They are observable clues, not a diagnostic checklist. A pig may show one sign, several signs, or very subtle changes, and the same behavior can have causes other than pain.
Watch the pig over more than one brief encounter, compare it with its own usual behavior and, when practical, unaffected pigs in the same pen, and arrange veterinary assessment when a change persists or concerns you. Do not use an online sign list, a consumer device, or medication without case-specific veterinary guidance to decide what is wrong.

What these four signs can—and cannot—tell you
Pain in pigs is often inferred from movement, posture, vocalization, and behavior rather than from one definitive test. A recent review notes that pigs can hide pain and that signs vary with the individual, the condition, and the way the animal is examined. The review authors also write that “Currently, no harmonized nomenclature or categorization of indicators has been established.” Part I of the 2025 Acta Veterinaria Scandinavica review therefore supports using a group of observations in context, not counting signs as if they were points on a validated score.

| Limping or difficulty moving | Uneven steps, carrying a foot, reduced weight bearing, trouble standing, or reluctance to walk | Mobility problems can reflect painful legs, feet, joints, muscles, or other illness; a pig may also struggle to reach feed or water | Lameness has several possible causes and needs examination |
| Unusual posture | Rounded or arched back, low head, unusual sitting or lying, or reluctance to rise | A guarded posture can accompany discomfort | Posture alone does not identify an injury, disease, or pain source |
| Unusual vocalization | Squealing or screaming, especially during movement or handling | High-pitched vocalization is included among pain and fear measures in handling guidance | Noise can also reflect fear, restraint, excitement, or handling stress |
| Change in normal activity or behavior | Less movement, interaction, feeding, drinking, or engagement | A departure from the pig’s baseline can be an early clue of pain or illness | Only a comparison with a reliable baseline gives the change meaning |
1. Limping or difficulty moving
Look for an uneven gait, a pig that favors one side, carries a foot, places less weight on a limb, or stops more often than usual. Difficulty rising, standing stiffly, or reluctance to cross the pen also belongs in this category. Reviews of pig pain assessment identify locomotor changes and lameness as important observable indicators, and the World Organisation for Animal Health’s 2024 pig-welfare chapter lists leg abnormalities, abnormal posture, and lameness among welfare-relevant observations.
Mobility is a practical welfare issue as well as a clue. A pig that cannot move comfortably may have difficulty reaching a feeder or drinker, avoiding competition, or lying down and rising safely. Note whether the problem affects one limb, appears after rest, worsens on turns, or follows an obvious injury. Those details help a veterinarian, but they do not establish the cause at home.
What to check without forcing movement
- Watch the pig walk at its own pace on the normal pen surface.
- Observe weight bearing on all four limbs rather than pulling, lifting, or making it run.
- Check whether it can reach feed and water and settle into a normal resting position.
- Record when you first noticed the change and whether it is becoming more pronounced.
2. An unusual posture
A pig in discomfort may stand with a rounded or arched back, hold its head unusually low, sit or lie in an atypical position, or remain down when it would normally rise. Reluctance to get up when the group moves is another observation worth recording. Abnormal posture is recognized as a potential welfare indicator by the WOAH chapter and is discussed in the 2025 review of pain associated with naturally occurring disease and injury.

Posture is highly context dependent. A pig may crouch after exertion, rest in an unusual position because of heat or crowding, or change its stance for reasons unrelated to pain. Treat a guarded posture as a prompt to look at movement, appetite, social behavior, and other changes, not as proof of a particular injury or disease.
3. Unusual vocalization
Squealing, screaming, or a sudden increase in high-pitched calls can occur when a pig is hurt, particularly during movement or handling. The European Food Safety Authority’s 2020 opinion on pigs at slaughter includes high-pitched vocalization among measures used when assessing pain and fear during handling, alongside injuries, lameness, and escape attempts.
Sound is not specific to pain. Pigs can vocalize energetically when frightened, restrained, separated, excited, or responding to a handler. The 2025 review emphasizes that pigs are challenging to examine because most, except minipigs, “vocalize energetically and readily.” Listen for the setting: Does the sound begin when the pig bears weight, is touched at a particular area, or is moved? Does it continue after the stressful event ends? Pair the answer with physical and behavioral observations before drawing conclusions.

