A dog that has been hit by a car needs an urgent veterinary examination even if the obvious problem is a limp. A collision can dislocate the hip, fracture the pelvis or femur, damage the femoral head, or cause life-threatening injuries elsewhere. Radiographs and, when necessary, CT help identify the injury. Surgery is recommended when the joint or fracture cannot be restored and kept stable with nonsurgical treatment, but the exact operation and recovery plan depend on the individual dog.
Until a veterinarian has assessed the dog, keep movement to a minimum, support the body during transport, and treat the incident as a possible multiple-trauma emergency rather than an isolated hip problem.

What to do immediately after a car strike
Move the dog away from traffic only if you can do so safely. Approach slowly: pain and shock can make a normally gentle dog bite. Keep the head, neck and spine as straight as practical, and use a rigid board, blanket or large towel as a stretcher for a dog that cannot walk. Do not manipulate the hip, pull on a leg, or try to “pop” the joint back into place.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problems- Call an emergency veterinary hospital while arranging transport.
- Apply gentle pressure with clean cloth to an actively bleeding external wound.
- Keep the dog warm, quiet and as still as possible, but do not overheat a dog that is panting or struggling to breathe.
- Do not give food, water, human pain medicine or leftover prescriptions unless a veterinarian specifically directs you. An anesthetic procedure may be needed, and many human drugs are toxic to dogs.
- Tell the hospital when the collision occurred, whether the dog lost consciousness, and anything you observed about breathing, urination, bleeding or vomiting.
Collapse, pale or gray gums, labored breathing, a swollen abdomen, inability to urinate, uncontrolled bleeding or rapidly worsening weakness are emergency signs. They can occur with chest, abdominal, head or urinary-tract trauma even when the leg injury looks like the main issue.

What “a hip problem” can mean after a collision
The title does not identify a diagnosis. A veterinarian may find one injury or several injuries at the same time.
Traumatic hip dislocation (luxation)
The hip is a ball-and-socket joint: the femoral head is the ball and the acetabulum is the pelvic socket. A forceful impact can displace the femoral head from that socket. Dogs commonly have severe hind-leg lameness or refuse to bear weight, but those signs also occur with fractures and other damage.
Radiographs confirm whether the hip is dislocated, show the direction of displacement, and reveal an associated fracture. Under general anesthesia, a surgeon may first attempt a closed reduction, meaning the femoral head is placed back into the socket without opening the joint. If the joint will not stay reduced, if important soft tissues are damaged, or if a fracture is present, an operation to stabilize or reconstruct the joint may be needed.
Pelvic or femoral fracture
Motor-vehicle trauma frequently causes fractures of the pelvis. A fracture can involve the socket, the weight-bearing portions of the pelvis, or other pelvic bones. The upper femur can also break, and a fracture may coexist with a dislocation. Treatment ranges from carefully managed confinement for selected stable fractures to surgical fixation with pins, screws, plates or other implants chosen for the fracture pattern.
Femoral-head damage and secondary joint problems
The impact can severely damage the femoral head or the cartilage around the hip. Scar tissue, altered joint mechanics and persistent pain may remain after the original injury. A University of Illinois Veterinary Teaching Hospital case published on September 27, 2020, describes Spike, a five-month-old Doberman struck by a car. He had multiple pelvic fractures and severe femoral-head damage, underwent orthopedic surgery with pins used to stabilize the pelvis, and later required rehabilitation for pain, lameness and scar tissue. This is an individual case report, not a prediction of another dog’s outcome.

How the veterinary team finds the injury
Stabilization before orthopedic treatment
Clinicians generally assess immediate threats first: breathing and circulation, neurologic status, bleeding, abdominal injury and the ability to pass urine. Cornell University’s orthopedic service notes that dogs and cats hit by vehicles may have injuries to the head, chest, abdomen and urinary tract in addition to fractures. Treating a hip before checking those systems can be dangerous.
Physical and orthopedic examination
Once the dog is stable enough, the veterinarian evaluates posture, limb alignment, pain, pulses, sensation and joint motion. A painful dog may need sedation for a safe, thorough examination. The pattern of pain and limb position can suggest a dislocation, but it cannot replace imaging.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Radiographs and CT
Plain radiographs are the standard way to confirm a hip dislocation and diagnose many pelvic and femoral fractures. Multiple views are usually needed. CT may be recommended when fractures are complex or when the team suspects several traumatic injuries that radiographs cannot fully define. Blood tests, urine testing and other scans may be added according to the dog’s condition.
Why surgery may be recommended
Surgery is not automatically required for every post-collision hip injury. The decision turns on anatomy, stability, pain, the dog’s overall health and the likelihood of useful function with each option.
| Finding | Possible approach | What determines the choice |
|---|---|---|
| Hip dislocation that can be replaced and remains stable | Closed reduction under general anesthesia, followed by a support or restricted activity if prescribed | Whether the femoral head seats correctly and stays in the socket; associated fractures and soft-tissue damage |
| Dislocation that cannot be kept in place | Surgical reduction and stabilization using a technique selected by the orthopedic surgeon | Direction and duration of the luxation, joint damage, body shape and other injuries |
| Pelvic or femoral fracture | Confinement for selected stable injuries or surgical fixation for unstable, displaced or weight-bearing fractures | Fracture location, displacement, pelvic canal involvement, pain and the dog’s ability to function |
| Severe femoral-head or hip-joint destruction | A reconstructive or salvage procedure may be considered | Extent of damage, age and size, concurrent injuries, surgeon expertise and expected function |
The American College of Veterinary Surgeons describes several surgical stabilization methods for hip luxation. There is no universally “best” technique that can be selected from the accident description alone. A surgeon must match the method to the dog’s imaging and physical findings.

