Coccidia in Cats: How Infection Spreads, When It Causes Illness, and What Vets Do

Coccidia can infect cats without making them sick. Learn how infection spreads, when diarrhea needs veterinary attention, and how diagnosis, treatment, and cleanup work.

Coccidia are microscopic intestinal parasites; cats can carry and shed them without appearing sick. Diarrhea is the main sign when infection causes illness, especially in kittens, and a veterinarian confirms the diagnosis with fecal testing interpreted alongside the cat’s age, history, and symptoms. Treatment may not be needed for every infection, but an unwell or dehydrated cat may need veterinary care and supportive treatment.

What are coccidia in cats?

The common feline coccidia are Cystoisospora felis and Cystoisospora rivolta. Older references often use the name Isospora. These are single-celled parasites that infect the intestine. A cat may shed parasite eggs, called oocysts, in its feces without showing signs of illness; finding infection and finding the cause of a cat’s symptoms are not always the same thing. How do cats get coccidia?

Cats become infected by swallowing an infective oocyst from contaminated surroundings. Oocysts passed in feces need time in the environment to develop into the infective stage, and environmental conditions affect how quickly that happens. The parasite can also move through an indirect route involving transport hosts such as rodents; Cornell notes that cats may acquire infection through insects carrying cysts as well. Exposure does not guarantee that a cat will become ill.

Which cats are most likely to become sick?

Many infected cats have no obvious symptoms. Clinical illness is a greater concern in kittens, particularly around weaning or other environmental stress, and in cats living in crowded settings. Concurrent illness or a suppressed immune system may also make disease more likely. Merck Veterinary Manual’s coccidiosis chapter was last updated in August 2026.

What are the symptoms of coccidia in kittens and cats?

Diarrhea is the main sign. It may be watery or contain mucus. In more severe cases—particularly in kittens—owners may also notice:

  • Blood in the stool
  • Dehydration, weight loss, or abdominal discomfort
  • Vomiting or reduced appetite
  • Low energy, weakness, or lethargy

These signs do not establish coccidiosis on their own: diarrhea has many possible causes. Contact a veterinarian promptly if a kitten has diarrhea, or if any cat has blood in its stool, poor appetite, vomiting, weakness, or signs of dehydration. Seek veterinary advice as well when diarrhea is significant or persistent.

How do vets test for coccidia?

A veterinarian considers the cat’s age and history, clinical signs, and fecal test findings together. Oocysts are microscopic, so looking at stool at home cannot confirm the infection. Testing options described by CAPC include centrifugal fecal flotation, antigen assays, and PCR; Merck also describes fecal examination for oocysts. These methods have different strengths and weaknesses, so a veterinarian chooses and interprets tests based on the case. CAPC notes that antigen testing may be combined with microscopic examination to broaden parasite detection.

A positive fecal result does not necessarily mean coccidia explain every symptom: some cats shed oocysts without illness, and another condition may be present at the same time. When diarrhea is severe or does not resolve, the veterinarian may need to investigate other infectious or noninfectious causes too.

How is coccidia treated?

Treatment depends on whether the cat is sick, how severe the illness is, and the veterinarian’s assessment. Merck says many infections in kittens and puppies resolve spontaneously, so medication may not always be necessary. A cat that is affected may need warmth, hydration, and fluid or nutritional support. A veterinarian can determine whether antiparasitic medication is appropriate.

Medication status varies by country. CAPC identifies sulfadimethoxine as the only label-approved drug for enteritis associated with coccidiosis in its guidance, while Merck discusses additional veterinary options and cautions that licensing varies by country. Do not use a medication or dose without veterinary direction; the veterinarian can advise what is suitable and licensed where you live.

How do I clean up after a cat with coccidia?

Remove feces promptly and keep litter boxes and animal areas clean. Good sanitation helps limit environmental contamination, particularly where several animals share space. Wash your hands after handling feces or cleaning a litter area. The cited veterinary guidance supports prompt feces removal and general sanitation, but does not establish that a particular household cleaner or consumer product eliminates coccidia.

Can cat coccidia infect humans?

No. Feline Cystoisospora is not considered transmissible to people: Cornell University’s Feline Health Center states, “Isospora of cats cannot cause disease in humans.” This is specific to feline coccidia, not a reason to skip routine hand hygiene around cat feces. Other organisms found in cat feces, including Toxoplasma gondii, are zoonotic and are distinct from feline coccidia.
