Flat-Faced Cats vs. Other Cat Breeds: Health and Care Differences

Flat-faced cats can have added risks involving breathing, eyes, teeth and facial folds, but severity varies. Learn what to watch for, how coat care differs, and when to consult a veterinarian.

Flat-faced cats are not automatically unhealthy, but a shortened muzzle can be associated with narrowed airways, eye irritation, dental crowding and skin-fold problems. The degree of flattening varies between breeds and individual cats, so a breed name or appearance cannot tell you how well one cat can breathe or what care it needs. Pay attention to the individual cat’s breathing, eyes, teeth, skin and coat, and ask a veterinarian about persistent or painful signs.

What makes a cat flat-faced?

Brachycephaly means a shortened head or muzzle. In cats, that shape can affect more than the visible outline of the face: it may occur alongside narrowed nostril openings or nasal passages, prominent eyes, facial folds and changes in the mouth. How much these features are present differs between breeds and between cats of the same breed. Some breeding lines have been selected for more extreme profiles.

Persian, Burmese, Himalayan, British Longhair, British Shorthair, Exotic Shorthair and Scottish Fold are among breeds that may be described as brachycephalic. That list is not a diagnosis, nor does it mean every cat in each breed has the same degree of facial shortening. A cat’s observed breathing and comfort matter more than its label.

Compared with cats whose muzzles are not shortened, a more extreme flat face can bring additional risks related to airflow, eye exposure, tooth alignment and skin folds. Those risks are not certainties. Other breeds and individual cats can have health problems too; the useful comparison is the specific cat’s conformation, symptoms and care needs, not a claim that one broad group is universally healthy or unhealthy.

It is also possible for two cats with similar-looking faces to have different day-to-day needs. One may breathe comfortably and have no persistent eye or skin symptoms, while another with a more pronounced profile may need closer observation or veterinary care. Facial shape is a reason to be attentive, not a way to diagnose disease at home.

How health and care needs can differ

Area Flat-faced cats Cats without a shortened muzzle What to watch or do
Breathing Narrow nostrils or nasal passages and other airway changes may restrict airflow, especially in more extreme cases. The specific comparison depends on the individual cat; there is no single non-flat-faced baseline established here. Notice snoring or snorting, unusual sleeping positions, and difficulty breathing during activity. Discuss persistent noisy breathing with a veterinarian.
Eyes Prominent eyes and altered tear drainage may contribute to overflow, irritation, dryness or corneal exposure. Eye problems can occur in any cat; facial shape may add particular exposure or drainage concerns. Seek veterinary advice for persistent discharge, redness, squinting or signs of pain.
Teeth and eating A shortened face can be associated with crowded or misaligned teeth, which may affect eating or grooming. Individual dental needs vary; no general rate comparison is established by the cited material. Ask a veterinarian about difficulty eating, discomfort or a change in grooming.
Facial skin Folds can trap moisture and become irritated or inflamed; severe cases may involve dermatitis or infection. Fold-care needs depend on whether the cat has folds, not simply on whether it belongs to a particular breed group. Keep folds clean and dry as appropriate, and have persistent irritation or odor assessed.
Coat and grooming Needs vary by breed and coat. Persian and Himalayan coats are long; the Exotic Shorthair has short, coarse hair. Coat-care needs also vary between breeds and individuals. Choose grooming routines for the cat’s coat. TICA recommends daily combing for Persians to prevent tangles and mats.

The table describes potential differences, not a diagnosis or a measured comparison of disease rates. The cited sources do not establish a population-wide prevalence figure for these conditions or a universal health ranking for flat-faced and other cats. A practical comparison is therefore about what you can observe in a particular cat: how comfortably it breathes, whether its eyes stay comfortable, how it eats, whether folds remain healthy and how much coat maintenance it needs.

Breathing: which signs deserve attention?

In flat-faced cats, shortened muzzles can accompany internal upper-airway changes that make airflow harder. A UK government-hosted expert opinion identifies very narrow nostril openings, narrow nasal passages and a relatively long soft palate as possible contributors to brachycephalic obstructive airway syndrome. These are possible anatomical contributors, not findings that can be confirmed just by looking at a cat’s face.