Questions to ask before consenting to an operation
Ask the treating veterinarian or orthopedic surgeon to show you the imaging and explain the diagnosis in plain language. Useful questions include:
The Tool Desk
Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →- Is the primary problem a dislocation, a fracture, femoral-head damage, or more than one of these?
- Are the chest, abdomen, head and urinary tract clear, and were additional tests needed?
- Can the joint be reduced without opening it, and if so, how likely is it to stay reduced?
- What procedure is being proposed, what implants or supports will be used, and what are the important complications?
- How much weight may the dog put on the limb during each stage of healing?
- What signs would require an immediate call or an emergency return?
- Is a rehabilitation referral recommended, and when should it begin?
Ask for written discharge instructions, medication labels and a recheck schedule. If cost is a concern, request an itemized estimate that separates emergency stabilization, imaging, anesthesia, surgery, hospitalization, medications and rehabilitation. The available clinical guidance does not establish a universal price for these injuries, and costs vary substantially by region, hospital, procedure and complications.
Preparing for surgery and anesthesia
The hospital may stabilize shock, control pain, treat wounds, place a urinary catheter or address other injuries before orthopedic surgery. A dog with chest or abdominal trauma may need observation before anesthesia is considered safe. Follow fasting instructions exactly; do not withhold water or food beyond the period the hospital specifies.
Tell the team about every medication, supplement, prior illness and possible toxin exposure. Bring any records you have. Ask whether an overnight stay is expected and who to contact after hours. Do not delay emergency care while searching for a particular surgeon or equipment.

The operation determines the restrictions. A dog recovering from a stabilized dislocation does not have the same protocol as a dog with pelvic fixation or a total hip replacement. The surgeon’s written plan overrides generic advice.

Confinement and controlled movement
Many dogs need a crate, recovery pen or small room that prevents running, jumping and wrestling. Outdoor trips are usually short and on a leash. Block access to furniture, stairs and other pets unless the veterinary team says otherwise. Carry a small dog over steps; for a larger dog, use a properly fitted support sling only if the team demonstrates how.
Texas A&M Veterinary Medical Teaching Hospital’s total-hip-replacement protocol illustrates how restrictive a procedure-specific plan can be: it calls for crate confinement and leash-only trips outside for three months, avoidance of slippery surfaces and long flights of stairs, and gradual short leash walks after an initial confinement period. Those instructions apply to that institution’s total hip replacement guidance, not to every hip operation.
Pain control and medication safety
Give prescribed analgesics and anti-inflammatory drugs at the stated dose and interval. Never add aspirin, ibuprofen, acetaminophen, sedatives or another pet’s medication without veterinary approval. Contact the hospital about vomiting, severe diarrhea, black stools, marked sedation, agitation, refusal to eat or pain that breaks through the prescribed medication.
Incision, bandage and support checks
Keep the incision clean and dry. Prevent licking with the protective device recommended by the hospital. If a bandage, sling or hip support is used, check for slipping, odor, wetness, swelling, cold toes or skin rubbing and report problems promptly. Do not rewrap or remove it unless instructed.
Floors and daily logistics
Slippery floors can make a recovering dog fall. If the team advises traction, place secure non-slip runners or mats on the routes between the resting area, food, water and toilet spot. Keep nails trimmed only when the veterinarian says handling the feet is safe. Raise bowls only if recommended; an awkward posture can increase strain.

Rehabilitation and follow-up
Rehabilitation may include carefully timed range-of-motion work, supported standing, controlled leash walks, muscle-strengthening exercises or other modalities. The timing depends on bone healing, joint stability, incision status and pain. Do not start an online exercise routine, brace, supplement or exercise device without approval from the treating team.
Spike’s University of Illinois case included four months of therapy to improve function after surgery and illustrates why rehabilitation can be important when scar tissue, pain and lameness persist. It does not establish a standard four-month timeline for other dogs.
Recheck examinations and follow-up radiographs confirm healing and determine when activity can increase. A dog may appear brighter before a fracture or repair is strong enough for unrestricted activity, so do not use enthusiasm as permission to abandon restrictions.

Warning signs during recovery
Contact the veterinary team urgently for sudden worsening lameness, inability to stand, a new deformity, uncontrolled pain, fever, marked lethargy, repeated vomiting, breathing difficulty, abdominal swelling, inability to urinate, incision opening, pus or rapidly increasing swelling. A fall or unsupervised burst of activity should also be reported, even if the dog seems normal afterward; the surgeon may decide that an examination or imaging is needed.
Helping a dog emotionally while activity is restricted
Quiet companionship, gentle grooming when permitted, food puzzles approved by the team and calm attention can reduce boredom. Choose activities that keep all four feet still and do not encourage twisting or sudden movement. Separate the recovering dog from excitable children and other animals. Ask whether sedation or a different confinement arrangement is appropriate if the dog repeatedly attempts to escape.
Expected outlook
Prognosis depends on the exact injury, how quickly life-threatening trauma was addressed, the stability of the repair, infection or other complications, and adherence to restrictions. Some dogs regain comfortable function; others retain stiffness, reduced range of motion or intermittent lameness. A specialist can give the most meaningful estimate only after reviewing the imaging, examination and response to treatment. No case report or general webpage can promise the outcome for the dog in front of you.
Use recovery products only to carry out the prescribed plan: a correctly sized crate or pen for ordered confinement and secure traction surfaces when slippery flooring is a concern. Equipment cannot diagnose a dislocation, stabilize a fracture or replace follow-up with the veterinary team.
