Helping an underweight dog gain weight safely starts with finding out why the dog is thin or losing weight—not simply adding larger portions. A dog may need more calories, but unexplained weight loss can also signal parasites, dental or swallowing problems, digestive disease, diabetes, Addison’s disease, organ disease, infection, or cancer. Arrange a veterinary assessment if your dog is losing weight without an obvious explanation, has clearly visible ribs, or shows other signs of illness.

Your veterinarian can assess body and muscle condition, review the current diet and symptoms, rule out disease with appropriate tests, and set a feeding and monitoring plan for your dog’s age, activity, life stage, and health.

Why a dog’s weight loss needs more than extra food

Weight loss occurs when a dog uses more energy than it absorbs, but a calorie deficit is not the only possibility. A dog may be eating less, unable to chew or swallow normally, failing to digest or absorb nutrients, or losing calories because of illness. Behavioral changes, parasites, and changes in activity can also affect body weight.

VCA’s guidance on abnormal weight loss recommends starting with a detailed history and physical examination rather than treating the scale number in isolation. The veterinarian will consider appetite, the amount and type of food offered, activity, thirst and urination, vomiting, diarrhea, stool changes, and any difficulty chewing or swallowing. Common initial tests can include a complete blood count, serum biochemistry, urinalysis, and parasite testing; further tests depend on what the examination and first results show. See VCA’s overview of abnormal weight loss in dogs and its testing guide.

Do not assume that every slim dog is unhealthy. Some dogs are naturally lean, and body condition is more informative than a single weigh-in. Merck also estimates that 59% of dogs and 61% of cats in the United States were overweight or obese in 2022; that statistic describes excess weight, not the prevalence of underweight dogs, and it is not a reason to make a dog gain weight without an individual assessment.

Tip 1: Confirm that your dog is genuinely underweight

Use your hands and your dog’s outline as prompts for a veterinary conversation. The number on the scale should be interpreted alongside body condition and muscle condition.

What you observe What it may indicate
Ribs are easy to feel beneath a slight layer of fat, the waist is proportionate when viewed from above, and the abdomen tucks up when viewed from the side. These are features Merck describes for an ideal body condition.
Ribs are plainly visible with little or no fat cover, the waist looks sharply narrowed, and the abdominal tuck is severe. These findings are consistent with a very thin dog and warrant professional assessment.

Look at the back, hips, shoulders, and thighs as well as the ribs. Loss of muscle can be important even when body weight has changed little. Conversely, a naturally narrow dog may have a visible outline without being ill. Your veterinarian can distinguish normal build from loss of fat or muscle more reliably than a home comparison with another dog.

  • Check whether the change is recent or whether your dog has always had the same build.
  • Note whether appetite is normal, reduced, unusually hungry, or difficult to assess.
  • Record changes in energy, thirst, urination, stool, vomiting, chewing, or swallowing.

If the dog is losing weight unintentionally, do not wait for a dramatic change before calling. Early assessment can prevent an underlying problem from being masked by extra food.

Tip 2: Investigate unexplained loss before changing the diet

A feeding plan only works when it addresses the reason the dog is underweight. Bring the food packaging or a written diet history to the appointment, including treats, chews, supplements, table food, and how much is actually eaten. Measuring what is offered and what remains can reveal that the dog receives less food than assumed.

Questions your veterinarian may ask

  • What food, treats, and supplements does the dog receive, and in what portions?
  • When did the weight change begin, and was it gradual or sudden?
  • How is appetite changing, and does the dog tire while eating?
  • Are there vomiting, diarrhea, unusual stools, or other digestive changes?
  • Has thirst or urination increased?
  • Has activity, stress, household routine, or access to food changed?
  • Does the dog have pain or difficulty chewing or swallowing?

Tests that may be part of the first work-up

Depending on the history and examination, a veterinarian may recommend a complete blood count, serum biochemistry, urinalysis, and parasite testing. These are common starting tests, not a fixed checklist for every dog. Additional imaging or other testing is selected when the initial findings point toward a particular organ, digestive, endocrine, infectious, or cancer-related problem.

