Cryptococcus in Cats: Causes, Symptoms, and Treatment

Cryptococcosis is a fungal infection cats acquire from the environment, usually starting in the nose. Here are the causes, signs, diagnostic tests, and why treatment must be managed by a veterinarian.

Cryptococcosis is a fungal infection cats pick up from their environment, most often by breathing in fungal particles. It usually starts in the nose, but it can also involve the skin, eyes, and central nervous system (the brain and spinal cord). Diagnosis, choice of medication, and treatment length all need to be handled by a veterinarian, and treatment commonly lasts months to years rather than a few weeks.

What causes cryptococcosis in cats

The organisms

The disease is caused by fungi in the Cryptococcus complex. The Merck Veterinary Manual professional reference names Cryptococcus neoformans and C. gattii among the species involved. These fungi live in the environment rather than in cats, and they are commonly found in soil, decaying vegetation, and bird droppings, particularly pigeon droppings.

How cats become infected

Cats most often acquire the infection by inhaling fungal particles from a contaminated environment. Infection can also occur when the fungus enters through a contaminated wound. The European Advisory Board on Cat Diseases guideline (updated June 11, 2024) lists avian guano and decaying vegetation as sources, which is consistent with the Merck description. Because the fungus is environmental, the disease is not tied to a single region or to one type of household, although exposure is more likely in cats that spend time outdoors.

Incubation and contagion

The Merck Veterinary Manual professional reference (review dated April 2023) reports an incubation period of 2–13 months. That range describes reported cases in general. It is not a timeline for any individual cat, and it means signs can appear long after an exposure the owner never noticed.

The European Advisory Board on Cat Diseases guideline describes cryptococcosis as non-contagious between animals and environmentally acquired. An infected cat does not ordinarily pass the infection to housemates or other cats. The shared risk is the environment where the fungus grows, not the cat itself.

Symptoms owners may notice

Nasal disease is the most common presentation. Other body systems can also be affected, sometimes without any nasal signs at all. The table below groups the signs reported in the Merck pet-owner reference (Fungal Infections in Cats, updated September 2024) and the Cornell Feline Health Center’s respiratory infection information.

Nose and sinuses Sneezing; long-lasting discharge that may be clear, bloody, mucus-like, or pus-filled; a polyp-like mass in one nostril; firm swelling beneath the skin over the nose
Skin Raised bumps or nodules that may be soft or firm; larger lesions may break open (ulcerate)
Eyes Dilated or unresponsive pupils; retinal detachment; blindness
Nervous system Depression; behavior changes; seizures; circling; paresis (partial paralysis or weakness)
General Possible voice changes, noisy breathing, reduced appetite, and weight loss (Cornell Feline Health Center)

None of these signs is specific to cryptococcosis. Persistent nasal discharge or facial swelling has several possible causes, including other infections, foreign material, and growths. Only a veterinary examination can determine whether fungal testing or imaging is needed.

Contact a veterinarian promptly if your cat has any of the following:

  • Nasal discharge or sneezing that lasts more than a few days or keeps returning
  • Swelling on the face or over the nose, or a lump visible inside a nostril
  • Skin lumps that persist, enlarge, or open
  • Changes in the eyes, such as a dilated pupil that does not respond to light, or sudden vision loss
  • Seizures, circling, weakness in the legs, or unusual behavior

How a veterinarian confirms the diagnosis

Diagnosis usually combines examination of affected material with blood or fluid testing. The veterinarian chooses tests based on where the disease appears and how far it may extend.

Antigen testing is also the basis for monitoring treatment, which is covered below. A single test result is not a diagnosis on its own; the veterinarian interprets it with the cat’s signs and examination findings.

Treatment

Treatment is individualized. It depends on which body systems are affected and how severe the disease is, so there is no single protocol for every cat.

Antifungal medications

The Merck Veterinary Manual professional reference lists fluconazole and itraconazole as treatment choices. Which drug is used, at what dose, and for how long are decisions for the veterinarian, based on the case. Owners should not start or adjust antifungal medication without veterinary direction. Merck advises that liver enzyme activity be monitored during treatment, which is one reason regular blood work matters.

Surgery and nervous-system care

Some cats need surgical removal or debulking (reduction of bulk) of nasal or facial lesions. When the central nervous system is involved, the veterinarian may need to add other treatment. Merck notes that a short course of a glucocorticoid (a corticosteroid) may be used in some cases of central nervous system disease, typically for the first days to weeks. This is a clinician decision and is not a home measure.

Duration and monitoring

Treatment is long. The Merck professional reference states:

“Treatment should continue until at least two cryptococcal antigen tests are negative, which may be months to years.”

This is that reference’s guidance on when to stop, and it means an improvement in visible signs does not by itself end treatment. Stopping early is the main risk the veterinary references highlight, so follow-up antigen testing should be scheduled in advance and kept.

Outlook

Cornell Feline Health Center states that the outlook is generally favorable when the disease is diagnosed early and the central nervous system is not involved. Cornell also stresses that owners need to keep up with treatment, because therapy can last months to years. The outlook depends on how far the infection has spread and how the cat responds, so no fixed cure rate or treatment length applies to every case.

Prevention and reducing exposure

The practical focus is reducing exposure. Cornell notes that keeping cats indoors minimizes exposure, because outdoor cats encounter C. neoformans more often. This lowers risk but does not guarantee prevention, since cats can be exposed indoors as well. Reasonable precautions include keeping cats away from pigeon droppings and decaying vegetation and having any cat with unexplained nasal or skin problems examined early.

The veterinary references reviewed here do not describe a vaccine or a commercial product that prevents feline cryptococcosis.

What to do next

  1. Book a veterinary examination and describe the signs, how long they have lasted, and whether the cat has been outdoors or near bird droppings.
  2. Ask which tests are recommended for your cat’s signs, including whether cryptococcal antigen testing or imaging is appropriate.
  3. If a diagnosis is made, ask the veterinarian to outline the medication plan, the expected monitoring schedule, and the test that will decide when treatment can stop.
  4. Keep every follow-up appointment and blood test, even if your cat appears well.
