A U.S. veterinary practice should not dispose of all clinical waste the same way. Split the waste into separate streams, apply federal hazardous-waste rules to anything that is listed as hazardous or exhibits a hazardous characteristic, and follow the state and local medical-waste program for infectious waste and sharps. Hazardous-waste pharmaceuticals should never go down a drain as a routine step.
Exact packaging, labels, storage limits, records and approved vendors depend on the practice’s state and locality, which no general guide can settle. This article explains the federal framework, the points where practices most often go wrong, and what to confirm locally.
Why hazardous waste and medical waste follow different rules
In everyday speech, anything dangerous that leaves a clinic gets called hazardous waste. Under U.S. federal rules the terms mean different things, and the difference determines who regulates the waste and how.
- RCRA hazardous waste is waste that is listed as hazardous or exhibits a hazardous characteristic under the federal Resource Conservation and Recovery Act framework.
- Regulated medical waste is infectious or related clinical waste. The U.S. Environmental Protection Agency says medical waste is primarily regulated by state environmental and health departments, and that state programs differ.
Infectious character alone does not make a waste RCRA hazardous. EPA’s Medical Waste page states that medical waste is not classified as RCRA hazardous just because of its infectious nature. The two categories can overlap: a waste that is medical waste and also exhibits a hazardous characteristic, or that contains or is mixed with listed hazardous waste, can be subject to both RCRA and medical-waste controls at once. Keep the categories separate in your planning, and do not assume that compliance with one covers the other.

Hazardous-waste pharmaceuticals: no routine drain disposal
Veterinary clinics fall within the federal pharmaceutical rules
EPA’s frequently asked questions on the management standards for hazardous waste pharmaceuticals identify veterinary clinics and hospitals among the healthcare facilities the program covers. Coverage of the program does not mean every discarded drug is a hazardous-waste pharmaceutical. The practice must determine which discarded products are hazardous, what generator category applies to the facility, and which management pathway the rules require. Document those decisions before the first disposal.
Why the sewer prohibition matters
EPA’s April 2022 factsheet on its ban on sewering pharmaceuticals explains that the prohibition applies to healthcare facilities, which include veterinary facilities, and describes limited regulatory exceptions. Treat the prohibition as the default. In practice that rules out flushing expired injectables or tablets and pouring leftover liquid medication into a sink or floor drain, unless someone has confirmed that an exception applies to that specific waste.

Drugs that are not hazardous waste, and controlled substances
The sewer prohibition described above covers hazardous-waste pharmaceuticals. Whether a non-hazardous drug may go to a sewer, the regular trash or a medical-waste vendor is a state and local question, so do not treat the absence of a federal rule as permission. Controlled substances are governed by a separate federal drug-control framework that this article does not cover; check those requirements before any take-back or destruction step.
Sharps: contain them at the point of use
Sharps are their own containment category. Loose needles and blades injure the people who handle the waste, and a container that can be punctured defeats its purpose. EPA’s I-WASTE disposal guidance describes a closed, rigid, break-resistant, puncture-resistant container for sharps.

Whether a particular sharp is also regulated as hazardous waste depends on the material and the applicable rules. Do not assume it either way. Use your state medical-waste program and your contractor to settle the classification and pickup.
Set up the sharps stream as follows:
- Drop sharps into the container at the point of use, rather than collecting them in a bag or open tray for later sorting.
- Close and seal the container when it reaches the fill line your local rules or contractor specify.
- Mark and label the container as your local rules and contractor require, since those specifications vary.
- Store the closed container for only as long as, and under the conditions that, state and local rules allow. Storage time and temperature limits come from those rules, not from the federal guidance cited here.
Regulated medical waste follows state and local programs
For infectious waste and other regulated medical waste, the governing rules come from the state. EPA states: “Medical waste is primarily regulated by state environmental and health departments.” Its Medical Waste page notes that requirements vary from state to state. Depending on the state, the environmental agency, the health department, or both may hold this authority. Ask the responsible agency about definitions, packaging, treatment, transport, records and disposal.

A qualified collection and treatment vendor can carry out the approved pathway. Before you sign, confirm the vendor’s authorization in your jurisdiction, the treatment facility it uses, and whether it accepts each waste stream your practice generates.

- Inventory the streams. Walk every treatment room, pharmacy shelf, surgical area and lab bench, and list each waste type generated there.
- Set containers at the point of generation. Give each stream its own clearly identified container, and keep sharps out of general waste.
- Classify drugs before they leave the room. Decide whether each discarded drug is a hazardous-waste pharmaceutical, and route it through the pharmaceutical pathway rather than a sink or ordinary trash.
- Define the route for each stream. Name the person responsible, the storage location, the pickup or shipment method and the records you will keep.
- Get local requirements in writing. Use the checklist below to obtain answers from the state agencies and your vendor, and file them with the plan.
- Train and retrain staff. Do not combine pharmaceutical hazardous waste, chemical hazardous waste, sharps and other regulated medical waste in one container unless the applicable rules and the vendor’s instructions allow it.

A provider has to be legally able to take your waste, so the criteria below come before price. Costs depend on your volume and on local vendors, and this article does not establish a typical price range.

- Waste classes accepted. Confirm whether the provider handles hazardous-waste pharmaceuticals, chemical hazardous waste, or only regulated medical waste and sharps.
- Authorization and destination. Confirm state and local authorization and the treatment or disposal facility where the waste ends up.
- Containers and packaging. Check that the provider’s containers match your state’s requirements and the sharps specifications.
- Pickup and records. Ask about pickup frequency, documentation, and manifest or other record support where applicable.
- Quoted cost for your actual volume. Request a quote based on your practice’s real waste volume, not a generic package price.
Details to confirm for your state and locality
Federal guidance alone cannot answer the following. Take these questions to your state environmental agency, your state health department, any local authority with jurisdiction, and your waste vendor:
- The state definition of regulated medical waste.
- Sharps rules, including approved containers, labels and fill lines.
- Storage time and temperature limits.
- Treatment options available to your practice.
- Transporter and treatment-vendor authorization.
- Required records.
- Any state hazardous-waste rules that are more stringent than federal rules.
- Current rules for hazardous-waste pharmaceuticals in your state.