Cats Protection identifies snorting or snoring, unusual sleeping positions and difficulty breathing while running around as signs to discuss with a veterinarian. Airway problems can interfere with exercise and sleep. Noisy breathing should not simply be written off as a harmless breed trait, particularly if it is persistent, worsening or accompanied by visible effort.

Veterinarian and feline specialist Dr. Ashlie Saffire of Faithful Friends Veterinary Clinic in Dublin, Ohio, has described some cats’ small nasal openings this way: “Their nasal openings are so small that they’re basically breathing through a straw.” This is an analogy, not a clinical measurement, but it conveys why a veterinarian may pay attention to nostril size as well as the cat’s breathing.

When observing your cat, consider both rest and ordinary movement rather than judging from a single moment. Note whether noisy breathing recurs, whether the cat adopts an unusual sleeping position, and whether normal play seems to bring on difficulty. These observations can help you describe what you have seen when asking for veterinary advice; they cannot determine the cause or severity on their own.

When to call a veterinarian

  • Ask for veterinary advice about recurrent snoring or snorting, unusual sleep positions, or breathing difficulty during ordinary play or movement.
  • Seek prompt advice if breathing seems labored, activity is followed by panting or struggle, or the cat’s breathing has changed. A veterinarian can assess the individual cat and decide what evaluation or management is appropriate.
  • Do not try to diagnose airway disease from the breed name or facial profile alone. A veterinary assessment is needed to determine whether a particular cat has a problem.

For a cat already known to have breathing concerns, ask the veterinarian what signs should prompt follow-up and what level of activity is suitable. Avoid using another cat’s experience as a guide: facial profiles and symptoms vary, even among cats of the same breed.

Eyes, teeth and facial folds

Eyes and tear overflow

Prominent eyes and altered tear drainage can contribute to persistent discharge, irritation, dryness or exposure of the cornea. Severe cases may include corneal ulceration. A little visible tear staining does not establish a particular condition, but persistent discharge, redness, squinting or apparent pain warrants veterinary advice. Avoid treating an eye problem at home without guidance; eye symptoms can have different causes and need an individual assessment.

Daily eye wiping is included in TICA’s Persian care recommendations, but wiping is grooming, not treatment for an ongoing eye problem. If discharge or irritation persists, ask a veterinarian whether the eye needs assessment and how to clean around it safely. Do not assume that repeated wiping alone will resolve a cause related to tear drainage or exposure.

Teeth and eating comfort

A shortened face can leave teeth crowded or misaligned. That may make eating or grooming uncomfortable or difficult. Notice how the cat eats and whether grooming changes; ask a veterinarian about difficulty chewing, signs of discomfort or a new reluctance to groom. Appearance alone cannot show whether a cat’s teeth are causing a problem.

Pay attention to changes rather than trying to judge tooth alignment from a photograph or breed description. A cat that seems less comfortable eating or has changed its grooming habits deserves an individual assessment. The same facial shape does not tell you whether teeth are crowded enough to cause difficulty.

Skin folds

Facial folds can become irritated when moisture and debris collect. Inflammation, dermatitis or infection are concerns in more severe cases. If the folds are part of a cat’s facial conformation, keep them clean and dry as appropriate, but do not scrub irritated skin or apply treatments without veterinary advice. Persistent redness, soreness, odor or discharge should be assessed rather than managed as routine grooming.

Regular observation can help distinguish routine care from a problem that needs attention: look for changes such as persistent redness, soreness, odor or discharge, and seek advice if they appear. Cleaning should be gentle and appropriate to the individual cat. If you are unsure what is safe for a particular fold, ask a veterinarian rather than trying products or treatments on irritated skin.

Heat, weight and activity

Practical precautions can reduce avoidable strain without assuming every flat-faced cat is heat-intolerant or unable to play. Cats Protection advises providing water and a cooler place away from heat in hot weather, monitoring weight and keeping activity gentle enough that the cat does not pant or struggle. Short, gentle play can help avoid overexertion. Humane World for Animals likewise advises keeping cats cool and dry in hot, humid weather and monitoring weight.

