The correct amount depends on the exact product and concentration, the parasite and life stage being treated, the route, your dog’s weight and health, and susceptibility to macrocyclic-lactone toxicity. A dose printed for one chewable preventive must not be converted into a volume for livestock ivermectin or another medicine.
For the FDA-labeled IVERHART PLUS ivermectin/pyrantel chewable, the label specifies a minimum monthly dose of 6 micrograms of ivermectin per kilogram and 5 milligrams of pyrantel per kilogram, delivered through product-specific weight bands. Those directions apply only to that named combination product. A veterinarian must choose the product and plan, especially when disease is suspected or a dog may have received the wrong formulation.

Why a universal dog ivermectin calculator does not exist
Ivermectin is not one standardized medicine. Veterinary products differ in concentration, active ingredients, formulation, route and intended use. A low-dose, FDA-labeled monthly heartworm preventive is pharmacologically different from a concentrated livestock solution, an injectable preparation or an extra-label dermatology regimen. Even when two products contain ivermectin, their volumes and dosing instructions cannot be substituted safely.
The indication changes the answer as well. Heartworm prevention, treatment of mites and treatment of an established heartworm infection are different clinical problems with different protocols, monitoring and risks. A dog’s weight alone cannot determine a safe dose. Age, pregnancy, liver or neurologic disease, concurrent medicines, previous reactions and genetic susceptibility may all change the veterinarian’s decision.

For these reasons, a calculator that asks only for pounds and returns a number can create false confidence. It may encourage a caregiver to measure a concentrated product with an inappropriate syringe, split a tablet that was not designed to be split or use a preventive dose as if it were a treatment dose.
The only chart you can read directly: the IVERHART PLUS label
The following table describes the labeled IVERHART PLUS ivermectin/pyrantel chewable only. It is not a general ivermectin chart and cannot be used to calculate another product.
| Dog’s labeled weight band | Chewables each month | Ivermectin in the labeled chewable | Pyrantel in the labeled chewable |
|---|---|---|---|
| 0–25 lb | 1 | 68 micrograms | 57 milligrams |
| 26–50 lb | 1 | 136 micrograms | 114 milligrams |
| 51–100 lb | 1 | 272 micrograms | 227 milligrams |
The product label states a monthly minimum of 6 micrograms of ivermectin per kilogram and 5 milligrams of pyrantel per kilogram. The chewable strengths and weight bands are how that labeled dose is supplied; they are not permission to prepare an equivalent dose from another source. Dogs weighing more than 100 lb receive an appropriate combination of chewables under the label, rather than an improvised measurement of another ivermectin product.

How to use the chart safely
- Read the complete package label and confirm that the product is IVERHART PLUS, not a different ivermectin medicine.
- Use the dog’s current, accurately measured body weight and the label’s weight band.
- Give the labeled chewable at the stated monthly interval; do not increase frequency to compensate for a suspected infection.
- Ask a veterinarian before dosing a puppy, a sick dog, a pregnant or nursing dog, or a dog taking other medicines.
If the package, strength or weight band is unclear, do not estimate. Bring the container or a photograph of its label to a veterinary clinic or ask the prescribing clinic to verify the directions.

Prevention is not treatment for diagnosed heartworm disease
A monthly preventive is intended to kill susceptible immature heartworm stages acquired since the previous dose. It does not remove adult worms safely from a dog with established heartworm disease. The Merck Veterinary Manual identifies melarsomine adulticide treatment as the only FDA-approved approach for canine heartworm disease.
Veterinarians may discuss nonarsenical, ivermectin-based approaches in unusual circumstances, but those protocols have important limitations. They can require prolonged treatment, leave ongoing lung damage while worms die slowly, create uncertainty about when worm death occurs and contribute to resistance concerns. They are not a home treatment plan and should never be substituted for diagnostic testing and a veterinary protocol.
A dog that has missed prevention may need a heartworm test, a timing assessment and a product-specific restart plan. Do not give extra chewables or a concentrated ivermectin dose in an attempt to “catch up.” The American Heartworm Society’s 2024 guidance recommends year-round use of an FDA-approved preventive and notes that infection can occur when a dose is skipped or delayed.

In the United States, veterinarians sometimes use ivermectin outside a product label for conditions such as demodicosis or sarcoptic mange. Merck’s antiparasitic guidance describes indication-specific regimens, but these are treatment decisions, not consumer charts. The regimen can involve a different dose, interval, duration and monitoring plan from a monthly heartworm preventive.

