Ear cancer in cats is not one disease. Tumors can arise in the ear canal, outer ear, middle ear, or inner ear, and infections, inflammatory polyps, and other non-cancerous tissue changes can look similar. A problem confined to one ear that persists despite prescribed treatment deserves a veterinary recheck; it raises concern but does not prove cancer. Bloody or foul discharge, a visible mass, worsening pain or swelling, deafness, head tilt, loss of balance, or other neurologic changes should prompt timely veterinary advice.

What “ear cancer” means in a cat

Veterinarians use “ear tumor” or “ear cancer” for several distinct growths. Some are malignant and can invade nearby tissue or spread; others are benign, inflammatory, or hyperplastic (an overgrowth caused by irritation). The same-looking lump may therefore require very different treatment.

Site Examples described in feline patients Why location matters
Outer ear and external canal Squamous cell carcinoma, earwax (ceruminous) gland tumors, inflammatory or nasopharyngeal polyps The canal is narrow, so swelling can block airflow, trap discharge, and make examination difficult.
Middle or inner ear Lymphoma, fibrosarcoma, squamous cell carcinoma, and other growths Deeper disease can affect hearing, balance, and nerves controlling the face and eye movements.

Malignant ear tumors are reported more often in cats than in dogs, although ear tumors are still less common than many other feline skin tumors. Older cats, particularly those over about 11 years, are more often diagnosed with malignant tumors. Inflammatory polyps are seen more commonly in younger cats, roughly 3 months to 5 years. These are population patterns, not diagnostic rules: an older cat can have a polyp or infection, and a young cat can develop a tumor.

Merck Veterinary Manual’s professional overview states: “A neoplasm of the ear canal should be suspected when unilateral otitis is refractory to medical treatment.” In practical terms, inflammation or infection that stays in one ear and fails to improve with the veterinarian’s treatment should be reassessed. It should not be interpreted as proof of cancer.

What are the signs of ear cancer in cats?

Signs depend on which part of the ear is affected and how much the growth obstructs or invades tissue. None of these findings confirms cancer because infection and inflammatory disease can produce the same changes.

Changes in the outer ear or ear canal

  • Discharge that keeps returning, including waxy, pus-filled, or bloody material
  • A foul or unusually strong odor
  • Repeated head shaking or scratching at one ear
  • Pain when the ear or side of the head is touched
  • Swelling, a visible lump, thickened tissue, or an ulcer on the ear flap or canal opening
  • An abscess or draining area below the ear
  • Reduced hearing or apparent deafness on that side

A mass may be hidden deep in the canal, so the absence of a visible lump does not rule out a growth. Conversely, a visible lump may be an inflammatory polyp or another benign change.

Signs that suggest middle- or inner-ear involvement

  • Head tilt
  • Stumbling, falling, or loss of balance (ataxia)
  • Rapid involuntary eye movements (nystagmus)
  • Facial weakness or an altered blink
  • Marked disorientation or difficulty navigating

These vestibular and facial-nerve signs can occur with infection, inflammation, trauma, or other neurologic disease as well as a tumor. A new head tilt, severe imbalance, rapidly worsening pain, or neurologic change warrants prompt veterinary contact rather than watchful waiting.

Can an ear infection that won’t go away be cancer?

It can be a reason to investigate, but it is not a diagnosis. A one-sided infection that does not respond as expected may reflect resistant organisms, an obstructing foreign body, an inflammatory polyp, allergy or other skin disease, incorrect medication use, or a tumor. Chronic inflammation has been proposed as a contributor to tissue change and neoplasia, yet the precise cause of feline ear tumors is unknown and chronic infection has not been proven to cause cancer.

Arrange a recheck when treatment has been completed as directed but discharge, odor, pain, or swelling persists or quickly returns. Do not put cleaners, drops, human medicines, cotton swabs, or instruments into an ear containing an unexplained mass unless the veterinarian specifically instructs you. Irritation or pressure can worsen pain and may damage a diseased eardrum or deeper structures.

How do vets diagnose an ear tumor?

Diagnosis is a process rather than a visual impression. Merck notes that true neoplasia can be difficult to distinguish from hyperplastic or inflammatory tissue on physical examination, and inflammation-related growths may shrink or disappear when the inflammation is treated.

