If a fishhook is embedded in your hand—especially if the barb is under the skin—don’t yank it out. A hand has tendons, joints, nerves and blood vessels close together, so a deep hook, a hook near one of those structures, or any hook you’re unsure about should be left in place and assessed promptly by a medical professional. Go to emergency care for a hook in or near the eye.

Decide whether it is safe to try removing the hook

Do not pull a barbed hook backward through skin: the barb can catch and cause more damage. For a hook embedded in the hand, favor prompt medical assessment if the barb entered the skin or you cannot tell how deep it is. MedlinePlus notes that a clinician can remove a fishhook using local anesthetic. HealthyChildren.org recommends emergency evaluation for barbed objects such as fishhooks.

  • Seek urgent medical care: the hook is deep, near a joint or tendon, may involve a nerve or blood vessel, is very painful, resists movement, or you are uncertain about its depth or location.
  • Do not attempt home removal: the hook is in or near a joint, tendon, artery, eye or eyelid, or removal could injure a nerve or blood vessel. MedlinePlus gives this warning in its fishhook-removal guidance.
  • Eye injury: treat a hook in or near the eye as an emergency. Do not press on it; protect the eye without touching the hook and go to emergency care.
  • Point only: if only the point entered and the barb never went into the skin, MedlinePlus says the point may be pulled back out in the direction it entered. This is different from a barb embedded beneath the skin.

Public guidance differs on barbed hooks: MedlinePlus describes a limited method for superficial hooks, while HealthyChildren.org recommends emergency evaluation for barbed fishhooks. Because a hand injury can involve important structures and depth may be hard to judge, arrange prompt professional assessment when a barb is embedded or the situation is unclear.

When a superficial hook may be removed with line

MedlinePlus and HealthLink BC describe a fish-line method for a hook that is only superficially embedded just beneath the skin. It is not appropriate for a deep injury or a hook near a joint, tendon, nerve, blood vessel or eye. If the hook is embedded in your hand, use this method only when the injury is clearly superficial, away from those structures, and you are confident it can come out easily. Stop if it hurts substantially, meets resistance or does not come out promptly; seek medical care instead.

  1. Wash your hands and the skin around the hook.
  2. Pass a loop of sturdy line through the bend of the hook.
  3. Hold the hook shaft and press it slightly down and inward to disengage the barb.
  4. Keep that pressure on the shaft and give the line one quick pull directly backward, in line with the shaft.

Do not improvise other extraction techniques on a hand injury. The American Academy of Family Physicians notes that fishhook-removal technique depends on the hook’s location and anatomy; its clinical procedures are not do-it-yourself instructions.

Clean and protect the puncture after removal

  1. Wash the wound thoroughly with soap and water.
  2. Cover it with a loose sterile dressing. Do not close the puncture with tape.
  3. Arrange medical care if you think any part of the hook or other material remains, or if symptoms worsen.

Watch for increasing redness, swelling, pain or drainage. Get medical attention for warmth, throbbing pain, fever, red streaks or pus-like drainage—warning signs listed in MedlinePlus guidance on cuts and puncture wounds.

Ask about tetanus protection

A fishhook puncture can warrant a tetanus review. Contact a healthcare provider if your vaccination is not current or you are unsure of your history. The CDC says puncture or deep wounds call for a provider discussion; whether vaccination or tetanus immune globulin is appropriate depends on the wound and your immunization history. Follow current local clinical guidance rather than relying on one booster interval for everyone. See the CDC’s tetanus guidance.

Prevent another hook injury

Handle hooks carefully and keep electrician’s pliers with a wire-cutting blade in your tackle box, as MedlinePlus recommends for prevention. Pliers are not a reason to cut or pull a deeply embedded hook from your hand; leave that to a clinician.
