Regurgitation and vomiting can look almost identical from across the room, but they start in different places and involve different effort. Regurgitation is usually a passive return of food or fluid from the esophagus, often within moments of eating or drinking, without the heaving that comes with vomiting. Vomiting is an active expulsion of material from the stomach, usually preceded by signs of nausea. Telling them apart is a useful first step, but it is a clue for your veterinarian, not a diagnosis you can make at home.
The core difference: effort and where the material comes from
The distinction rests on two things: whether the cat works to bring material up, and whether that material has reached the stomach. VCA Animal Hospitals states that vomiting typically involves abdominal contractions and effort, while regurgitation typically occurs quickly, without abdominal contractions, and often immediately after eating or drinking. Merck’s professional overview of the digestive system makes the same point from the anatomy side: regurgitation may signify disease of the mouth-throat region or the esophagus and is not accompanied by the premonitory signs seen with vomiting (Merck Veterinary Manual).

The appearance row deserves caution. Undigested food can be found in both cases, and a cat that vomits quickly after a meal can look like it is regurgitating. Effort and pre-episode behavior are more reliable clues than what ends up on the floor.

- Timing relative to food or water: Did the material come up within moments of eating or drinking, or hours later?
- Body effort: Did your cat hunch, stretch, or heave with visible abdominal contractions?
- Warning behavior: Did you see lip licking, drooling, repeated swallowing, or restlessness first?
- Posture: Did your cat extend the head and neck, or seem to have trouble getting food down?
- Coughing versus vomiting: Coughing can be mistaken for vomiting. A cat may cough up froth and swallow it, so note whether anything was actually expelled.
- Frequency and other changes: How often it happens, and any change in appetite, energy, breathing, drinking, or stool.
VCA suggests that a short video of an episode can help your veterinarian distinguish coughing, regurgitation, and vomiting, so recording one is worth the effort if it can be done safely.
What may be behind regurgitation
Regurgitation points toward the mouth, throat, or esophagus. It is a symptom of several possible disorders, and none can be confirmed from timing or appearance alone.
Esophagitis
Merck lists feline esophagitis among esophageal disorders. It can be associated with certain medications, foreign objects, or reflux of stomach contents. Signs may include regurgitation, drooling, repeated swallowing, pain, reduced appetite, or extending the head and neck (Merck Veterinary Manual, cat owner edition, last updated September 2024).

Esophageal dysmotility and diverticula
Abnormal movement of the esophagus, known as dysmotility, and small pouches called diverticula are also listed by Merck among feline esophageal disorders. Each can cause food to pool or come back up rather than moving on to the stomach.
Megaesophagus
In megaesophagus, the esophagus becomes enlarged and loses movement, which makes it hard to move food and liquid into the stomach. VCA describes regurgitation as the most common sign of this condition (VCA Animal Hospitals, Megaesophagus).
Congenital esophageal abnormalities in kittens
Some kittens are born with esophageal abnormalities. Merck notes that regurgitation and swallowing problems may become evident when kittens begin eating solid food, and that regurgitated material can enter the lungs and cause pneumonia (Merck Veterinary Manual, congenital and inherited disorders, cat owner edition). A kitten that regurgitates repeatedly at weaning should be examined promptly.

What vomiting can mean
Vomiting has a much wider range of possible causes. VCA lists dietary indiscretion, parasites, hairballs, infection, obstruction, toxins, pancreatitis, and disease of the kidneys, liver, thyroid, or intestines, as well as inflammatory bowel disease and cancer. Because the list is so broad, persistent or unexplained vomiting needs a veterinary assessment rather than guesswork.
Hairballs are a common cause of occasional vomiting in cats, but they are not automatically harmless. If hairball vomiting is frequent, or comes with the warning signs listed below, it should be discussed with your veterinarian rather than assumed to be normal.

- Fever or lethargy
- Loss of appetite
- Abdominal pain
- Diarrhea
- Signs of dehydration
- Blood in the vomit
- Suspected ingestion of a foreign object or toxic substance
VCA also advises a veterinary visit when otherwise well-appearing vomiting continues for more than one to two days.
Regurgitation that needs a call
Contact your veterinarian when regurgitation happens repeatedly, or when it comes with trouble swallowing, drooling, pain, reduced appetite, or breathing problems. Esophageal disorders can affect swallowing and may carry a risk of aspiration, where material enters the airway and lungs.
Do not wait it out if your cat is weak, distressed, unable to swallow, or having trouble breathing. Those signs warrant urgent care.

No universal safe number of episodes or duration has been established for feline regurgitation in the owner-facing sources reviewed here. Describe the pattern to your veterinarian, especially if it recurs.

The veterinarian begins with your history and a physical examination, then chooses tests for the suspected problem. Not every cat needs every test.
- Vomiting: Blood and urine tests, radiographs, ultrasound, and endoscopy with biopsy, depending on the picture (VCA). Cornell’s feline health guide describes a similar sequence: history and examination, then bloodwork and fecal testing, with X-rays, ultrasound, or biopsy when needed (Cornell Feline Health Center, last updated 2021).
- Suspected esophageal disease: Merck identifies fluoroscopy and endoscopy as preferred methods for some conditions, and contrast radiographs can help identify diverticula (Merck Veterinary Manual).
Management depends on the diagnosis
Treatment follows the cause, so there is no single home fix for regurgitation. For some esophageal conditions, Merck describes soft, small, frequent meals. For small esophageal diverticula, it notes that a cat may eat upright, with forelegs raised above the rear legs, and stay in that position briefly after eating. An elevated cat feeding station is relevant only when a veterinarian recommends that position for a diagnosed diverticulum. It is not established as a way to prevent ordinary regurgitation or to treat an undiagnosed cat.
Avoid starting over-the-counter or human medicines, changing the diet on your own, or withholding food and water as a blanket response. Follow your veterinarian’s plan for any confirmed condition.
