FIP in Cats: Symptoms, Diagnosis, and Treatment Options

FIP is a serious disease linked to feline coronavirus, but exposure or a positive antibody test alone does not diagnose it. Learn what signs warrant veterinary care, how vets assess FIP, and what treatment access means in the United States.

Feline infectious peritonitis (FIP) is a serious disease associated with feline coronavirus, but exposure to the virus—or a positive coronavirus antibody test—does not mean a cat has FIP. Its signs can resemble those of many other illnesses, so diagnosis and treatment need a veterinarian. Antiviral treatment has changed the outlook, but there is no single test or treatment plan that applies to every cat.

What is FIP, and how does a cat develop it?

FIP is associated with feline coronavirus, a virus that is common among cats. Only a subset of cats exposed to feline coronavirus develop FIP. Cornell Feline Health Center gives an estimate of 5–10% of exposed cats on a page carrying a 2020 copyright notice; this is a source-attributed estimate, not a newly measured rate for today’s cat population.

A positive feline coronavirus antibody result shows exposure to the virus; it does not establish that the cat has FIP. The antibody test cannot distinguish ordinary feline coronavirus exposure from FIP-associated disease.

What are the symptoms of FIP in cats?

Early signs are often nonspecific. Fever, low energy or depression, reduced appetite, and weight loss can occur with FIP, but they can also signal many other conditions. The pattern of illness and other findings matter; symptoms alone cannot confirm FIP.

Presentation What may happen
Effusive, sometimes called wet Fluid may accumulate in the abdomen or chest. Chest fluid can be associated with breathing difficulty.
Non-effusive, sometimes called dry Illness may develop more slowly. Neurologic or eye signs can occur.

These are broad patterns, not separate diagnoses a pet owner can reliably identify at home. Presentations can evolve, and signs vary between cats.

Contact a veterinarian promptly if FIP is a concern. Seek urgent veterinary care if your cat is struggling to breathe, is very unwell, or develops neurologic signs such as problems with balance or movement. Those signs can indicate serious illness and need assessment regardless of the cause.

Can my cat be tested for FIP?

There is no single definitive test that confirms every case. The 2022 AAFP/EveryCat FIP Diagnosis Guidelines describe diagnosis as particularly challenging when fluid accumulation is absent. A veterinarian weighs the cat’s history and signs alongside examination findings and test results.

Each package comes with six (6) easy to apply, fully waterproof topical applications that ensure your friend has safe and reliable flea protection and control for up to six (6) months
  • Bloodwork and examination: These contribute to the overall picture but must be interpreted in context.
  • Imaging: A veterinarian may use imaging to look for fluid accumulation or other findings that help assess the illness.
  • Fluid or tissue testing: If fluid or tissue is available, testing it may add useful evidence. The appropriate test depends on the sample and the individual case.
  • Coronavirus antibody testing: A positive result indicates exposure, not a diagnosis of FIP.

Ask your veterinarian what each result does—and does not—show, and how it fits with the cat’s signs. A test result should not be treated as proof of FIP without considering the rest of the clinical evidence.

Can FIP be treated?

Yes. Antiviral treatment has changed the outlook for cats with FIP. Cornell reports that studies in laboratory settings and client-owned cats support GS-441524 as a safe and effective treatment option. That evidence does not guarantee an outcome for an individual cat, and important questions remain, including long-term effectiveness, minimum dose, resistance, and which drug approach is best.

Treatment is a veterinary decision. The veterinarian considers the cat’s condition, available medicines and formulations, ability to take medication, need for supportive care, and follow-up. Do not try to diagnose FIP, select an antiviral, or calculate a dose yourself.

Antivirals and supportive care

Veterinarian-managed options discussed in the FelineVMA 2025 guide include injectable remdesivir, compounded oral remdesivir in some circumstances, and hospitalization when needed. GS-441524 is another studied treatment option. Which approach is appropriate depends on the cat and the circumstances; these options are not a do-it-yourself protocol.

Supportive treatment may also be needed. Cornell describes interventions such as fluids, draining accumulated fluid, and transfusions in selected cases. A veterinarian determines whether these are appropriate as part of the cat’s care.

How can U.S. owners find legitimate FIP treatment?

In the United States, it is important to distinguish an FDA-approved drug from a compounded drug. In a notice dated May 10, 2024, the FDA said no FDA-approved drug was available for FIP at that time. The agency also said it did not intend to enforce new animal drug approval requirements for products compounded from GS-441524 when a veterinarian prescribed them for a specific cat under the conditions in Guidance for Industry #256. The FDA clarified that these compounded products remain unapproved drugs.

That notice describes the FDA’s U.S. position on a specific date; it does not mean that a compounded product is FDA-approved, nor does it establish current stock or availability. Local law and access may differ outside the United States or change over time. Ask a veterinarian about lawful options where you live and whether a pharmacy can currently supply the prescribed formulation.

Cornell reported that Stokes Pharmacy, in collaboration with Bova Group, announced an oral compounded formulation would be available beginning June 1, 2024, with a veterinary prescription required. That dated announcement is not a guarantee of present-day supply. Avoid relying on unregulated sellers or online dosing instructions in place of veterinary care.

How can I protect my cat from FIP?

Preventing feline coronavirus infection can be difficult because the virus is widespread, particularly in high-density settings such as shelters and catteries. Cornell recommends housing cats at a density at or below three per room to reduce stress associated with crowding. This is a housing recommendation, not a guarantee that cats will avoid infection or FIP.

Cornell describes FIP as developing in individual cats after feline coronavirus infection and says the FIP-associated virus itself is not believed to spread between cats, while noting that research continues. Avoid treating that understanding as certainty about every household or setting. If a cat in a multi-cat home is ill, ask a veterinarian for advice tailored to the cats and their living conditions.

The AAFP/EveryCat 2022 guidelines estimate that FIP accounts for around 0.3% to 1.4% of deaths at veterinary institutions. That figure describes deaths at those institutions, not the chance that an individual cat or the general cat population will die from FIP.
