The Age-Friendly Health Systems 4Ms—What Matters, Medication, Mentation, and Mobility—turn that principle into repeatable clinical actions. The framework is a care-delivery method, not a building-design certification or a guarantee of better outcomes.
Hospitals should pair the 4Ms with interdisciplinary workflows, staff training, measurement, and reliable follow-through. In the United States, they must also distinguish this clinical framework from the separate CMS Age Friendly Hospital Measure, whose reporting specifications can change.

What is a senior-friendly hospital?
A senior-friendly hospital is organized around the needs and goals of older adults rather than treating age as a diagnosis. Clinicians ask what outcomes the person wants, review medicines for harm and burden, identify cognitive problems, and help the patient move safely. Those actions are coordinated across nurses, physicians, pharmacists, therapists, aides, social workers, and caregivers.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsThe Age-Friendly Health Systems framework describes this work through four connected domains: What Matters, Medication, Mentation, and Mobility. Knowing a patient’s priorities can change which medicines are worthwhile, how aggressively to pursue tests, what mobility goal is realistic, and how the team responds to confusion. The 4Ms are therefore a practical clinical framework, not a label that proves every process is consistently delivered. The 2022 health-system implementation review describes the framework and the organizational work needed to scale it.

“Senior-friendly” also does not automatically mean a hospital complies with every accessibility, fire, construction, or dementia-design requirement. Those obligations depend on the hospital’s jurisdiction, facility type, renovation scope, and applicable codes. Project teams should consult the relevant authorities and qualified architects or engineers for physical-design decisions.
What are the 4Ms in age-friendly care?
What Matters: discover the person’s goals
What Matters means eliciting the older adult’s health goals, preferences, values, and acceptable trade-offs, then aligning treatment with them. A useful conversation asks what the patient hopes to be able to do after discharge, what outcomes would make treatment worthwhile, who should participate in decisions, and what burdens the person wants to avoid.
Document the patient’s words in a place the whole team can see. “Return home and walk to the bathroom independently” leads to different decisions than “maximize longevity regardless of treatment burden.” Revisit the conversation when the patient’s condition, prognosis, or decision-making capacity changes. If a surrogate is involved, record the patient’s known wishes and the surrogate’s role rather than treating family preference as a substitute for the patient’s voice.
Medication: make every drug earn its place
Medication work starts with the best possible reconciliation: compare the patient’s actual use with admission orders, outpatient records, pharmacy information, and caregiver knowledge. Review indications, doses, duplication, interactions, renal or hepatic considerations, and medicines that increase falls, bleeding, low blood pressure, or cognitive impairment. Identify potentially inappropriate or high-risk drugs and consider deprescribing when the expected benefit no longer matches the patient’s goals, function, or life context.
A pharmacist, prescriber, nurse, and patient or caregiver should agree on changes and explain what happens next. The discharge list must state which medicines stop, start, or change, why the change was made, and who will monitor it. A technically complete list without a response to identified risk is not medication optimization.

Mentation: prevent and respond to cognitive harm
Mentation covers delirium, dementia, and depression. Establish the patient’s baseline cognition and function, then use a validated process to identify new inattention, disorganized thinking, altered alertness, or other delirium signs. Screening should trigger an action pathway: look for causes such as infection, pain, constipation, urinary retention, dehydration, hypoxia, medication effects, or sleep disruption; correct reversible contributors; and communicate changes promptly.
Delirium prevention and management generally rely on coordinated, nonpharmacologic measures rather than a single drug. Depending on the patient, that can include regular orientation, hearing and vision aids, sleep protection, hydration and nutrition support, pain treatment, toileting, familiar people, and early activity. The 2024 review “Optimizing Delirium Care in the Era of Age-Friendly Health System” discusses this multicomponent approach. Medication used for severe agitation requires a clinical indication and monitoring; sedation should not become the default response to an unmet need.

Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Mobility: make safe movement routine
Mobility means assessing baseline and current function, identifying safety limits, and creating repeated opportunities to move during the hospital stay. Ask how the patient normally transfers, walks, and uses stairs; check strength, balance, orthostatic symptoms, lines, footwear, and assistive-device needs; and involve physical or occupational therapy when indicated.
Write a specific mobility plan: for example, transfer with one staff member and a walker, sit in a chair for meals, or walk a defined distance three times daily. Nurses and aides need to know the plan and document whether it occurred and why it was deferred. Safe movement is a clinical and workflow responsibility. A mobility device or a fall-prevention poster alone does not make a hospital age-friendly.
How can a hospital turn the 4Ms into daily workflows?
Implementation works when each M has an owner, a documented action, and a reliable handoff. A practical sequence is:

- Secure leadership and physician sponsorship. Name an executive sponsor and an interdisciplinary implementation team. Explain why the change matters to patients, families, and staff, and give clinicians authority to remove workflow barriers.
- Map the current and ideal workflow. Follow an older adult from emergency arrival through admission, daily rounds, transfer, and discharge. Mark where goals, medicines, cognition, and mobility are assessed, where information is lost, and which electronic-record fields or alerts create unnecessary work.
- Define a minimum 4Ms bundle. Specify who asks the What Matters questions, who completes reconciliation, when mentation screening occurs, what constitutes a positive result, and how mobility orders are communicated. Keep documentation concise enough for frontline use.
- Train every role that touches the patient. Education should include recognition, communication, escalation, and practice with real scenarios. Nurses, physicians, pharmacists, therapists, aides, case managers, and volunteers may each have a different task in the same bundle.
- Build response pathways, not just screens. A positive delirium screen should open a cause-and-prevention checklist; a high-risk medication should prompt review; a mobility limitation should result in assistance or therapy; a stated goal should appear in rounds and discharge planning.
- Use electronic data carefully. Pull completion and response data from the record where possible, but audit a sample of charts to ensure that a checked box represents meaningful action. Avoid adding duplicate fields that staff cannot maintain.
- Run iterative Plan-Do-Study-Act cycles. Start with one unit, test a small change, review misses with frontline staff, and adjust the process before spreading it. Share results visibly and recognize teams that solve practical problems.
- Connect inpatient and post-acute care. Send the patient’s goals, medication changes, cognitive status, mobility level, equipment needs, and follow-up responsibilities to the receiving facility, primary-care practice, and family or caregiver. Transitions are part of the intervention, not an administrative afterthought.
Implementation experience emphasizes leadership communication, physician buy-in, frontline culture, workflow fit, staff education, and attention to transitions. Non-physician leaders can initiate change, but broad adoption usually requires clinicians who can integrate the 4Ms into ordinary rounds and orders. Cross-system implementation findings also warn that inpatient and ambulatory programs can become disconnected if ownership stops at discharge.

A 2022 study on an Acute Care for Elders unit illustrates both feasibility and the difficulty of achieving all four domains reliably. The academic hospital enrolled 519 eligible patients admitted from April 2021 through January 2022. Its reported process-goal results were:
| 4Ms process goal | Patients meeting goal | Share of 519 eligible patients |
|---|---|---|
| Medication | 454 | 87% |
| Mentation | 187 | 36% |
| What Matters | 130 | 25% |
| Mobility | 6 | 1% |
These are process results from one unit and one implementation period, not universal benchmarks and not proof that the program caused better clinical outcomes. The authors wrote, “We found implementing 4M care processes at an academic hospital to be feasible.” Their project mapped current and ideal workflows, trained staff, used electronic-record data, and applied iterative improvement cycles. Read the full implementation study for its inclusion criteria and methods.
The uneven percentages are instructive. Medication reconciliation may fit an existing admission process, while documenting personal goals, completing repeated cognitive assessments, and creating frequent mobility opportunities require more coordination. Leaders should measure each M separately instead of reporting a single composite score that hides weak domains.
How can a hospital prevent delirium in older patients?
Begin with a baseline: ask the patient or caregiver about usual alertness, memory, communication, sleep, and function. Screen at a defined frequency and whenever staff notice a change. When the screen is positive, notify the responsible clinician and investigate reversible causes rather than labeling the patient as “confused.”

