Diabetic Ketoacidosis in Cats: Signs, Diagnosis and Emergency Care

Diabetic ketoacidosis is a life-threatening emergency in cats. Learn the warning signs, how veterinary teams diagnose and treat DKA, and what to do if you suspect it.

Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes in cats. If your cat may have DKA—especially if they are diabetic and suddenly become weak, stop eating, vomit, or have trouble breathing—call a veterinarian or emergency veterinary hospital and seek care now. DKA cannot be confirmed or safely treated at home; affected cats generally need hospital treatment.

What is diabetic ketoacidosis?

DKA happens when a cat does not have enough effective insulin to use glucose normally. The body turns to fat for energy, and the liver produces ketones. As ketones build up, they can contribute to blood acidity, dehydration, and electrolyte disturbances. DKA can be the first sign of diabetes, but it can also develop in a cat already receiving diabetes treatment.

It is a serious illness, not simply a high blood sugar reading. Dehydration, changes in blood chemistry, and another illness may all be part of the problem. The American Animal Hospital Association’s 2026 feline diabetes guideline describes DKA as a life-threatening complication.

What signs can DKA cause in cats?

Possible warning signs include:

  • Eating less than usual or refusing food
  • Vomiting
  • Lethargy, weakness, or seeming acutely unwell
  • Increased thirst and urination
  • Trouble breathing

These signs are not unique to DKA, and a cat may not show every sign. They cannot tell you at home whether DKA is present. In a diabetic cat—or one who may have diabetes—contact a veterinarian immediately if these symptoms appear. Merck Veterinary Manual’s owner guidance identifies DKA as an emergency requiring immediate treatment and hospitalization.

What should you do if you suspect DKA?

  1. Call your veterinarian or an emergency veterinary hospital now. Explain that your cat has diabetes, if known, and describe the symptoms and when they began. Follow the clinic’s instructions about arriving for urgent assessment.
  2. Do not wait for symptoms to pass or try to correct the problem at home. DKA needs veterinary assessment and usually hospital care. Do not change insulin doses, give extra insulin, or attempt fluid or electrolyte treatment unless a veterinarian specifically directs you.
  3. Do not use a ketone strip or meter result as a substitute for an examination. Ketone results alone cannot establish whether a cat has the acidosis and other abnormalities that define DKA.

How do veterinarians diagnose DKA?

Veterinarians assess the cat’s condition and look for compatible illness together with evidence of both ketosis (ketones in the body) and acidosis. They may use blood tests and other diagnostic checks to assess hydration, blood chemistry, and possible concurrent illness. A ketone result by itself is not enough to confirm DKA: for example, ketones in the urine of a cat that otherwise seems well do not establish the diagnosis without matching clinical and biochemical findings.

Blood and urine ketone tests measure different things

A blood ketone meter measures beta-hydroxybutyrate (BHB), while standard urine ketone strips measure acetoacetate. Both can help a veterinary team assess ketones, but the AAHA guideline says BHB testing more accurately identifies earlier ketoacidosis. A urine strip does not measure acidosis, and a result should not be used on its own to rule DKA in or out or decide on treatment.

How is DKA treated?

Treatment is generally provided in hospital and tailored to the cat’s examination and test results. The veterinary team works to rehydrate the cat, address electrolyte abnormalities, use insulin to stop ketone production, and investigate and treat any illness that may have triggered or complicated the episode.

Fluids, insulin, and close monitoring are central parts of care. Potassium and phosphorus need particular attention, alongside blood glucose and other electrolyte measurements. Fluids alone may improve some abnormalities, but insulin is still required to treat DKA; the AAHA guideline notes that it is required even in uncommon euglycemic DKA associated with SGLT2 inhibitor therapy. Severe cases may need referral to a 24-hour facility. Treatment protocols and insulin routes vary, so dosing and fluid decisions belong to the veterinary team.

Could another illness have triggered DKA?

Yes. A concurrent condition can contribute to a diabetic cat becoming critically ill. The AAHA guideline lists pancreatitis, urinary tract or other infections, hypercortisolism, and kidney disease among possible precipitating conditions. The veterinary team may therefore investigate and treat more than the blood glucose or ketones alone.

What happens after a cat is stabilized?

After the acute episode, work closely with your veterinarian on the diabetes plan and on any monitoring they recommend. Cornell’s feline diabetes guidance suggests owners may track daily water intake, urine output, appetite, body weight, activity, and the amount of insulin or other medication given; glucose monitoring may also be recommended. These observations help with ongoing diabetes management, but they are not a way to treat an active DKA episode at home.

The sources cited here establish that DKA is serious and requires hospitalization, but do not provide one survival percentage that applies to every cat. Your veterinarian can discuss your cat’s condition and the factors affecting their individual outlook.