  1. History and general examination. Tell the veterinarian when signs began, whether one or both ears are affected, which medicines were tried, how the cat responded, and whether hearing, balance, appetite, or behavior changed.
  2. Ear examination. The veterinarian inspects the ear flap and canal and may use an otoscope. Sedation or anesthesia may be needed when the canal is painful, blocked, or too deep to examine safely in an awake cat.
  3. Sampling of material. Discharge may be examined for organisms and inflammatory cells. This can guide treatment of infection but cannot by itself determine whether a mass is malignant.
  4. Imaging when deeper disease is suspected. If a mass may involve the bone behind the ear or the middle ear, computed tomography (CT) or magnetic resonance imaging (MRI) may be recommended. The choice depends on the suspected site and the information needed for planning.
  5. Tissue diagnosis. A biopsy or removal of representative tissue lets a veterinary pathologist identify the cell type and whether it is benign, inflammatory, or malignant. Tissue results are the basis for discussing treatment and prognosis.

There is no single staging package required for every cat. Imaging, lymph-node assessment, chest or abdominal tests, and additional sampling are selected according to the suspected tumor, location, clinical signs, and overall health. Ask what each proposed test will change about the treatment decision.

What treatment does a cat with an ear tumor need?

The plan is individualized. Key factors include the tissue diagnosis, whether the middle-ear bone is involved, local extent and any spread, neurologic signs, the possibility of complete removal, the cat’s age and health, expected comfort and hearing, recovery demands, and access to a specialist.

Approach When it may be discussed Important trade-offs to ask about
Surgical removal Often considered for a resectable tumor, including many malignant earwax-gland tumors; deeper disease may require opening the ear canal and removing affected middle-ear bone. Ask whether margins are likely to be complete, what hearing or facial-nerve effects are possible, expected hospitalization, pain control, and recurrence monitoring.
Referral to a veterinary surgeon Particularly useful when the mass extends into the middle ear, bone, or other complex anatomy. Referral can add travel, cost, and anesthesia planning, but may provide advanced imaging and specialized surgical options.
Radiation therapy Listed by the Veterinary Society of Surgical Oncology as an option for selected feline ear tumors when diagnosis and circumstances support it. It is not routine for every tumor. Discuss treatment schedule, anesthesia requirements, side effects, access, and the goal—local control, symptom relief, or both.
Medical and supportive care Used to control infection, inflammation, pain, nausea, or neurologic discomfort before, after, or instead of tumor-directed treatment when appropriate. Medicines may improve signs without removing a tumor. Clarify what improvement should occur and when a lack of response requires reassessment.

A board-certified veterinary surgeon or veterinary oncologist may help when pathology is unusual, the tumor is deep, surgery is technically difficult, or radiation is being considered. Bring the pathology report and imaging results to any consultation.

What is the prognosis?

There is no single life-expectancy figure for “ear cancer.” Histologic type, depth, invasion, neurologic signs, spread, and the completeness and type of treatment all influence outcome. A number from one tumor type must not be applied to another.

The Veterinary Society of Surgical Oncology summarizes median survival times of 49 months for ceruminous gland adenocarcinoma, 3.8 months for squamous cell carcinoma, and 5.7 months for undifferentiated carcinoma. The same page reports medians of 1.5 months for cats with neurologic signs versus 15.5 months for those without neurologic signs. These are specialist-page summaries; the page does not state a publication year or provide the underlying study details alongside each figure, so they are not individualized predictions. Your veterinarian should interpret any estimate in light of the cat’s pathology and imaging.

A 2023 peer-reviewed review examined 25 cats with aural squamous cell carcinoma diagnosed from 2010 through 2021. That sample describes one cancer type and a selected clinical population; it does not establish the prevalence or expected survival of all feline ear tumors.

What to do while waiting for the appointment

  • Keep the cat indoors and prevent scratching if the veterinarian has supplied a safe protective collar.
  • Do not squeeze, probe, or attempt to remove a mass.
  • Use only medications prescribed for this cat and this ear; do not restart leftover drops without advice.
  • Record the side affected, discharge color and odor, head-shaking frequency, appetite, hearing response, and any balance or facial changes.
  • Seek urgent advice for continuous bleeding, rapidly increasing swelling, severe pain, repeated vomiting with balance problems, inability to stand, or a sudden head tilt.

At the visit, ask: What did the tissue sample show? Is the middle ear or bone involved? Is imaging needed before treatment? What are the goals and likely effects on comfort, hearing, and balance? Would referral to a veterinary surgeon or oncologist change the available options? These questions help you compare realistic choices without assuming that every mass needs the same operation or therapy.
