Dog Vertigo: Signs, Causes, Diagnosis, and What to Do

A dog’s sudden head tilt, stumbling, falling, or abnormal eye movements can signal vestibular disease—but the signs have several possible causes and need veterinary assessment.

Sudden head tilt, stumbling, falling, circling, flicking eye movements, or disorientation can signal a problem with your dog’s balance system. Contact a veterinarian promptly: these signs are not enough to tell whether the cause is idiopathic vestibular syndrome, an inner-ear problem, or another illness.

What “vertigo” means in dogs

People often use “vertigo” to describe a dog that seems dizzy or off balance. The veterinary term is usually vestibular disease: dysfunction in pathways that help control balance, posture, and spatial orientation. The problem may involve the inner ear and vestibular nerve (peripheral disease) or the brain (central disease). A head tilt and loss of balance can point toward vestibular pathways, but appearance alone cannot identify the location or cause.

Vestibular signs can begin suddenly. Affected dogs may tilt their heads, stumble, lean, circle, or fall, often toward the tilted side. Their eyes may move abnormally from side to side or rotate (nystagmus), and disorientation, nausea, or vomiting can occur. Merck Veterinary Manual explains that inner-ear inflammation can impair the vestibulocochlear nerve, causing signs of peripheral vestibular disease and sometimes hearing loss (Merck Veterinary Manual: Otitis Media and Interna in Animals).

What to do if your dog suddenly loses balance

Call your veterinarian promptly when these signs appear, even if your dog is old or the episode resembles a previous one. Do not try to determine at home whether the dog has had a stroke, an ear infection, a tumor, or idiopathic vestibular disease. A veterinarian needs to assess the dog and decide whether urgent examination or hospital care is needed.

Seek urgent veterinary help if your dog cannot stand or walk safely, is repeatedly vomiting, cannot drink, becomes less alert, worsens, or is otherwise seriously unwell. While arranging care, keep the dog away from stairs and other fall hazards; avoid forcing it to walk.

Possible causes: idiopathic syndrome is only one explanation

“Old dog vestibular disease” is a common name for idiopathic vestibular syndrome, which means no underlying cause is identified. Age alone does not establish that diagnosis. Other conditions can affect the same balance pathways, including middle- or inner-ear infection, trauma, tumors or polyps, hypothyroidism, rare medication effects, and other neurologic or systemic disease.

Diagnostic possibility What it means How it is distinguished
Idiopathic vestibular syndrome Vestibular signs occur without an identified cause; it is commonly assessed from the pattern and course of illness after considering other explanations. The veterinarian evaluates onset, examination findings, and early progression. Further tests may be recommended if the pattern is atypical, signs worsen, or improvement does not occur.
Inner- or middle-ear disease Infection or inflammation can disrupt peripheral balance pathways and may affect hearing. Ear and neurologic examinations, otoscopy when possible, and sometimes imaging or ear sampling help assess the possibility.
Other neurologic, traumatic, or systemic causes Brain disease, trauma, tumors or polyps, hypothyroidism, and other conditions can produce vestibular-like signs. History and examination guide whether blood or urine testing, blood-pressure measurement, imaging, or other targeted tests are appropriate.

These are distinctions for the veterinary workup, not a home checklist for choosing treatment. A specialist review notes that idiopathic cases commonly have an acute or very rapid onset and often improve quickly, but that pattern does not prove the diagnosis (Mertens et al., 2023, specialist review of idiopathic vestibular syndrome).

How veterinarians investigate vestibular signs

The assessment usually starts with the history and physical, neurologic, and ear examinations. The clinician will consider when the signs began, how they have changed, whether the dog has ear symptoms or hearing changes, and what the neurologic examination shows. Not every dog needs every available test; the choices depend on examination findings and suspected causes.

  • Ear assessment: Otoscopy may be possible during the initial examination. If needed, examination under anesthesia and ear cytology or culture can help investigate infection.
  • General health checks: Blood and urine tests and blood-pressure measurement may help identify underlying or contributing problems.
  • Imaging: CT or MRI may be recommended when ear disease, a brain problem, or another structural cause needs investigation. Merck notes that CT or MRI is more sensitive than routine radiographs for definitive assessment of otitis media, but imaging findings must be interpreted alongside the clinical examination.
  • Selected additional tests: Depending on the case, a veterinarian may consider spinal-fluid analysis, infectious-disease testing, or BAER testing to assess hearing.

Veterinary neurologists do not follow one identical diagnostic pathway in every case; the suspected location and cause, the dog’s findings, and available testing all shape the workup (Mertens et al., 2023; VCA Animal Hospitals: Vestibular Disease in Dogs).

Treatment depends on the cause and the dog’s needs

There is no single treatment for all vestibular signs. If an underlying condition is found, treatment is directed at that condition; for example, ear infection requires veterinary-directed care. In idiopathic cases, care is often supportive, with veterinary treatment for nausea and dehydration and help keeping the dog safe and comfortable. Do not give medication unless your veterinarian directs you to.

A dog that is severely disoriented or unable to stand, walk, eat, or drink safely may need hospitalization. VCA describes hospital care, including fluids and support until a dog can eat and walk, for severe cases. The veterinarian should decide whether home nursing is adequate based on the dog’s condition, not a universal threshold.

Some dogs need close nursing or hospital support rather than routine home care. A veterinarian can advise whether the dog can be moved safely and whether reassessment is needed.

Recovery: what the usual timeline can and cannot tell you

For idiopathic vestibular syndrome, signs are often most severe during the first 24–48 hours. VCA reports that many dogs begin improving within 72 hours and most recover over two to three weeks; a specialist review describes improvement within a couple of days and typical resolution over two to four weeks. Mild head tilt or unsteadiness can remain. These are general patterns for idiopathic cases, not a prediction for an individual dog or for vestibular signs caused by another condition.

Worsening signs or no improvement should prompt veterinary reassessment. Depending on the findings, the veterinarian may recommend further testing or referral to a veterinary neurologist. Even when an episode resembles idiopathic disease, recovery alone should not be used as a reason to skip the initial assessment.

Questions to take to the veterinary visit

  • When did the signs start, and have they changed since then?
  • Has the dog fallen, vomited, stopped eating or drinking, or become less alert?
  • Could an ear problem, injury, medication, or other recent health change be relevant?
  • What findings suggest a peripheral ear-related problem versus a central neurologic one?
  • Which tests are recommended now, and what would make additional imaging or referral appropriate?
  • What level of assistance is safe at home, and which changes should trigger an urgent call?
