Feline Infectious Peritonitis (FIP): Diagnosis, Treatment and 2026 Update

FIP is a serious disease associated with feline coronavirus, but antivirals have made it treatable. Learn how diagnosis, veterinary treatment and monitoring work.

Feline infectious peritonitis (FIP) is a serious disease associated with feline coronavirus (FCoV), but it is now treatable with antiviral medicines. There is no single definitive test: veterinarians diagnose FIP by combining a cat’s history and examination with laboratory findings. If FIP is suspected, contact a veterinarian promptly; treatment, dosing and monitoring need veterinary oversight.

What causes FIP, and how does feline coronavirus spread?

FCoV infection is common in cats and often causes no signs or only mild illness. Only a smaller proportion of infected cats develop FIP. The European Advisory Board on Cat Diseases (ABCD) describes FCoV transmission as faecal-oral, meaning infection can spread through contact with virus shed in faeces.

FIP is associated with FCoV, but a cat’s exposure to the virus is not the same as having FIP. That distinction is important when interpreting antibody tests and deciding what further investigation is needed.

How is FIP diagnosed? Is there a test for FIP?

No single test definitively diagnoses FIP. Cornell’s Feline Health Center explains that coronavirus antibody titers show exposure to FCoV; they cannot distinguish exposure to common feline coronavirus from FIP-associated disease. A positive antibody result alone does not confirm FIP.

PCR tests look for viral genetic material in a sample such as tissue or body fluid. They can help, but a positive result alone is not conclusive, and a negative result does not rule out FIP. Cornell notes that PCR tests can produce false positives and false negatives. ABCD adds that blood PCR can have poor sensitivity.

A veterinarian interprets results alongside the cat’s history and examination. For example, ABCD says FIP is very likely when an effusion—a build-up of fluid—has a high FCoV RNA load and cytological and biochemical findings compatible with FIP. Even in that situation, the PCR result is part of the overall assessment rather than an infallible standalone answer.

  • Antibody titer: evidence of FCoV exposure, not proof of FIP.
  • PCR: may add useful evidence, but results depend on the sample and must be interpreted in context.
  • Diagnosis: a veterinarian’s synthesis of history, examination and laboratory findings.

Can FIP be treated?

Yes. Antiviral treatment has changed the outlook for cats with FIP, although an individual cat’s response cannot be guaranteed. Cornell identifies GS-441524 as the antiviral studied most extensively in cats with naturally occurring FIP. The choice of medicine and route, dose, treatment duration and follow-up should be made with the attending veterinarian, ideally one experienced in FIP.

ABCD generally favours oral GS-441524. It also describes injectable remdesivir as an option when oral medication cannot be tolerated or is unavailable. Which approach is suitable depends on the cat’s clinical form and circumstances, including whether ocular or neurological signs are present, response to treatment, tolerability, access and local rules. These are veterinary decisions, not do-it-yourself treatment choices.

Injectable remdesivir ABCD describes it as an option when oral medication cannot be tolerated or is unavailable. Specific suitability, access and treatment details are not stated in the reviewed guidance; ask a veterinarian.

U.S. access to compounded oral GS-441524

Cornell reported that a compounded oral GS-441524 formulation became available in the United States on June 1, 2024, through Stokes Pharmacy working with the Bova Group, and that a veterinary prescription is required. Cornell described this access under the FDA Center for Veterinary Medicine’s Guidance for Industry #256. This is a dated U.S. access report, not a claim of global approval or availability. Ask a veterinarian about current local law, prescribing and supply. Cornell also warns that unregulated products may vary in reported concentration and purity.

How long do I treat with GS-441524 antivirals?

An 84-day course is the duration represented in most studies cited by ABCD. However, course length is not a universal rule for every cat: the veterinarian tailors treatment to clinical findings and response.

ABCD’s 2026-updated guideline summarizes a prospective randomized controlled study in which oral GS-441524 for 42 days was as effective as 84 days in cats with mostly effusive FIP; the study reported 95% survival. That result is important, but it should not be generalized automatically to every form of FIP or every individual cat. Do not shorten, stop or change a course without veterinary direction.

What progress should I expect during treatment?

Improvement can begin within days. In its July 2025 treatment update, International Cat Care says appetite, demeanour, fever and effusions may improve within two to five days after successful treatment. It reports a response rate greater than 85%, but that figure describes the guidance’s reported treatment outcomes, not a guarantee for an individual cat.

Progress still needs assessment over time. The veterinarian may use the cat’s clinical response and laboratory findings to judge whether the plan is working and whether it needs adjustment. A lack of expected improvement or a worsening condition warrants prompt veterinary contact.

Track weight at home

ABCD recommends checking a recovering cat’s weight every one to two weeks. Weight gain can help show recovery and gives the veterinarian information needed to recalculate medication dose as the cat’s weight changes. A digital pet scale can make this check easier, but it is a monitoring aid—not a diagnostic test or treatment.

Can FIP relapse after treatment?

Relapse can occur. International Cat Care’s July 2025 update reports relapse in fewer than 10% of treated cats and says it is most likely during the month after treatment ends. Contact the veterinarian if concerning signs return; do not restart or alter antiviral treatment on your own.

Long-term lifelong cure is not yet established. International Cat Care notes that follow-up in treated groups is limited, so current reports cannot settle every cat’s long-term outcome.

What is the 2026 update on FCoV-23?

On April 27, 2026, Cornell University College of Veterinary Medicine reported that an FCoV-2 variant previously associated with the Cyprus outbreak had also been identified in archived U.S. material. The report described retrospective analysis of 20 tissue, faecal and abdominal-fluid samples from cats, including archived samples from 2013 and 2016. Researchers identified spike-protein domain-zero deletions in two cats with prolonged disease.

This is a research finding, not a diagnostic test and not evidence that a particular cat has FCoV-23. In Cornell’s report, virologist Gary Whittaker said: “Our data suggest that the transmissible form of the virus carries the long spike protein.”