Nonpharmacologic prevention is usually multicomponent. A protocol should state who performs each action, how completion is recorded, and what happens when the patient cannot participate. Do not present an orientation aid, clock, or single checklist item as a stand-alone delirium treatment.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do hospitals help older adults stay mobile?
Mobility begins with a baseline and a safety screen, not with a generic “up as tolerated” order. Record transfer ability, walking aid, distance, continence needs, dizziness, pain, oxygen or line requirements, and supervision level. Reassess after procedures, medication changes, a fall, or a change in alertness.
Make movement opportunities visible in the care plan and routine: sitting up for meals, transferring to a chair, walking to the bathroom, and scheduled hallway walks when safe. Coordinate therapy recommendations with nursing and aide assignments so the patient is not left waiting for a single specialist. If mobility is deferred, record the clinical reason and a plan to try again. At discharge, communicate the person’s functional level, equipment, assistance needs, and follow-up therapy.
Can the hospital environment support orientation?
Environmental supports can reduce avoidable confusion, but they complement rather than replace clinical care. Clear signage, consistent room cues, good lighting, visible calendars, and access to glasses or hearing aids may help patients understand where they are and what is happening. A large-display digital clock or easy-to-read bedside clock can be offered as an ancillary orientation aid when it suits the patient; no particular product or model has been established as a clinical intervention.

Do not prescribe a universal room layout, door width, bathroom configuration, alarm system, or construction standard without knowing the jurisdiction, facility classification, and project scope. Accessibility, fire safety, infection prevention, and building-code requirements come from applicable authorities and qualified design professionals. A hospital can improve its 4Ms workflows in an existing building, while a renovation requires a separate, code-based design process.

What does the CMS Age Friendly Hospital Measure require?
The CMS Age Friendly Hospital Measure is separate from the 4Ms clinical framework. A 2025 policy analysis describes it as a five-domain reporting measure for hospitals in the CMS Inpatient Quality Reporting program, with reporting beginning January 1, 2025. The domains described are:
| CMS measure domain | What it addresses |
|---|---|
| Patient health-care goals | Eliciting the older adult’s goals |
| Responsible medication management | Safer, more appropriate medication use |
| Frailty screening and intervention | Identifying frailty and responding to it |
| Social vulnerability | Recognizing social factors that affect care |
| Age-friendly care leadership | Organizational accountability and leadership |
The analysis says reporting is intended to be visible to older adults and family caregivers through CMS Care Compare. It describes reporting as the requirement, while attestation content has separate implications; do not assume that an affirmative attestation is required unless the current specifications say so. Hospitals should verify the latest CMS measure documents, technical specifications, submission calendar, and definitions before using the January 1, 2025 date for compliance planning. See the 2025 policy analysis.
The relationship is complementary: a hospital can use the 4Ms to organize bedside care and use the CMS measure to meet a reporting obligation. The domains overlap conceptually but are not interchangeable, and success on one does not prove success on the other.

How should leaders measure and sustain a senior-friendly program?
Track each domain with both completion and response measures. Examples include the proportion of admissions with a documented goal, the proportion of medication reconciliations reviewed by a qualified clinician, positive mentation screens with a documented response, and patients receiving the planned mobility opportunities. Add transition measures such as medication discrepancies identified after discharge or whether goals and mobility status reached the next care setting.
Stratify results by unit, shift, patient characteristics, and workflow stage to find inequities and bottlenecks. Review a small sample of records and speak with patients and caregivers; a high completion rate can still conceal checkbox documentation or care that did not match the person’s wishes. Display unit-level results, discuss misses without blame, and assign a named owner and due date for each corrective action.
Sustainability depends on orientation for new staff, periodic refresher training, reliable supplies and equipment, leadership rounding, and feedback loops that reach frontline teams. Revisit the bundle when staffing, electronic records, formularies, or discharge partners change. Treat the 4Ms as ordinary care rather than a temporary campaign.
Quick Recap
Common mistakes to avoid
- Calling a designation an outcome. A 4Ms adoption claim or age-friendly label does not guarantee better results for an individual patient.
- Screening without action. A delirium, frailty, medication, or mobility screen has value only when it triggers assessment and support.
- Making one profession responsible for everything. Each M needs clear interdisciplinary roles and handoffs.
- Stopping at admission. Goals, medication decisions, cognition, and mobility can change daily and during transfers.
- Using the study percentages as targets. The 87%, 36%, 25%, and 1% figures come from one academic unit and period; they are not universal standards.
- Confusing the CMS measure with the 4Ms. The CMS measure has five reporting domains and time-sensitive specifications.
- Assuming a product makes a hospital senior-friendly. Clocks, mobility aids, and other environmental supports are ancillary to coordinated clinical care.
- Giving code advice without a jurisdiction. Building and accessibility requirements must be confirmed with the authorities and professionals responsible for the project.
