The biggest gains often come from practical changes: separate cats from dogs, provide hiding or perching choices, keep carriers off the floor, reduce noise and strong scents, and give staff time to adapt handling to each cat. A welcoming clinic also prepares caregivers, explains options clearly, and treats them as partners in their cat’s care.
Design the visit around what a cat experiences
Leaving familiar territory, encountering unfamiliar animals and smells, and being handled can all feel threatening to a cat. Fear may make examination harder and can affect the safety of patients and staff. The 2022 ISFM/AAFP environment guidelines treat the clinic visit as a connected environmental and interaction challenge—not something that begins only when the cat reaches the examination table. There is no single room layout or handling sequence that suits every cat. The useful design principle is to offer safety, choice, and predictability, then respond to the individual patient’s behavior and clinical needs.
Prepare caregivers and cats before arrival
Help caregivers make the carrier a familiar part of home life instead of bringing it out only on appointment day. Share clear instructions about choosing a secure, suitable carrier, bringing familiar bedding or a favored treat when appropriate, and what to expect on arrival. Caregiver preparation and the home-to-clinic journey are part of a cat-friendly visit, not separate from it.

- Explain how to transport the cat and where to go when they arrive.
- Suggest familiar bedding or a favored treat when appropriate for that cat and visit.
- Where suitable, a carrier with a removable top can make it easier to let a cat remain in a familiar space during examination.
For clinic textiles, use washable towels or blankets and clean them appropriately between patients.
Make reception and waiting less stressful
Separate cats from dogs
A cat-only waiting room is the clearest option where space allows. If it is not feasible, create a cat-only area with visual barriers or schedule routine cat appointments during cat-only periods. Avoid direct contact and sightlines between dogs and cats. A cover over a carrier may help a cat feel secure, but it does not replace a barrier between the cat and dogs.

Give carriers a stable place off the floor
Provide a stable shelf, table, or chair for carriers. Keeping a carrier elevated can give the cat more security and reduce exposure to activity at floor level. Use a clean towel or blanket as a visual screen when appropriate, without blocking ventilation.
Control noise, odors, and movement
Keep the waiting area calm and as spacious as the clinic permits. Reduce avoidable noise from ringing phones and alerts; ask staff to use calm voices and movements. Avoid perfumes and air fresheners, and clean away patient odors with minimally scented products. Synthetic feline pheromone products are an optional adjunct described in professional guidance—not a replacement for separation, hygiene, or behavior-aware handling.

Set up examination rooms for choice and efficient care
A room reserved for cats can reduce exposure to other species. Where that is not possible, cat-only appointment periods and thorough cleaning between patients are alternatives. Keep necessary supplies ready so the team can avoid unnecessary interruptions once the cat is in the room.
- Use cleanable, non-slip surfaces and provide a clean towel or blanket for each patient.
- Allow the cat to remain in the carrier, use a lap, or choose another safe surface when clinically appropriate.
- Offer a place to hide or perch, while recognizing that some cats prefer to remain visible.
- Allow time to acclimate when feasible and adapt the examination to the cat’s response.
These options are not a universal protocol: choose the approach that fits the patient’s behavior and the clinical task.

Reduce stress in treatment and hospitalization areas
Arrange cages to limit sightlines between cats, and consider the effects of cage doors, equipment, and neighboring patients on noise and visual exposure. Provide resting, hiding, and perching opportunities using cleanable materials. Raising cages above floor level may help, but placement must also account for staff access and the patient’s risk of falling. A cat that is distressed by confinement may need a different suitable housing plan.

Reception staff, veterinary nurses, clinicians, and caregivers all influence the visit. Train the team to recognize feline fear and other behavior signals, move calmly, and tailor interactions rather than relying on one handling routine. Tell caregivers what to expect, explain what the team is doing, and discuss alternatives if the cat is not coping. AAHA quotes veterinarian Sarah Hilliard describing the caregiver as the expert on their own cat and the veterinarian as the medical expert. That partnership also means discussing investigation and treatment options, likely costs, and consent before proceeding. Prioritize improvements by feasibility
Start with workflow changes that do not depend on renovation, then assess whether physical changes are practical for the clinic.
- Adjust appointment timing and flow. Offer cat-only periods where possible and move cats from arrival to a room promptly when one is available.
- Provide elevated carrier surfaces and clean textiles. Make stable carrier spots and clean towels or blankets easy to access.
- Reduce avoidable stimulation. Review noise, scents, movement, and visual contact with other animals in reception and clinical spaces.
- Coach staff in feline behavior and handling. Build time for acclimation into the workflow when feasible and adapt to the cat.
- Evaluate larger layout changes. Consider room zoning, sound control, dedicated cat rooms, and hospitalization arrangements based on space, patient needs, and staff access.
Use the clinic’s own records to monitor practical signals such as caregiver feedback, visit completion, handling escalation, and staff safety. These measures can help identify where workflow or environment needs attention; they do not establish that a particular design change will produce a specific business result.
What the available practice survey can—and cannot—show
AAHA reports that, in FelineVMA’s 2024 survey of 903 designated Cat Friendly Practices, 77% said their revenue had increased due to being a CFP, and 87% reported positive client feedback. The 2022 ISFM/AAFP environment guidelines combine literature review with task-force experience in areas where evidence is limited. Their practical recommendations should be understood as professional guidance, not as findings from controlled trials unless a specific controlled result is cited.
