Horse Toothache Care: How Equine Dental Specialists Find and Treat Oral Pain

Dropped feed, drooling, weight loss and bit resistance can signal painful dental disease. Learn what specialists investigate, when referral is needed and why owners should not attempt dental procedures at home.

A horse that drops feed, chews slowly, resists the bit or suddenly changes behavior may have a painful dental problem, but those signs do not identify the cause. The safest route is a prompt examination by your primary-care equine veterinarian, who can diagnose oral disease, provide pain control and refer the horse to a veterinary dental specialist when advanced care is needed.

Equine dental specialists do more than smooth sharp edges. They evaluate the teeth, gums, mouth, jaw and related facial structures, using sedation, detailed examination and imaging when appropriate, then discuss treatment options based on the findings. Relief is possible, but neither a “toothache” nor an immediate outcome can be assumed from behavior alone.

How can I tell if my horse may have a painful dental problem?

Watch for changes while the horse eats, drinks, works and interacts. The American Association of Equine Practitioners (AAEP) lists these warning signs:

  • Dropping partially chewed balls of hay or feed (quidding)
  • Difficulty chewing or taking longer to finish a meal
  • Excessive drooling
  • Weight loss or poor body condition despite an apparently adequate ration
  • Foul odor from the mouth or a nasal discharge
  • Resistance to the bit, head tossing or other new problems when ridden
  • A change in attitude or behavior, including irritability

These signs warrant professional attention; they are not a diagnosis. A horse may show similar behavior with a bit fit problem, gastrointestinal disease, sinus disease, injury or another condition. Conversely, some dental disease produces few obvious signs until it is advanced. AAEP’s June 30, 2026 owner guidance puts the practical point plainly: “It is important to catch dental problems early.”

Call your veterinarian when eating habits, weight, comfort or behavior changes. Tell the practice what you observed, when it began, whether the horse can eat and drink normally, and whether there is swelling, odor or discharge. Do not wait for a horse to stop eating before seeking advice.

What conditions can feel like a horse toothache?

Owners commonly use “toothache” to describe oral pain. Clinically, that term can cover several different problems. An examination is needed to distinguish them.

Sharp enamel points

Horse teeth continue to erupt and wear throughout life. Uneven wear can leave sharp points that irritate the cheeks or tongue and make chewing uncomfortable. A horse may drop feed, chew on one side or resist the bit. The location and severity of the points determine whether treatment is needed.

Hooks and overgrowths

A hook is an overgrown portion of a tooth that can interfere with the opposing tooth or injure soft tissue. AAEP notes that hooks may cause interference and mouth lacerations. Removing an overgrowth requires a controlled examination; trying to reach it with a hand tool can injure both horse and handler.

Wave mouth and abnormal wear patterns

In a wave mouth, the chewing surface develops an uneven wave-like profile. This can impair grinding and, in serious cases, contribute to digestive problems because feed is not adequately broken down. Management may require staged treatment and follow-up rather than one aggressive correction.

Abscesses, decay and infected teeth

A diseased tooth can become infected or develop decay. An abscess may cause pain, swelling, odor or drainage through the mouth or nose. Dental infection can also involve tissues around the tooth or adjacent sinus structures, so a specialist may recommend imaging when the source is not visible during a routine examination.

Malocclusion and missing teeth

Malocclusion means the upper and lower teeth do not meet normally. Missing teeth can leave opposing teeth without a wearing partner, allowing them to overgrow. Both situations can create areas that trap feed or interfere with chewing and may need a long-term management plan.

What does an equine dental specialist do?

AAEP defines equine dentistry as the evaluation, diagnosis, prognosis, treatment and prevention of conditions affecting the teeth, oral cavity, mandible, maxillofacial area and associated structures, including links between systemic disease and oral health. A specialist investigates the source of pain rather than treating a behavior in isolation.

Recognize the credential

An AVDC Equine Dental Specialty Diplomate (AVDC-EQ) is a licensed veterinarian who meets the American Veterinary Dental College’s entrance requirements and passes its equine examination. In Europe, a European Veterinary Specialist in Equine Dentistry follows the European Veterinary Dental College pathway, including the required training, residency and examination. “Equine dentist” is not by itself a uniform credential, so ask which veterinary license, board certification and referral relationship the provider holds.

Expect a staged examination

The exact sequence depends on the horse and the suspected problem. A veterinary dental visit may include:

  1. History and triage. The veterinarian reviews feed intake, weight change, drooling, odor, nasal discharge, ridden behavior, previous dental work and medications.
  2. General and external examination. The head, jaw, facial symmetry, lymph nodes and chewing muscles are assessed for swelling, pain or other clues.
  3. Safe oral inspection. A thorough examination of the mouth is performed with appropriate restraint. Sedation may be used when needed for safety and a complete view; it is a veterinary procedure.
  4. Imaging when indicated. Intraoral images, dental radiographs or endoscopy can reveal roots, diseased teeth, sinus involvement or lesions hidden from a surface view. A specialist may sometimes recommend CT for complex cases. These tests are not necessary for every routine visit.
  5. Diagnosis and options. The veterinarian explains which findings account for the signs, what is urgent, whether treatment can be staged and what follow-up is required. EVDC describes discussing options after the examination rather than promising a particular result in advance.