  • Keep an eye on weight, and ask a veterinarian for advice if you are unsure what is healthy for your cat.
  • Choose gentle activity and stop if the cat pants, struggles or has trouble breathing. Ask a veterinarian what level of exercise is suitable for that individual.

These measures are risk management, not proof that a flat-faced cat will have heat or exercise intolerance. A cat that struggles during activity needs individual veterinary advice rather than a one-size-fits-all exercise plan.

Weight monitoring is useful as part of general care, but it is not a substitute for checking breathing comfort or asking for veterinary advice. Likewise, gentle play should be adjusted to the cat in front of you: the aim is not to avoid all activity, but to avoid pushing a cat into panting, struggling or breathing difficulty. If you are uncertain whether a sign is normal for your cat, describe it to a veterinarian.

Grooming depends on the coat as well as the face

Facial shape does not determine coat length. TICA describes Persian, Himalayan and Exotic Shorthair cats as part of the Persian breed group: Persians and Himalayans have long hair, while Exotics have short, coarse hair. Do not apply a long-coat routine to every flat-faced breed.

Persian and Himalayan coats

TICA recommends combing a Persian daily with a metal comb to help prevent tangles and mats. Regular combing is particularly relevant to a long coat that can knot if neglected. TICA also recommends daily eye wiping and regular, careful facial cleaning for Persians. These are grooming recommendations; they do not prevent airway, dental or other medical problems.

For a Persian, a metal cat grooming comb is a practical tool for the daily combing TICA recommends. The purpose is to help prevent tangles and mats in the long coat, not to address breathing, eye disease or other medical concerns. If the coat is already badly tangled or the cat seems uncomfortable during grooming, ask a veterinarian or an appropriate grooming professional how to proceed rather than pulling at mats.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Other coats and facial cleaning

For an Exotic Shorthair or another short-coated cat, use a routine suited to that coat rather than assuming the Persian long-hair schedule applies. If a cat has facial folds or recurring eye discharge, ask a veterinarian how to clean the area safely and how often. Persistent discharge, irritation or odor is a reason for assessment, not simply more wiping.

Grooming time is therefore a breed and coat difference that sits alongside, but is separate from, the possible health effects of facial conformation. A long-haired flat-faced cat may need frequent coat combing; a short-coated flat-faced cat does not automatically need that same routine. Care for the face should likewise reflect the cat’s actual folds and symptoms.

What the owner survey does—and does not—show

The 2023 paper “Flat-Faced or Non-Flat-Faced Cats? That Is the Question” reports an online questionnaire completed by 278 owners of brachycephalic cats and 250 owners of non-brachycephalic cats. Respiratory disease was the main health problem reported for the studied brachycephalic breeds, and most surveyed owners of those breeds were unaware of its incidence.

Those figures count questionnaire respondents, not cats in a representative population sample. They do not establish what proportion of all flat-faced cats have respiratory disease, and the findings should not be presented as a population-wide prevalence estimate. They do underline why owners should know the possible breathing signs and raise concerns with a veterinarian.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The questionnaire can be useful as a reminder that respiratory concerns may be under-recognized by owners, but it cannot tell an owner whether an individual cat has an airway condition. Nor does the respondent count provide a percentage of all flat-faced cats affected. For an individual decision, observed signs and a veterinarian’s assessment are more relevant than applying survey numbers to every cat.

Do flat-faced cats live as long as other cats?

There is no single lifespan that applies to every flat-faced cat, and the cited material does not provide a universal numerical lifespan comparison between flat-faced and other cats. Cats Protection says a healthy, well-cared-for flat-faced cat can have a lifespan similar to that of other cats, while breed and health issues can affect it. That qualification matters: facial type alone cannot predict an individual cat’s lifespan, and known symptoms or health conditions deserve veterinary attention.

