How to Handle Dog Urine Leakage: Causes, Emergencies, Treatment, and Home Care

Dog urine leakage is a symptom with several possible causes, so new leakage needs veterinary assessment. Straining with no urine or only a few drops is an emergency; meanwhile, keep your dog clean and dry and do not restrict water.

Dog urine leakage is involuntary loss of urine, not a diagnosis. Wet bedding or dribbling while resting can be a sign of incontinence, but infection, stones, obstruction, congenital anatomy, increased urine production, and neurologic problems can look similar. Arrange a veterinary assessment for new leakage; go to an emergency clinic if your dog repeatedly strains but passes no urine or only a few drops.

Do not punish your dog or restrict water. While arranging care, keep urine off the skin, use absorbent bedding, and offer more frequent opportunities to urinate.

First, decide whether this is leakage or a urination problem

True urinary incontinence is unconscious loss of urine during storage. A dog may leak while asleep, resting, walking, or changing position without deliberately squatting or appearing aware. Frequent urination, straining, pain, or an overfilled bladder that leaks may indicate a different urinary problem. These patterns overlap, so appearance alone cannot identify the cause.

What you notice What it may suggest
Wet bedding or a wet spot where the dog was sleeping; the dog seems unaware Storage incontinence, including possible urethral sphincter weakness
Frequent small urinations, straining, pain, blood, or strong-smelling urine Infection, inflammation, stones, or obstruction
Large urine volumes or a sudden change in thirst A systemic illness or another cause of increased urine production
Leakage from puppyhood or early life A congenital urinary abnormality, such as an ectopic ureter, among other possibilities
Leakage with weakness, altered gait, back pain, or other neurologic changes A possible neurologic or bladder-emptying problem

These are clues, not diagnoses. Contact a veterinarian about new leakage, blood, painful urination, repeated urgency, or a sudden change in drinking or urine volume.

When dog urine leakage is an emergency

Seek emergency veterinary care immediately if your dog repeatedly strains but produces no urine or only a few drops, especially if urination is painful. A blockage can prevent the bladder from emptying and needs prompt care. Go urgently as well if your dog cannot urinate or is severely unwell, weak, or collapsing.

Common causes of urine leakage in dogs

Urethral sphincter weakness

Urethral sphincter mechanism incompetence (USMI) means the urethra does not stay closed well enough to store urine. It may cause leakage when a dog is relaxed, lying down, or asleep. It is seen particularly in spayed female dogs, but that association does not establish the cause in an individual dog. UC Davis describes acquired USMI as a diagnosis of exclusion, meaning other causes need consideration.

Infection or inflammation

A urinary infection or bladder inflammation may cause urgency, frequent small urinations, accidents, straining, odor, or blood. A dog with these signs should be assessed; infection can also coexist with incontinence. Urine testing helps the veterinarian identify bacteria and choose appropriate treatment.

Stones, debris, and other urinary tract problems

Stones, crystals or debris, prostate disease, trauma, cancer, and other urinary tract problems can cause abnormal urination. A stone may lodge in the urethra and prevent urination, which is an emergency. A dog that strains or passes only drops should not be assumed to have uncomplicated incontinence.

Ectopic ureters and other congenital anatomy

An ectopic ureter is a congenital abnormality in which a ureter enters the urinary tract beyond its normal bladder location. Leakage from puppyhood or early life makes congenital anatomy worth investigating, but it does not prove an ectopic ureter. Veterinary testing may include imaging or cystoscopy. Surgical or selected minimally invasive treatment may be possible, but correcting the anatomy may not fully stop leakage if another problem, such as USMI, is also present.

Increased urine production

Some systemic illnesses or medications can increase urine production and overwhelm normal bladder storage. Tell the veterinarian if your dog is drinking more or producing larger amounts of urine. Do not restrict water to reduce accidents.

Neurologic or bladder-emptying problems

Spinal cord or other neurologic problems can affect bladder storage or emptying. Leakage accompanied by weakness, an altered gait, back pain, or other neurologic changes deserves veterinary evaluation rather than an assumption of USMI.

