veterinarian selects the antibiotic and its duration based on factors such as culture results and the cat’s response.

Chronic lymphocytic disease

Treatment may include an immune-modulating medicine, such as a corticosteroid, adjusted gradually and paired with monitoring. The plan depends on the individual cat and the diagnostic findings.

Obstruction and nutritional support

If bile flow is blocked or there is a gallbladder problem, surgery may be considered in selected cases. A cat that cannot eat enough may need nutritional support, including a feeding tube; the veterinary team should choose the feeding plan and diet. Can CCHS occur with other illnesses, and what is the outlook?

Cholangitis may occur alongside pancreatitis and inflammatory bowel disease, a combination often called triaditis. These conditions can share signs such as poor appetite and vomiting, and concurrent disease may also involve diarrhea or abdominal pain. Outlook varies with the inflammatory form, how promptly care begins, whether bile-duct obstruction is present, and the extent of liver injury. Cornell notes that neutrophilic cases may recover with prompt, aggressive treatment, while obstruction can worsen the outlook. Some cats with lymphocytic disease may achieve long-term remission; others need ongoing treatment and monitoring.
