Hand scaling of root surfaces is a professional periodontal procedure. A clinician uses a periodontal curette below the gumline to remove calculus and subgingival biofilm from a tooth root inside a periodontal pocket. It is one form of subgingival instrumentation, an umbrella term that also includes sonic and ultrasonic devices.
Patients may hear this described as “scaling and root planing” or a “deep cleaning.” The instrument choice, number of visits and need for local anesthetic depend on the pocket anatomy, deposits, treatment objective and the clinician’s and patient’s preferences. This guide explains what an educational image should show and what the procedure means clinically; it is not a guide to using dental instruments at home.

What hand scaling of a root surface means
A periodontal pocket forms when the gum has separated from the tooth surface. Calculus (hardened deposits) and biofilm can collect in that subgingival space, where a toothbrush cannot reach. During hand scaling, a trained dental professional places a curette into the pocket and works along the accessible root surface to disrupt and remove those deposits.

“Root planing” is the patient-facing term often used for the root-surface part of scaling and root planing (SRP). The American Dental Association (ADA) describes scaling as removing plaque and tartar above and below the gumline, while root planing addresses the root surface down toward the base of a pocket. ADA patient information
How it fits into periodontal treatment
SRP is a nonsurgical treatment for periodontitis. The ADA guideline identifies SRP as the initial nonsurgical treatment for chronic periodontitis and reports a moderate benefit, with expected benefits judged to outweigh potential adverse effects. The recommendation still has to be integrated with the dental professional’s examination, clinical judgment and the patient’s needs and preferences. ADA evidence-based guideline
A recommendation for subgingival instrumentation is not made solely because a patient wants a more intensive cleaning. The clinician evaluates the periodontal diagnosis, pocketing and deposits, then determines which sites need treatment and whether treatment should be staged. A “deep cleaning” is therefore a treatment plan, not a particular brand of instrument or a fixed number of strokes.

- Assessment and treatment planning: The dental professional examines the gums and periodontal pockets, identifies sites with subgingival deposits and explains the proposed nonsurgical treatment. The extent and distribution of treatment determine whether one or several visits are appropriate.
- Comfort measures: SRP may require local anesthetic, especially when deposits are deep or the tissues are sensitive. The clinician discusses anesthesia and other comfort considerations before instrumentation begins.
- Access to the pocket: A periodontal curette is selected and positioned to reach the root surface below the gingival margin. Curettes are shaped for access to particular tooth surfaces and pocket areas; selection is a professional decision based on anatomy and visibility.
- Deposit removal: The clinician uses controlled instrumentation to remove calculus and disrupt subgingival biofilm from the accessible root surface. The objective is effective debridement while limiting unnecessary trauma to the gum and tooth.
- Staged treatment when needed: Extensive disease, patient comfort or site access may make more than one appointment appropriate. Different areas can be treated at separate visits rather than attempting every site in one session.
- Reassessment: A later visit evaluates healing and periodontal pocket depths. The findings help determine whether the nonsurgical response is satisfactory and what maintenance or additional periodontal care is needed.
Hand curettes versus powered instruments
No single instrument type is best for every tooth or every patient. The EFP and the Scottish Dental Clinical Effectiveness Programme (SDCEP) both describe hand and powered devices as valid options whose selection depends on the clinical situation, access, treatment goal and clinician and patient preferences. SDCEP instrument-choice guidance
| Aspect | Hand curettes | Sonic or ultrasonic instruments | Combined approach |
|---|---|---|---|
| Instrument type | Manual periodontal curette contacting the root surface. | Power-driven tip using sonic or ultrasonic energy. | Powered instrumentation and hand finishing or site-specific use. |
| Access considerations | Useful when tactile control and a particular root-surface adaptation are needed. | May offer efficient access in some deposits and pocket anatomies; tip selection and access remain clinician-dependent. | Allows the operator to match the instrument to each site. |
| Clinical trade-offs | Requires manual control and can be technique- and operator-sensitive. | Can differ from hand instruments in calculus removal, root-surface smoothness, soft-tissue trauma and treatment time. | May balance efficiency and tactile finishing, but adds planning and does not guarantee a better result at every site. |
| Evidence on outcomes | Systematic reviews summarized by the American Academy of Pediatric Dentistry (AAPD) report broadly similar treatment results across approaches; many clinicians combine instruments. AAPD best practices | ||
Powered devices can also create aerosol, noise or vibration concerns that the dental team manages according to the clinical setting. Those practical considerations do not make one category universally superior. The relevant question is which instrument or combination gives the clinician safe, effective access to the involved sites.
Root surfaces, cementum and the meaning of “planing”
Root surfaces are not identical to enamel. They may include cementum, the mineralized tissue covering the root. Instrumentation can remove some root-surface material along with calculus, but complete cementum removal is not a treatment goal.