When considering a particular cat, avoid treating the breed label as a lifespan prediction. Health, care and any individual condition matter, and persistent breathing, eye, dental or skin problems merit attention in their own right. Cats Protection’s statement is conditional on the cat being healthy and well cared for; it is not a guarantee for every cat.

How to care for an individual flat-faced cat

  1. Observe breathing at rest and during ordinary activity. Note snoring or snorting, unusual sleeping positions, or difficulty breathing while playing. Discuss persistent or concerning signs with a veterinarian.
  2. Watch the eyes, mouth and facial skin. Seek advice for ongoing eye discharge, redness, squinting, eating difficulty, apparent discomfort, or irritated or odorous folds. Do not treat persistent symptoms as a grooming issue without an assessment.
  3. Match grooming to the coat. For a Persian, TICA recommends daily metal-comb grooming to prevent tangles and mats, along with daily eye wiping and regular careful facial cleaning. Other breeds and coat types may need a different routine.
  4. Make sensible adjustments for heat and activity. Provide water and a cool resting spot in hot weather, monitor weight, and keep play gentle enough that the cat does not pant or struggle. Ask a veterinarian about an individual exercise plan if breathing or fitness is a concern.
  5. Discuss breed-related screening where relevant. For Persians, TICA recommends discussing screening for polycystic kidney disease, respiratory problems, progressive retinal atrophy and hypertrophic cardiomyopathy with a breeder and veterinarian. These are topics for screening discussions, not a claim that every Persian will develop any of them.
  6. Base decisions on the cat, not the label. A veterinarian can assess facial anatomy, symptoms and overall health; the same breed name does not guarantee the same profile or care needs.

For screening questions, TICA’s Persian guidance gives owners a concrete starting point: discuss polycystic kidney disease, respiratory problems, progressive retinal atrophy and hypertrophic cardiomyopathy with a breeder and veterinarian. Screening recommendations are not predictions that a cat has, or will develop, one of these conditions. Ask which topics are relevant to the individual cat and what follow-up is appropriate.

Frequently Asked Questions

Are flat-faced cats healthy?

Some are healthy and comfortable, but a shortened muzzle can be associated with breathing, eye, dental and skin-fold concerns. The amount of flattening and its effects vary, so a breed name is not a diagnosis. Ask a veterinarian about persistent or painful signs.

Do flat-faced cats have breathing issues or snore a lot?

Some may have restricted airflow associated with narrow nostrils or nasal passages and other airway changes. Snorting or snoring, unusual sleeping positions, or difficulty breathing during activity are signs to discuss with a veterinarian; persistent noisy breathing should not simply be dismissed as a breed quirk.

Do flat-faced cats have dental and eye problems?

Facial shape may contribute to crowded or misaligned teeth, tear overflow, eye irritation, dryness or corneal exposure. These are possible concerns, not guarantees. Seek veterinary advice for eating difficulty, persistent discharge, redness, squinting or apparent pain.

How long do flat-faced cats live?

There is no single lifespan for every flat-faced cat. Cats Protection says a healthy, well-cared-for flat-faced cat can have a lifespan similar to that of other cats, though breed and health issues can affect it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How should I care for a flat-faced cat?

Observe breathing, eyes, eating and facial skin; seek veterinary advice for ongoing or painful signs; provide water and a cool resting place in hot weather; monitor weight; and keep activity gentle enough that the cat does not pant or struggle. Match grooming to the cat’s coat. TICA recommends daily metal-comb grooming for Persians to help prevent tangles and mats.

Do all flat-faced breeds need the same grooming?

No. Coat and facial-care needs vary. TICA describes Persians and Himalayans as long-haired and Exotics as short-haired with a coarse coat. Its daily long-coat combing recommendation is for Persians, not a universal routine for every flat-faced cat.

The Bottom Line

Flat-faced cats are not all unhealthy, but a shortened muzzle can bring added risks involving breathing, eyes, teeth and facial folds. Watch the individual cat, seek veterinary advice for persistent symptoms, and tailor grooming to coat type. Heat precautions, gentle activity and appropriate breed-related screening discussions can support care without assuming every cat will develop a problem.
