How Often Should Older Cats Have Their Blood Pressure Checked?

ISFM recommends annual blood pressure checks for healthy cats aged 7–10 and checks every 6–12 months from age 11. Illness or possible organ damage can mean sooner assessment and closer monitoring.

Yes. Older cats should have their blood pressure checked regularly, even when they seem well. The 2017 International Society of Feline Medicine (ISFM) consensus guideline recommends at least annual checks for healthy cats aged 7–10, and checks every 6–12 months for healthy cats aged 11 and older. Cats with certain illnesses or signs of possible hypertension-related damage need assessment sooner and closer follow-up.

How often should an older cat have a blood pressure check?

Use age and health status as a starting point, then ask your veterinarian to tailor the schedule to your cat. ISFM’s recommendations are:

As an Amazon Associate I earn from qualifying purchases.

Cat’s situation Suggested blood pressure schedule
Healthy, age 7–10 years At least every 12 months
Healthy, age 11 years or older At least every 6–12 months
Recognized risk factor or evidence of target-organ damage Measure promptly and reassess at least every 3–6 months

These are screening recommendations, not a diagnosis or a fixed schedule for every cat. The ISFM guideline dates from 2017, and the 2021 American Association of Feline Practitioners (AAFP) Senior Care Guidelines also place blood pressure in the minimum diagnostic assessment for apparently healthy and ill senior cats, with evaluation individualized to the patient.

Why screen before a cat looks sick?

Hypertension is more common in older cats and may not be obvious until it has caused damage. Potential target organs include the eyes, brain, kidneys, and cardiovascular system; some damage may be irreversible. ISFM reports a median age of 13–15 years at feline hypertension diagnosis. Waiting for visible problems can therefore mean waiting until harm has occurred.

Hypertension in cats is often associated with another illness, particularly chronic kidney disease or hyperthyroidism. The underlying condition may cause or contribute to high pressure, so a veterinarian needs to assess and manage both. A minority of cats have no identified underlying cause.

Which cats may need checks more often?

Discuss prompt measurement and closer monitoring with the veterinarian if your cat has a recognized risk condition or evidence of possible target-organ damage. ISFM specifically highlights chronic kidney disease and hyperthyroidism, including cats receiving treatment for hyperthyroidism. Other illnesses and some drug therapies can also affect risk. The veterinarian should set the interval for the individual cat; the guideline recommends reassessment at least every 3–6 months for risk groups.

Changes involving vision or the eyes, unusual neurologic signs, or other concerns should be raised promptly rather than waiting for the next routine screening date. This is not a complete symptom checklist, and an owner cannot confirm hypertension from signs alone.

What does a high reading mean?

A high clinic result does not automatically establish persistent hypertension. Stress or arousal can raise a cat’s blood pressure, and readings may also vary with the operator, setting, equipment, the cat’s position, cuff size, and measurement site. The 2022 ISFM/AAFP Cat Friendly Veterinary Environment Guidelines address the clinical environment, which can affect feline stress. The veterinarian interprets readings in context and may repeat measurements using an appropriate technique.

The IRIS systolic categories reproduced in the 2017 ISFM guideline describe estimated risk of target-organ damage, not a home diagnostic rule:

Systolic blood pressure IRIS category Estimated risk
Less than 150 mmHg Normotensive Minimal
150–159 mmHg Borderline Low
160–179 mmHg Hypertensive Moderate
180 mmHg or above Severely hypertensive High

These categories help guide veterinary assessment; they do not mean a single reading by itself proves disease or predicts that damage has occurred. Ask how the result was obtained, whether it should be repeated, and how it fits with your cat’s health and any other test results.

How is blood pressure measured at a veterinary clinic?

Clinics commonly use indirect measurement with Doppler sphygmomanometry or oscillometric equipment. ISFM notes that traditional oscillometry may be less accurate than Doppler in conscious cats and may underestimate higher values. It describes high-definition oscillometry as a newer option with encouraging results but less extensive feline evidence.

Good technique matters as much as the number on the display. A reliable assessment takes account of equipment and cuff fit, measurement site and position, the cat’s state of arousal, and repeat readings. For that reason, a generic human cuff or an unverified consumer monitor should not be used to diagnose or treat a cat at home; the reviewed veterinary guidance does not establish such devices as substitutes for clinical measurement.

What to ask at your cat’s next visit

  • Given my cat’s age and health, when should blood pressure be checked next?
  • Does a current or past condition, treatment, or medication mean checks should be more frequent?
  • If a reading is elevated, how will you confirm and interpret it?
  • Can we keep a record of readings over time and review the trend?

The AAFP senior-care guidance notes that progressive increases over serial readings may help a veterinarian distinguish a situational elevation from persistent hypertension that may require medical treatment. Bring previous readings or test records if you have them, and agree on a follow-up plan with the veterinarian.