Thus, an educational illustration should not depict the clinician stripping away an entire cementum layer or intentionally shaving a root smooth. It should show targeted removal of deposits and biofilm while preserving the underlying tooth as far as clinically possible.
Does scaling and root planing hurt?
The ADA notes that SRP may require local anesthetic and may be completed over more than one visit. After treatment, soreness can last a day or two, while sensitivity can persist for up to a week. These are general expectations, not a prediction for an individual patient; the depth and extent of treatment, existing sensitivity and medical circumstances affect the experience. ADA patient information
A patient should tell the dental team about pain that is severe, worsening or accompanied by other concerning symptoms rather than trying to instrument the area independently. The treating professional can determine whether the symptoms fit expected recovery or need assessment.

Follow-up is part of SRP, not an optional add-on. The ADA describes a review appointment to check healing and measure periodontal pocket depth. Its periodontitis information explains that periodontal tissues need approximately four weeks to show the optimal effects of nonsurgical treatment; this is an approximate clinical timeframe, not a promise that every pocket will reach a particular depth. At reassessment, the clinician considers the recorded pocket measurements, tissue response and the patient’s ongoing risk factors. The next step may be routine periodontal maintenance, further nonsurgical care at selected sites or referral for additional periodontal evaluation. A single appointment cannot establish the long-term result.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How an educational image should label the procedure
A useful cross-sectional image can explain the anatomy without turning into a self-treatment diagram. Include the following labels:
- Tooth crown and root: Distinguish the visible crown from the root below the gum.
- Gingival margin: Mark the edge of the gum and show that the instrument is below this margin.
- Periodontal pocket: Illustrate the space between the gum and root where deposits may accumulate.
- Calculus and biofilm: Use separate labels so readers understand that hardened calculus and soft biofilm are related but different deposits.
- Periodontal curette: Identify the hand instrument positioned against the root surface.
- Powered scaler tip: If shown for comparison, label it as a sonic or ultrasonic instrument rather than implying that it is the same tool as a curette.
- Root surface: Show the target surface without implying that all cementum is removed.
The illustration should avoid arrows that prescribe a force, angle or stroke pattern to a lay reader. The sources support the use of curettes and powered instruments, but they do not establish one universal hand technique suitable for every site. A caption can state that the image is an anatomical explanation of professional instrumentation and not a home-care instruction.

Why buying a scaler is not a substitute for care
Periodontal curettes and powered scaler tips are professional instruments used within diagnosis, infection-control procedures, anesthesia decisions and follow-up. A person cannot determine from a mirror alone whether a rough area is calculus, exposed root, a restoration or another condition. Attempting to scrape below the gum can cut tissue, damage a root or delay diagnosis.

The EFP guidance describes subgingival instrumentation as a professional treatment procedure. It does not support self-treatment with a purchased scaler. Home care should follow the individualized advice of a dental professional, while subgingival debridement remains part of professional periodontal therapy.
How common is the underlying problem?
An ADA MouthHealthy page cites a July 2015 Journal of the American Dental Association study reporting periodontitis in 47.2% of adults older than 30 in the United States. This is a historical U.S. estimate for that age group and study period, not a current worldwide prevalence figure. ADA MouthHealthy
The statistic explains why patients may encounter recommendations for periodontal evaluation, but it does not determine whether any particular person needs SRP. Diagnosis and treatment decisions remain site-specific.
Questions to ask at a dental appointment
- Which periodontal sites require subgingival instrumentation, and what findings support that recommendation?
- Will treatment be completed in one appointment or staged, and why?
- Will local anesthetic be used?
- Are you planning hand curettes, powered instruments or both for my specific sites?
- When will pocket depths and healing be reassessed?
- What symptoms after treatment should prompt a call to the office?
These questions help connect the generic phrase “deep cleaning” with the diagnosis, instruments and follow-up plan actually proposed for the patient.