4. A change in normal activity or behavior
A pig that becomes noticeably less active, stops interacting with pen mates, withdraws, or shows less interest in feeding and drinking may be experiencing pain or another illness. Conversely, unusual restlessness, repeated shifting, or difficulty settling can also signal discomfort. The key is a meaningful departure from that individual pig’s ordinary pattern.
Use a baseline whenever possible. Think about the pig’s normal times for eating, drinking, exploring, sleeping, and social contact. Compare it with unaffected pen mates only when they are kept under similar conditions; differences in age, status, temperature, housing, or recent handling can otherwise mislead you. The review of spontaneously occurring disease and injury in pigs recommends unaffected animals in the same group or pen as a reference where feasible, while warning that subtle changes may still be missed.
How to observe a pig systematically
A calm, repeatable observation is more informative than a single glance. Avoid provoking the pig to “test” for pain, and do not chase it or manipulate a limb. Use this sequence:

- Observe from outside the pen first. Note breathing effort, posture, position in the group, willingness to rise, and whether the pig can access feed and water without interference.
- Watch ordinary movement. Let the pig walk naturally. Look for uneven steps, reduced weight bearing, stiffness, slipping, or repeated stops.
- Listen in context. Record vocalization during normal activity and distinguish it from sounds that occur only with restraint, fear, or separation.
- Compare with a baseline. Use your own recent observations and, when suitable, a healthy-looking pen mate under the same conditions.
- Write down timing and progression. Record the date, what changed, possible recent events such as a fall or mixing, and whether feed, water, movement, or social contact has changed.
- Contact a veterinarian when the concern persists or is substantial. Share your notes or video if the veterinarian requests it; do not delay care by trying to confirm a diagnosis yourself.
How to interpret several signs together
Combinations and persistence generally carry more meaning than a mechanical count. For example, limping plus reluctance to rise and reduced access to feed is more concerning than a single squeal during an otherwise normal interaction. An arched back together with reduced activity may justify a prompt call, while a brief vocalization during routine handling without any other change may reflect fear or stress.
Look for a coherent change across domains: movement, posture, vocalization, appetite or drinking, and social engagement. Also consider non-pain explanations such as heat, crowding, unfamiliar handling, transport, conflict, or a change in feed or housing. These alternatives do not make the observation unimportant; they explain why professional assessment is needed to identify the cause.

When to seek veterinary assessment
Arrange veterinary advice for persistent, worsening, or clearly abnormal signs, especially when a pig cannot move normally, has difficulty reaching feed or water, repeatedly cries during movement, remains isolated, or shows a marked change in eating or drinking. The available sources do not provide one urgency rule that applies to every cause, age, production system, and location. A veterinarian can examine the pig, assess the group and environment, and decide whether treatment or analgesia is appropriate within a veterinary animal-care framework. The University of Michigan’s swine anesthesia and analgesia guidance illustrates why pain care belongs in that clinical framework.

Seek help sooner rather than waiting for multiple signs if the pig is unable to stand or reach water, has a serious visible injury, is deteriorating rapidly, or appears in severe distress. These are reasons for immediate professional contact, not instructions to administer a particular medicine.
Common observation mistakes
- Counting signs as a score: No universal, validated four-sign consumer checklist was established. One sign can matter, and four signs do not identify a cause by themselves.
- Assuming every squeal means pain: Vocalization is especially sensitive to fear and handling context.
- Waiting for dramatic behavior: Pigs may hide pain, and subtle reductions in movement or feeding can be important.
- Forcing an examination: Chasing, restraining, or repeatedly manipulating a sore area can increase fear and injury risk and can distort what you observe.
- Giving leftover or human medication: Drug choice, dose, withdrawal requirements, and safety depend on the individual pig and the suspected condition; obtain veterinary direction.
- Ignoring access to resources: A pig that cannot comfortably reach feed or water needs attention even if its other signs seem mild.
What the evidence supports—and its limits
The 2025 scoping review screened 725 publications and included 49 publications dated from 2015 through the end of March 2023. That figure describes the review’s screening process; it is not an estimate of how common pain is in pigs. The authors found varied terminology and methods rather than a single harmonized catalog of indicators.
Earlier reviews, including the 2016 Frontiers in Veterinary Science review of pain assessment in pigs, support locomotor and behavioral observations while noting limits to behavioral measures. The 2025 review of naturally occurring disease and injury adds the value of comparing pigs with suitable unaffected pen mates. Together, these sources support careful observation followed by veterinary interpretation, not a home diagnostic product or a treatment decision based on one behavior.