Possible causes include an inadequate or inappropriate diet, parasites, dental or swallowing difficulty, digestive disease, diabetes, Addison’s disease, organ disease, infection, and cancer. Treatment therefore ranges from correcting the diet to managing a diagnosed illness. Increasing portions before testing can delay the right treatment or worsen digestive signs.

Tip 3: Feed a complete diet matched to your dog

Once immediate medical concerns are addressed, use a complete and balanced dog food appropriate for the dog’s life stage and health. “More food” is not automatically “more usable nutrition.” Energy needs differ with activity, age, life stage, lean body mass, and individual health, so ask the veterinary team which food and amount fit your dog.

Choose for life stage and health needs

Decision Why it matters What to do
Life stage Growth, adult maintenance, and other life stages have different nutrient and energy requirements. Use a complete diet labeled for the dog’s current life stage unless your veterinarian directs otherwise.
Puppy status Growing puppies need energy and nutrients for development; VCA cautions that an adult diet alone can leave a young active puppy underweight. Have the veterinary team select a growth-appropriate diet and portion.
Health condition Digestive, endocrine, organ, or other diseases can change which nutrients and energy density are appropriate. Follow the diet recommendation for the diagnosed condition rather than choosing a generic weight-gain formula.
Portion and energy density Individual calorie needs vary, and a label’s general serving guide may not fit every dog. Use the amount and adjustment schedule provided by your veterinarian.

Be cautious with homemade food

A narrow menu such as meat alone or hamburger and rice may provide calories but still be nutritionally inadequate. Merck advises that diets based on a limited number of foods can lack essential nutrients. If you want to feed a home-prepared diet, have it formulated for your dog by a board-certified veterinary nutritionist; do not improvise a recipe from a few ingredients.

Make changes in a controlled way

  1. Write down the current food, measured portion, treats, and how much the dog actually consumes.
  2. Confirm with the veterinarian whether the existing food is complete and balanced and appropriate for life stage and health.
  3. Follow the prescribed food, portion, and meal schedule rather than adding unplanned toppers or supplements.
  4. Report vomiting, diarrhea, refusal to eat, or worsening weight loss promptly instead of continuing to increase food on your own.

No particular brand, supplement, calorie-dense product, or topper is established as suitable for every underweight dog. The right choice depends on the diagnosis, nutrient requirements, and tolerance of the individual animal.

Tip 4: Track progress with your veterinary team

  • Body weight and the date measured.
  • Food offered, portions, treats, and how much was eaten.
  • Appetite, activity, and any difficulty chewing or swallowing.
  • Stool appearance, vomiting, thirst, and urination.
  • Changes in body outline and muscle over the ribs, spine, hips, shoulders, and thighs.

Ask your veterinarian to set a target weight or body-condition goal and a check-in schedule. The available veterinary guidance does not establish one safe weekly gain rate or one calorie increase for every dog. A plan that is appropriate for a growing puppy may not be appropriate for an older dog or for a dog with digestive, endocrine, or organ disease.

At each review, bring the log and discuss whether appetite, stool, energy, weight, and muscle condition are moving in the expected direction. The veterinarian may adjust the food, portion, testing, or treatment. If the dog is not gaining despite following the plan, that is a reason to reassess the diagnosis and actual intake—not simply to keep adding food.

When to arrange veterinary care

Contact a veterinarian before attempting a weight-gain program when weight loss is unexplained, ribs are clearly visible, or the dog has illness signs such as vomiting, diarrhea, changes in thirst or urination, reduced appetite, abnormal stool, or trouble chewing or swallowing. A prompt examination is especially important when the change is continuing. The goal is not merely a heavier dog; it is restoration of healthy fat and muscle while treating whatever caused the loss.

A practical checklist for the appointment

  • Write down when you first noticed the weight change and whether it is continuing.
  • Bring the food label, feeding measurements, treat list, and any supplement information.
  • Bring recent records of vomiting, diarrhea, stool, thirst, urination, appetite, and activity.
  • Ask how body and muscle condition will be assessed, not only the scale weight.
  • Ask which tests are recommended and what findings would lead to additional testing.
  • Get written instructions for food, portion, schedule, target condition, and follow-up.

With the cause identified and progress measured, most weight-gain decisions become specific: the veterinary team can determine whether the dog needs a different complete diet, a carefully adjusted amount, treatment for disease, or a combination of these steps.