Treatment might involve carefully correcting an overgrowth, managing periodontal disease, treating an infected tooth or referring for extraction, oral surgery or restorative work. The objective is comfortable function and protection of the remaining mouth, not simply making every tooth surface look flat. Pain relief and aftercare are individualized; a specialist cannot guarantee instant relief before the underlying condition is identified.

When should I call a primary veterinarian, and when is a specialist referral appropriate?

Start with your horse’s primary-care veterinarian whenever pain, changed eating or weight loss appears. The veterinarian can determine whether the problem is dental, address immediate welfare concerns and coordinate sedation, analgesia, testing and treatment. AAEP advises referral when care is beyond the veterinarian’s training or equipment.

A referral to a veterinary dental specialist is especially useful for advanced, unusual or unresolved cases, including:

  • Extractions that are difficult because of tooth position, roots or fracture
  • Severe or persistent periodontal disease
  • Suspected dental-related sinus infection or other sinus disorder
  • Oral masses or jaw fractures
  • Cases needing root-canal or restorative treatment
  • Pain or eating problems that continue despite an apparently routine correction
  • Findings that require specialized imaging or a second opinion

Some specialists accept referrals from a primary veterinarian, while others may advise the owner to involve that veterinarian first. Ask how records and images will be shared and who will provide follow-up care.

Routine care versus specialist care

Consideration Primary-care equine veterinarian Veterinary dental specialist
Typical case Preventive examination, common abnormalities and initial assessment of new signs Advanced, complicated, recurrent or unresolved oral disease
Credential Licensed veterinarian with equine medical responsibility Licensed veterinarian with AVDC-EQ or EVDC specialist credentials, where applicable
Diagnostics History, physical examination and oral assessment; referral if more is needed May add detailed intraoral imaging, radiographs, endoscopy or CT when findings justify it
Treatment scope Care within the veterinarian’s competence, with referral for complex procedures Advanced extractions, periodontal, sinus-related, restorative or surgical management within the provider’s training
Care relationship Coordinates prevention, general health and ongoing records Usually works with the primary veterinarian on a referred problem and follow-up plan

This is a care pathway comparison, not a promise that every specialist visit requires imaging or surgery. The examination determines what is appropriate.

Who may legally perform equine dental work?

Rules differ by country, state and province. AAEP’s 2025 position states that invasive oral procedures, prescribing or administering sedatives and analgesics, dental radiography, oral endoscopy, treatment of diseased teeth and extractions are veterinary dental procedures for licensed veterinarians. AAEP’s position on healthcare-provider roles adds that dental technicians must work within local statutes and veterinarian supervision; sedated floating should be under a veterinarian’s immediate supervision and within a valid veterinarian-client-patient relationship. Extractions and oral surgery belong to licensed veterinarians.

Because practice acts vary, ask the provider what license they hold, which veterinarian is responsible for the horse and how supervision will occur. Do not assume that a person advertising “floating” or “equine dentistry” is legally permitted to sedate, diagnose infection, take radiographs or extract a tooth in your jurisdiction.

What owners should not do at home

Do not try to float teeth, extract a tooth, sedate a horse, probe a suspected abscess or use dental instruments without professional training and veterinary direction. A painful mouth can make a normally quiet horse react suddenly, and an instrument can fracture a tooth, cut soft tissue or push feed and debris deeper into a wound. Consumer supplements, oral-care products and generic dental tools are not substitutes for diagnosis or treatment.

While waiting for the appointment, provide the usual safe access to water and feed unless your veterinarian gives different instructions. Note what the horse can and cannot eat, save a photograph of visible swelling or discharge if it can be taken safely, and avoid forcing a bit into a painful mouth.

How often should a horse have a dental examination?

AAEP’s 2025 owner guidance recommends a dental examination at least once a year. Horses older than 10 may need more frequent examinations, such as every six months, particularly when dental disease is already present. That is general guidance, not an individualized prescription: age, tooth-loss pattern, diet, previous procedures and current symptoms determine the schedule your veterinarian recommends.

Keep a record of examination dates, findings, images, procedures, medications and the next review interval. Regular checks can identify wear abnormalities before they become an obvious eating or riding problem, but a scheduled visit does not replace an earlier call when new signs develop.

How to choose and prepare for a provider

  1. Call your primary-care equine veterinarian first if the horse has pain signs, weight loss or changed eating.
  2. Ask whether the case needs referral and, if so, whether the veterinarian recommends an AVDC-EQ diplomate, an EVDC specialist or another appropriately trained veterinarian.
  3. Confirm the provider’s license, specialist credential, supervision arrangements and the procedures performed at the facility.
  4. Send the horse’s dental history, diet, current medications, previous radiographs and a concise timeline of signs.
  5. Ask which parts of the visit may involve sedation or imaging, what monitoring is provided and how pain control and follow-up will be handled.
  6. Request a written plan that distinguishes findings, recommended treatment, alternatives, warning signs and the next examination date.

A clear referral plan helps prevent a routine “floating” appointment from overlooking a diseased tooth, and it ensures that advanced procedures are performed by the appropriately qualified veterinarian.

The practical answer to a suspected horse toothache

Behavior can alert you, but it cannot tell you which tooth or condition is responsible. Contact a veterinarian when your horse drools, drops feed, loses weight, develops odor or nasal discharge, struggles to chew, resists the bit or changes behavior. A veterinary dental specialist can then investigate difficult cases with a complete oral examination and, when justified, imaging or advanced treatment. The safest way to soothe oral pain is timely diagnosis, lawful veterinary care and a follow-up plan tailored to that horse.
