Feline Leukemia Virus (FeLV): Causes, Symptoms, and Treatment

Feline leukemia virus spreads mainly through saliva and close contact. Learn transmission routes, signs, when to test, vaccination, and long-term care based on 2020 AAFP guidance.

Feline leukemia virus (FeLV) is a contagious retrovirus that spreads between cats, mainly through saliva and close contact. Infection does not mean a cat will look ill right away. Many infected cats show no obvious signs when they are tested, and outcomes vary widely. No established treatment eliminates the virus, so care focuses on testing at the right times, preventive veterinary care, prompt treatment of illness, and limiting spread to other cats.

The guidance behind this article is the American Association of Feline Practitioners’ 2020 feline retrovirus guideline and its accompanying education toolkit. The AAFP now operates under the name FelineVMA. Veterinary recommendations are revised over time, so treat the 2020 guidance as a foundation and confirm current advice with your veterinarian and local authorities.

What FeLV is

FeLV is a retrovirus that infects cats. Infection status cannot always be settled with a single test, which is why the testing and follow-up sections below matter as much as the diagnosis itself. The AAFP materials address transmission between cats. They do not describe ordinary human contact as a route of spread.

In the AAFP’s January 9, 2020 announcement, 4% of tested cats in North America were found infected with FeLV. That figure describes cats that were tested and is reported for 2020. It is not a current estimate of prevalence among all cats.

How FeLV spreads

The virus moves between cats mainly through saliva and close cat-to-cat contact. The AAFP lists these routes:

  • Grooming, including allogrooming between cats in the same household or colony.
  • Shared food and water bowls.
  • Queen to kitten. A mother cat can pass the infection to her kittens.
  • Less common routes: nasal secretions, milk, urine, and feces.

Kittens are more susceptible than adult cats to progressive infection, so kittens exposed to an infected cat warrant prompt testing and a veterinary plan.

Signs and outcomes

FeLV is associated with serious illness, including anemia and cancer, but infection does not follow one fixed course. Some cats remain well for long periods, while others develop serious disease. The AAFP client resource describes FeLV as able to affect quality of life and longevity, with outcomes depending on the individual cat.

Symptoms alone cannot tell you whether a cat is infected. A cat can carry the virus without obvious signs at the time of testing. The changes below are nonspecific, meaning they occur with many illnesses and are not a FeLV checklist. Any of them warrants a veterinary visit and, when appropriate, testing:

  • Lethargy or a drop in activity
  • Pale gums, which can point toward anemia
  • Recurrent infections or illness that does not resolve

Testing and what results mean

The AAFP toolkit recommends testing a cat:

  • As soon as possible after acquiring it
  • After exposure to an infected cat or to a cat of unknown status
  • Before FeLV vaccination
  • Whenever the cat shows clinical illness

Testing is done through a veterinarian. Do not attempt to diagnose FeLV at home, and do not interpret a positive result without veterinary follow-up.

Why one test may not be enough

Results can be discordant, meaning that one test and another, or one timepoint and another, may not agree. The veterinarian may recommend follow-up testing using a different method and at a time they select. Julie Levy, DVM, PhD, DACVIM, DABVP (Shelter Medicine), noted in the same 2020 release that quantitative testing may be used to better inform clinical decision-making, and that how test results, clinical signs, and prognosis relate to a cat’s immune response and circulating virus levels is still evolving.

Heather O’Steen, CEO of the AAFP, stated in the January 9, 2020 release: “Education and early testing can greatly assist in the treatment and management of feline retrovirus infections.”

Steps after a positive or uncertain result

  1. Ask whether a repeat or different test is needed and when it should be done.
  2. Get a baseline health assessment so later changes can be recognized against a known starting point.
  3. Set a plan for other cats in the home, covering contact, bowls, grooming, and any separation.
  4. Review vaccination for the other cats with the veterinarian.
  5. Agree on a monitoring schedule and the signs that should prompt a visit sooner.

Vaccination

FeLV vaccines are available in many countries. According to AAFP material, vaccines appear efficacious at preventing progressive infection and are clinically important. Vaccination reduces risk, but it does not replace testing, and it does not remove the need to identify infected cats.

The AAFP’s 2020 announcement describes vaccination for kittens and for cats with ongoing risk of exposure. It notes that some adult indoor-only cats may not need vaccination after the initial period. Individual decisions should be made with a veterinarian and checked against local guidance, because recommendations differ by location and by each cat’s risk.

Treatment and long-term care

The guideline notes a paucity of data evaluating treatments, particularly antiretroviral and immunomodulatory drugs. The AAFP toolkit states that long-lasting benefit from antiretrovirals has not been demonstrated, and that trial evidence for immunomodulatory antiviral drugs has limitations. For that reason, no drug, supplement, or “immune booster” should be presented or purchased as a cure. Any medication decision belongs with a veterinarian who knows the cat’s history.

The AAFP management resource states that euthanasia of positive cats is not recommended. Management is built around four goals:

  • Preventive health care, including regular examinations and parasite and dental care as your veterinarian advises
  • Prompt identification and treatment of illness when it occurs
  • Limiting spread to other cats
  • Individualized follow-up that tracks the cat’s condition over time

Managing a multi-cat household

Discuss testing, contact, and separation plans with a veterinarian before making changes. The AAFP toolkit identifies shared bowls, allogrooming, and alloplay as possible transmission contexts. Housing infected cats indoors, or in enclosed outdoor structures, reduces opportunities for the virus to pass to other cats.

Questions to settle with your veterinarian

After a positive or uncertain result, the AAFP guidance supports comparing the plan on the axes below rather than choosing a consumer product.