Before selecting such a treatment, the veterinarian generally confirms the diagnosis with an examination and appropriate testing. Similar-looking hair loss, crusting, itching or ear disease can have bacterial, fungal, allergic or immune causes that ivermectin will not correct. Treating the wrong condition delays effective care and exposes the dog to avoidable drug risk.
Genetic and individual factors can make ivermectin dangerous
Some dogs carry variants in the MDR1/ABCB1 gene. These variants can reduce a protective transport mechanism at the blood-brain barrier, allowing macrocyclic lactones to reach the central nervous system at higher exposure. Herding breeds and their crosses are overrepresented, but breed appearance does not reliably identify a dog’s status; dogs without a known risk can also react abnormally.
Merck reports transient neurologic signs in MDR1-affected dogs around 120–150 micrograms per kilogram in its ivermectin-toxicity discussion. Those figures are toxicology observations, not safe-dose limits, treatment targets or a threshold below which every dog is protected. The American College of Veterinary Pharmacists emphasizes that individual responses vary and that genetically sensitive dogs may become ill at lower amounts than most dogs.
Ask your veterinarian whether MDR1/ABCB1 testing is appropriate when a dog may need an extra-label macrocyclic-lactone regimen, has a history of neurologic reactions or belongs to a breed with recognized risk. A normal genetic result does not make an accidental overdose safe, and a negative history does not replace product-specific dosing.

Recognize possible ivermectin toxicity
Neurologic signs can develop after an excessive dose or an unexpectedly sensitive response. Reported signs include:
- Weakness, unusual fatigue or difficulty standing and walking
- Excess salivation
- Dilated pupils, visual impairment or apparent blindness
- Tremors, poor coordination or abnormal eye movements
- Seizures
- Collapse, coma or breathing difficulty in severe cases
These signs are an emergency, particularly after exposure to livestock ivermectin, a concentrated solution, another animal’s medicine or an unknown amount. Contact a veterinarian or an animal poison-control service immediately. Do not wait for symptoms, induce vomiting unless a veterinary professional specifically directs it, or use a calculator to decide that the exposure is harmless.
Take the container, product strength, estimated amount, time of exposure and the dog’s current weight with you or provide those details by phone. Tell the clinician about other medicines and any MDR1/ABCB1 test result. FDA adverse-event reports can identify safety signals, but they do not establish a reliable frequency or prove that every reported sign was caused by ivermectin; the individual exposure still requires professional assessment.

Follow the instructions for the specific product and call the prescribing veterinarian if the interval is uncertain. The appropriate response depends on how late the dose is, whether the dog has had recent heartworm testing, the dog’s age and travel history, and the product’s label. Never double a dose or switch to a livestock formulation.

How a veterinarian determines an appropriate ivermectin plan
- Identify the product. The clinician records the exact brand or generic name, active ingredients, concentration, formulation and route. A photograph of the label is useful when the product was purchased elsewhere.
- Confirm the dog’s weight. The dog is weighed rather than estimated from appearance. Weight bands on a chewable label are not interchangeable with milliliters of a solution.
- Define the purpose. Prevention, treatment of a diagnosed mite infection and management of heartworm disease require different goals and protocols.
- Check health and risk factors. The examination includes neurologic history, age, pregnancy or nursing status, liver and other systemic disease, concurrent drugs and possible MDR1/ABCB1 susceptibility.
- Test when indicated. Heartworm testing and skin or ear diagnostics help distinguish prevention from treatment and prevent an ivermectin regimen from masking another disease.
- Set monitoring and follow-up. The veterinarian explains the interval, duration, signs to report, recheck testing and any exercise restrictions that apply to heartworm disease.
This process is why a safe prescription may look different for two dogs of identical weight. The medicine, diagnosis and risk profile—not pounds alone—determine the plan.
Administer labeled chewables to reduce choking risk
FDA labeling updates for Heartgard Plus note a choking or intestinal-obstruction risk when a chewable is swallowed whole and advise administration that encourages chewing. Follow the instructions for the particular chewable, offer it in a way that allows the dog to chew and observe the dog afterward. Do not force a large, intact piece into the throat.
If the dog vomits immediately, spits out part of the chewable or you cannot tell how much was swallowed, contact the prescribing clinic before repeating the dose. Giving an unscheduled replacement can result in an overdose, while withholding a replacement may leave a prevention gap; the correct choice depends on the product and timing.
Questions to settle before giving any ivermectin
- Do I have the exact canine product and its current label, rather than a livestock or another animal’s preparation?
- Is this for routine prevention, or has a veterinarian diagnosed a condition that needs treatment?
- Has the dog’s current weight been measured accurately?
- Could the dog carry an MDR1/ABCB1 variant or have a neurologic or systemic illness that changes risk?
- Is the intended dose, route and interval written by a veterinarian or directly on the product label?
- Do I know whom to call if the dog receives too much, receives the wrong product or develops neurologic signs?
If any answer is no, pause before dosing and obtain veterinary guidance. The IVERHART PLUS table is useful for checking the labeled chewable’s weight band; it is not a substitute for diagnosis, a prescription or emergency advice.