What to record before the veterinary appointment

A brief diary can help the veterinarian understand the pattern. Record:

  • Your dog’s age, sex, breed, and spay or neuter status.
  • When leakage began and whether it is constant, intermittent, or changing.
  • Whether it occurs during sleep, rest, walking, or after urination.
  • Whether your dog seems aware of the leakage.
  • Approximate frequency and amount of leakage.
  • Whether the stream looks normal or your dog strains.
  • Blood, odor, pain, repeated attempts to urinate, or other changes.
  • Any change in water intake or urine volume.
  • Current medications and other signs of illness.

If practical, take a video of your dog urinating and note when leakage occurs. Do not delay emergency care to gather information if your dog cannot pass urine.

What the veterinarian may do

History and examination

The veterinarian will ask about the timing and pattern of leakage, awareness, ability to empty the bladder, medications, thirst, and other signs. A physical and, when indicated, neurologic examination helps guide the workup.

Urine tests

Urinalysis and urine culture may help detect infection or other urinary changes. Testing is important because infection and incontinence can occur together. The veterinarian will determine whether treatment is needed and select it based on the findings.

Blood tests and imaging

Blood tests may be appropriate when the history or examination suggests systemic illness or increased urine production. Radiographs or ultrasound can help investigate stones and other urinary tract problems. For suspected ectopic ureters or unclear, persistent problems, the veterinarian may consider CT, contrast imaging, cystoscopy, or other advanced tests. The appropriate tests depend on the individual dog.

How dog urine leakage is treated

Treatment depends on the diagnosis and whether the problem is one of urine storage, emptying, or increased urine production. The 2024 ACVIM consensus recommends an alpha-agonist such as phenylpropanolamine or an estrogen compound as initial treatment for female dogs with USMI, and an alpha-agonist for male dogs with USMI. These are veterinarian-directed medicines; the choice, formulation, and monitoring depend on the dog. The consensus authors note that large prospective veterinary clinical trials comparing treatment outcomes are lacking, so guidance is based on available evidence and expert consensus rather than a guarantee of cure.

Infection requires veterinary-directed treatment. Stones may need medical or surgical management. Ectopic ureters may be treated with surgery or selected minimally invasive approaches. Neurologic problems require evaluation of the underlying cause. Medication or a procedure that helps one diagnosis may not help another, so persistent leakage warrants reassessment rather than owner-directed dose changes.

Home care while you are waiting for diagnosis or treatment

Keep your dog clean and dry

Gently clean urine-soiled fur and skin and pat the area dry. Check for redness or irritation; urine left on skin can cause irritation or scalding. Long-haired dogs may benefit from safely trimmed hindquarter fur. Have sore or worsening skin checked by a veterinarian.

Protect bedding and offer more toilet breaks

  • Use absorbent towels, washable covers, or pads on the dog’s bed.
  • Replace wet bedding so your dog is not left on damp material.
  • Offer more frequent opportunities to urinate outdoors.
  • Consider washable bedding or absorbent pads that are easy to clean and replace.

Use diapers or belly bands as containment only

A washable diaper or belly band may help contain leakage if your dog tolerates it. Check and change it so your dog is not left in wet material, and inspect the skin regularly. These products do not treat the underlying cause.

Do not punish or restrict water

Leakage is involuntary, so punishment will not solve it. Clean calmly and focus on veterinary assessment. Do not restrict water; a change in thirst or urine volume is useful information for the veterinarian.

  • Does this look like involuntary leakage, frequent urination, or a problem emptying the bladder?
  • Should my dog have urinalysis and a urine culture?
  • Could a stone, congenital abnormality, systemic illness, or neurologic problem explain the signs?
  • Which tests are appropriate for my dog, and what will they help rule in or out?
  • If treatment is recommended, what should I monitor and when should we reassess?

FAQ

Is urine leakage after spaying normal?

It is a recognized association, particularly because spayed female dogs are overrepresented among dogs with USMI, but new leakage should not be dismissed as inevitable or normal. Other causes need consideration.

Can a male dog be incontinent?

Yes. Male dogs can have USMI, but a veterinarian should assess the cause of leakage and consider other urinary or neurologic problems.

Can a UTI cause urine leakage?

Infection or bladder inflammation can cause urgency, frequent small urinations, accidents, straining, odor, or blood. It can also coexist with incontinence, so urine testing may be important.

Should I limit my dog’s water?

No. Do not restrict water to manage accidents. Tell the veterinarian about increased thirst or larger urine volumes.

Are dog diapers safe?
